SUBJECT: Cancellation Agreement with Kami D. Silay
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to
execute a cancellation agreement with Kami D. Silay related to the provision of Consultation
Services to Childcare Accounting Reporting and Enrollment Services (CARES), effective at the
close of business April 27, 2007.
FISCAL IMPACT:
No financial impact. Contract payment limit was 30,640.
BACKGROUND:
Since November, 2005, Ms. Silay provided consultation services for the implementation of the
Childcare Accounting Reporting and Enrollment Services (CARES) au ated system. Ms.
Silay has accepted a permanent position in the Department as an EI' i ity W rker I. She

SUBJECT: Cancellation Agreement with Kami D. Silay
reports to the Eligibility Worker Training on April 30, 2007, and has requested a cancellation of
her contract effective April 17, 2007.
Ms. Silay assisted with the implementation and user support to assure CARES system populated
forms and generated reports in compliance with the California Department of Education and
California Department of Social Services regulations. Her work has contributed significantly to
the successful implementation of the Childcare Accounting Reporting and Enrollment Services
(CARES) automated system.
