BACKGROUND: On April 16, 2007, the Family and Human Services Committee received an updated Child Welfare System Improvement Plan (SIP) for 2007-2010. Valerie Earley, Director of the Children and Family Services Bureau of . the Employment and Human Services Department, provided an introduction to the report and a brief overview of the key Child Welfare outcomes that the County will focus on in the next three years. County Progress Since 2004: The County has made significant progress in addressing the focus areas identified in the August 2004 Self-Assessment. For example: • Recurrence of abuse in the home declined from 13.3 percent in 2004 to 7.8 percent in 2005. • The percent of children in foster care receiving their required monthly social worker visit increased from 78 percent in 2003 to 91 percent in 2005. • The incidence rate for first entries to Foster Care for African-American children decreased from 8.69/1000 in 1998 to 5.95/1000 in 2005. The Children and Family Services Bureau continues to meet federal and state performance improvement expectations and goals. The Bureau has been able to continue its redesign and improvement efforts by using funding from a variety of sources: State Redesign Pilot funds, Federal Promoting Safe and Stable Families, Federal System of Care, and grants from the Stuart Foundation and Hedge Funds Care. 2007 Updated SIP: The updated SIP will continue to focus on Safety, Permanency and Well-Being outcomes, as well as target areas in systemic factors, including Disproportionality and Foster Home Recruitment and Retention. While these systemic areas are not new, they are included in their own area to further highlight the issues identified in the Self Assessment. The outcomes addressed in this SIP are: Safety: a) Maintain timely response compliance on 10-day referrals at 90% or above. b) Maintain social worker visits compliance at 90% or above. c) Decrease child abuse/ neglect in foster care by 50% to .25%. d) Continue to decrease the re-occurrence of maltreatment of children investigated by Children and Family Services. Systemic Issue: e) Reduce the rate of first time removals for African American youth by 15%. Permanency: f) Increase the number of children with no more than two placements. g) Increase children who reunify within 12 months to 48%. SUBJECT: CHILD WELFARE UPDATED SYSTEM IMPROVEMENT PLAN h) Increase permanency for children/youth who are in foster care for 24 months to 15%. Systemic Issue: i) Increase the number of available foster homes for youth which will allow them to be placed within their own community. Well-Being: j) Increase the number of youth graduating from High School or completing a GED. k) Increase the percentage of siblings placed together with all or some of their siblings. Background on Outcome-Based Reform Efforts: Over the past few years, major local, state, and federal efforts have been underway to improve the child welfare system. These efforts share a focus on outcomes, including safety, permanence, and well-being. • County Child Welfare Redesign: Contra Costa Children and Family Services Bureau began a formal process in 2001 to "redesign" many Child Welfare practices with the goal of improving outcomes for children. This effort leveraged additional state funds as a child welfare redesign pilot county, and also allowed the County to become one of seven national sites for a Child Welfare System of Care grant. • Federal Adoption and Safe Families Act of 1997: This Act required all states to address specific child safety, permanence and well-being outcomes and systemic factors, in order to receive federal Child Welfare funding. • State Child Welfare System Improvement and Accountability Act of 2001 (AB 636): This Act required counties to implement a process of ongoing self-assessment and system improvement, to improve performance on key child welfare outcome indicators. The specific review processes include: 1) Peer Quality Case Review(PQCR): In Contra Costa County, this is a collaborative process among the Children and Family Services Bureau, Juvenile Probation, the Bay Area Academy(a regional professional development program for child welfare services), and the state Department of Social Services. The County's March 2006 PQCR focused on "Length of Time to Exit Foster Care to Reunification." 2) County Self Assessment: Each county must conduct an extensive self-assessment using data analysis and stakeholder discussion groups to learn what is and is not working in child welfare services in that county. Discussions are framed around specific outcome indicators, including seven federal systemic factors. Contra Costa County conducted a Self-Assessment in August 2004, and again in August 2006. 3) System Improvement Plan (SIP): The SIP is an action plan for program priorities developed as a result of the Self Assessment and PQCR process. Each SIP component includes an area for improvement, an improvement goal, strategies, rationales, and milestones. Contra Costa County submitted its first SIP to the state in September 2004. An updated SIP was submitted in September 2005. The most recently updated SIP was prepared in March 2007, for 2007-2010. ADDENDUM to D.1 May 8, 2007 On this day, the Board of Supervisors considered accepting a report from the Employment and Human Services Department(EHSD), Children and Family Services (CFS)Bureau, concerning the Child Welfare System Improvement Plan. Joe Valentine, Employment and Human Services Director, gave a brief overview of the annual update to the Child Welfare System Improvement Plan and turned the presentation over to Valerie Earley, Director of CFS Bureau of the EHSD. Ms. Earley provided the Board with an outline of the key Child Welfare outcomes the County will focus on in the next three years. [PowerPoint attached] Supervisor Piepho suggested the Board would support EHSD working with Dr. William Walker, Director of Health Services Department, in order to link the County on matters of issuance of pharmaceuticals to children. For lowering the number to .25 percent of abuse in foster homes, Supervisor Bonilla asked the County to continue to reiterate its "zero-tolerance policy"toward any type of abuse in a foster home. She said she appreciates the distinction between abuse and license violation procedures but suggested further addressing why abuse happens in a foster home. She said she would like to see active recruitment for more foster families. Ms. Earley responded, saying EHSD's goal is to continue to actively recruit recruitment foster. Mr. Valentine added that foster parents need much support and said one of their strategies is to focus on the new foster parent. Supervisor Bonilla asked what the percentage was of group homes and private agency placement for the 1600 foster kids and those in County monitored homes. Ms. Earley said she did not have that information available but would provide it later. Supervisor Gioia referred to Section IV Prevention Activities and Strategies of the County Self Assessment Report and noted one of