SUBJECT: Approval of Grant Agreement #28-786 with La Clinica de La Raza, Inc.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.) to execute
on behalf of the County, Grant Agreement #28-786 with La Clinica de La Raza, Inc., to pay County an
amount not to exceed $47,385, for the Patient Navigator Program, for the period from September 1, 2007
through August 31, 2010, including agreeing to hold the Grantor harmless for claims arising out of the
County's performance under the Agreement.
FISCAL IMPACT'
Approval of this agreement will result in an amount not to exceed $47,385 of Federal Health and Human
Services funding, from La Clinica de La Raza, Inc., on behalf of the Spanish Speaking Women's Cancer
Initiative ("SSWCI"), for the Patient Navigator Program. No County match required.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
Low income, underinsured clients often do not get diagnosed in a timely fashion or follow treatment
modalities as prescribed. This funding allows clients with cervical cancer to receive Patient Navigator
services. Working in collaboration with the Hospital and clinic systems, especially the Cancer Center,
services that have been offered through the Patient Navigator Program for breast cancer patients will now
be replicated for cervical cancer patients. The agencies of the SSWCI (American Cancer Society, La
Clinica de La Raza, Women's Cancer Resource Center, The Wellness Community, and Anna's Program of
East Bay Hospice) have worked for many years to develop of continuum of care.for cancer patients, which
prevent duplication and enhances coordination. Since this project replicates what is currently being
delivered to breast cancer clients, it is anticipated that clients with cervical cancer will have the same
positive outcomes as those who have had breast cancer.
Approval of Grant Agreement will expand the Patient Navigator Program to include cervical cancer
patients, through August 31, 2010.
Three sealed/certified copies of this Board Order should be returned to the Contracts
and Grants Unit.
