SUBJECT: Approval of Contract Amendment/Extension Agreement#22-401-31
with Pittsburg Pre-School and Community Council, Inc.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION /
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.) to
execute on behalf of the County, Contract Amendment/Extension Agreement #22-401-31 with
Pittsburg Pre-School and Community Council, Inc., a non-profit corporation, effective June 15,
2007, to amend Contract #22-401-29 (as amended by Contract Amendment Agreement
#22-401-30), to increase the payment limit by $45,336, from $22,664, to a new total of$68,000,
and extend the term from June 30, 2007 through February 29, 2008.
FISCAL IMPACT:
This Contract is 100% Federally funded, by the State, through the Ryan White CARE Act, Title I,
through an inter-governmental agreement with Alameda County, who is the Grantee of these funds.
No County funds are required.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
In February 2007, The County Administrator approved, and the Purchasing Services Manager
executed Contract #22-401-29 (as amended by Contract Amendment Agreement #22-401-30)
with Pittsburg Pre-School and Community Council, Inc., for the period from March 1, 2007
through June 30, 2007, for the provision of support services for County residents diagnosed with
HIV disease, including case management services, congregate meal services, and non-emergency
transportation.
Approval of Contract Amendment/Extension Agreement #22-401-31 will allow the Contractor to
continue providing services through February 29, 2008.