the County's goals is to strengthen families and look at prevention so kids do not have to enter the foster care system. Supervisor Gioia asked what kind of funds are used on prevention. Ms. Early said the funding comes from State General Funds and State Department of Social Services, and said most of the prevention dollars come from IV-B funds. Supervisor Gioia asked how much money is spent on the prevention side. ADDENDUM TO ITEM D.1 May 8, 2007 Page 2 of 3 Ms. Earley said she did not have that information available immediately but would provide the information later. Supervisor Gioia said to make sure the County is receiving the right prevention dollars staff should address the geographical coverage for the "Welcome Home Baby"program for the three main parts of the County where there is the greatest need—from the Monument Corridor, Concord, to parts of Bay Point to Pittsburg, and to parts of Richmond and San Pablo. The Board asked for public comment and the following person spoke: • Charles Evans II, Richmond resident, said information provided is good but the County needs to resolve his personal issues. He asked where does he fit into the CFS system and said the County needs to resolve this for his family. Supervisor Piepho asked Mr. Valentine the outcome of his conversation with Mr.,Evans. Mr. Valentine said he addressed some of Mr. Evans' particular concerns about information that had been filed with the State and said other concerns were outside of EHSD's jurisdiction. Supervisor Piepho pointed out the statistics relate to 2005 and asked when the statistics for 2006 would be out. Ms. Earley said the State updates information based on years behind and said this report is in the required format. Supervisor Piepho asked whether these numbers are continuing to be successful or improving. Ms. Earley said most of the numbers are improving. Supervisor Uilkema asked Mr. Valentine to consider imposing an even higher goal for timely visits by social workers and noted such contacts would provide a"control point"that could prevent tragedies. Mr. Valentine agreed that 100 percent would be the only acceptable goal but said it would require some realignment of priorities because EHSD is operating at the resources given by the state. Supervisor Gioia said he would like to understand the ramifications from the staff before changing any numbers. Mr. Valentine said EHSD would return with a plan but noted the County would face sanctions from the State if the target were not met. He suggested the County could set an operating goal of ADDENDUM TO ITEM D.1 May 8, 2007 Page 3 of 3 100 percent but what is sent to the State needs to be more realistic because of fiscal penalties involved. Supervisor Bonilla suggested ongoing internal discussions toward continued improvement for timely visits by social workers. She said this would enable EHSD to provide them internally in writing each year, and the Board could ask EHSD if the goal had been met. Supervisor Glover proposed referring the subject of a higher percentage of timely visits by social workers to the Family and Human Services Committee. By a unanimous vote with all Supervisors present, the Board of Supervisors took the following action: ACCEPTED an update on Child Welfare System Improvement Plan; SUPPORTED Children and Family Services Bureau's efforts on Child Welfare Redesign and Implementation of the System Improvement Plan required by the State's Child Welfare System Improvement and Accountability Act of 2001; REQUESTED that the Employment and Human Services Department staff provide periodic reports on timely response compliance to the Family and Human Services Committee; and REFERRED this matter to the Family and Human Services Committee for continuing discussions. EMPLOYMENT AND HUMAN SERVICES CONTRA COSTA COUNTY TO: Contra Costa County Board of Supervisors FROM: Joe Valentine, Director Employment and Human Services Department SUBJECT: Child Welfare Redesign System Improvement Plan DATE: April 30, 2007 Attached for your review prior to the May 8, 2007 Board of Supervisors Meeting is our updated "Child Welfare Redesign System Improvement Plan"plus the updated copy of our child welfare "Self Assessment"that the Plan is based on. This report was reviewed at the April 16, 2007 meeting of the Family and Human Services Committee and they recommended that it be referred to the full Board for a presentation and acceptance. As you may recall, the Employment and Human Services Department's Children and Family Services Bureau began a formal process in 2001 to "redesign"its child welfare practices with the goal of improving outcomes for children. This was partially in response to both federal and state mandates to track, report, and improve key measures of child well being. These included the provisions established by the 1997 Federal "Adoptions and Safe Families Act" which required all states to adopt plans that would improve child safety, permanency and well being outcomes in order to continue to receive Federal Child Welfare funding. In 2003, the state of California implemented the"Child Welfare Performance Outcomes and Accountability Act" which set state specific performance goals for California child welfare departments and required self-assessments, "System Improvement Plans", and the collection and reporting of specific outcome measures. However, Contra Costa County's commitment to improving its child welfare practices went beyond just meeting federal and state standards. The county was seeking to improve its entire "system"of services for children and families - an effort which it used to leverage additional funds as a"child welfare redesign"pilot county as well as one of 7 national sites for a"Child Welfare System of Care" grant. More recently, the Employment and Human Services conducted an intensive review of two highly publicized child fatalities of children who had been involved with the child welfare system. These reviews pointed out the need to reach out and involve other community systems and institutions that come into contact with families and children in specific strategies to protect the safety and well-being of children. The 2007 update to our System Improvement Plan—which also includes an appendix defining the key terms used -not only reports on and updates our strategies to improve child welfare outcomes, it also incorporates some of the strategies and lessons learned from our recent child fatality reviews. The other attached report—the December 2006 "County Self-Assessment" -provides a comprehensive look at the data that forms the basis for our updated strategies. It shows that we've been successful in making progress in many areas such as making a timely response to referrals of child abuse and neglect or reducing the disproportionate removal of African American children from their own homes. However, it also points out areas that we need to continue to improve such as protecting the safety of children in foster care, or keeping siblings together when they are placed out of the home.
