LANGO, DIRECTOR OF GENERAL SERVICES °' COSTA DATE: JUNE 10, 2008 COUNTY STA CpUN'� SUBJECT: FIRST AMENDMENT TO LEASE AGREEMENT FOR 215 PACIFICA AVENUE, BAY POINT FOR HEALTH SERVICES DEPARTMENT (T00521) (CP# 07-63) SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION CO//(A 1. APPROVE a First Amendment to Lease Agreement with Mt. Diablo Unified School District for a two (2) year lease extension and continued occupancy, for the premises at 215 Pacifica Avenue, Bay Point under the terms and conditions more particularly set forth in the First Amendment to Lease. 2. AUTHORIZE the Director of General Services, or designee, to EXECUTE the Amendment on behalf of the County. 3. DETERMINE that the project is a Class 1a Section 15301 Categorical Exemption under the California Environmental Quality Act (CEQA). 4. DIRECT the Director of Conservation and Development Department, or designee, to file a Notice of Exemption with the County Clerk, and DIRECT the Director of General Services, or designee, to arrange for the payment of the handling fees to the Conservation and Development Department and County Clerk for filing of the Notice of Exemption. FINANCIAL IMPACT Annual rent for the leased space is $1.00 per year. Health Services plans to remodel the Bay Point Community Wellness Center through grants it has attained and funds budgeted this fiscal year. The remodeling project is estimated to be approximately $400,000. BACKGROUND The Health Services Department has leased and occupied approximately 3,644 square feet at 215 Pacifica Avenue in Bay Point since May 1, 1996, for use as a Wellness Center. The lease was to expire September 30, 2011. This Board action will extend the expiration date to enable the County to remodel the premises to expand dental care services for the community.
