SUBJECT: Approval of Contract#26-630 with John Witczak,.D.O., P.C.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Jeff Smith, M.D.) to execute on
behalf of the County, Contract 426-630 with John Witczak, D.O., P.C, a professional corporation, in
an amount not to exceed S360,000, to provide professional orthopedic services at Contra Costa
Regional Medical Center and Contra Costa Health Centers, for the period from May 15, 2008
through May 14, 2011.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I and will be offset by a reduction. Cost to the
County depends upon utilization. As appropriate, patients and/or third party payors will be billed for
services.
BACKGROUND/R.EASON(S) FOR RECOMMENDATION(S):
For a number of years the County has contracted with Medical, Dental and Mental Health Specialists
to provide specialized professional services that are not otherwise available in its Hospital and Health
Centers.
Under Contract #26-630, Jolin Witczak, D.O., P.C. will provide orthopedic services at Contra Costa
Regional Medical Center and Contra Costa Health Center including clinic coverage, medical and
surgical procedures, consultation and training, for the period from May 15, 2008 through May 14,
2011.
