BACKGROUND: On June 18, 2002, the Board of Supervisors adopted Resolution No. 2002/377, which requires that each regular and ongoing board, commission, or committee shall annually report to the Board of Supervisors on its activities, accomplishments, membership attendance, required training/certification (if any), and proposed work plan or objectives for the following year, on the second Tuesday in December. Attached is the 2006 Annual Report from the In Home Supportive Services Public Authority Advisory Board, noting the 2006 activities, accomplishments, attendance and training, as well as the proposed work plan for 2007. John Cottrell, (IHSS) In Home Support Services Public Authority Executive Director presented the 2006 Annual Report. . Supervisor Uilkema noted Contra Costa County does not do background checks for care providers and asked the Board to request that In Home Support Services (IHSS)provide a report on the costs associated with background checks. She suggested IHSS work with the County Administrator's office(CAO) and the District Attorney's Office to compare the expenses of the cost of a trial to prosecute a fraudulent care provider versus the costs associated with making preventative criminal background checks. Mr. Cottrell explained the cost could be as high as $8000 a year to the County for 500 registry care providers,but to check all the 6000 care providers the cost would be elevated. Supervisor Gioia asked what efforts have been made to address issues of potential fraud, noting this is a statewide issue and not just an issue in Contra Costa County. Mr. Cottrell responded the County has two Quality Assurance Inspectors, IHSS social workers who approve services, a payroll department who processes time sheets and the Public Authority. Mr. Cottrell said they all meet monthly to go over all the cases and if there are any signs fraud, the Quality Assurance group and the District Attorney's office pursues such cases for fraud investigation. Supervisor Gioia asked what percent of the providers are themselves related to the consumers. Mr. Cottrell responded that 50 to 60 percent are relatives and the other 40 percent are neighbors, friends or family. He explained all care providers go through an orientation. Chair Piepho asked if the orientation provides information on fraud and punishment. Mr. Cottrell said he was not sure. Chair Piepho said it would be helpful to know that information. By a unanimous vote with Supervisor Glover being absent, the Board of Supervisors took the following action: ACCEPTED 2006 Annual Report from(IHSS) Public Authority Advisory Council. Report to Contra Costa County Board of Supervisors Name: Contra Costa County In-Home Supportive Services Public Authority Advisory Committee Meeting: 1:00 PM to 4:00 PM the third Tuesday (of every month) 1340 Arnold Drive, Conference Room Chair: Ethel Dotson Staff: John Cottrell, Executive Director; Fran Smith, Program Manager Report Period: December 1, 2005 —November 30, 2006 I Activities In-Home Supportive Services Provider/Consumer Training: Through recommendations from the Health, Safety and Education Committee, consumer and provider educational and training sessions were conducted in January, March, June, July, August, September, October and November '06. Topics presented include: • How to Recognize and Report Abuse • Community Resources • Employee Wellness • Exercise • CPR • Employee/Employer Training—The Many Faces of Home Care • How to Recognize Stroke • Workplace Violence A total of 529 consumers and providers attended the sessions in East, Central and West County. Rapid Response Program: In late 2005 and`early in 2006, the Advisory Committee initiated a plan to develop a Rapid Response Program for In-Home Supportive Services consumers who unexpectantly find themselves without a provider. On November 1, 2006 the Rapid Response Program was established. Since November 1, 2006, the Rapid Response Program has responded to 53 requests for a substitute provider. The Advisory Committee has organized a Rapid Response Committee that will be responsible for on- going monitoring and evaluation of the program. Every Generation: Participated in the Support Services Work Group for Every Generation. - 1 - Acute and Long Term Care Integration: Continued participation in county activities to establish an Acute and Long Term Care Integration program. II Accomplishments Training for In Home Supportive Services: Advisory Committee members continued to design and actively participate in training to consumers and providers regarding employee/employer relations. The Advisory Committee also considers this training an effective method of community outreach. Newsletter: Published four issues of"Living Independently," the Advisory Committee newsletter which is mailed to over 6,000 consumers and 6,000 providers in the county. III Attendance/Representation State Law, regulations and County Ordinance specify an 11 member Advisory Committee appointed by the Board of Supervisors. No fewer than 50 percent of the members shall be individuals who are current or past users of personal assistant services paid for through public or private funds or recipients of In-Home Supportive Services. In January 2006, there were seven appointed members with the following vacancies: Consumer under 60—Seat 2 Consumer 60 or older— Seat 3 District Appointments—I and II In March 2006,one member resigned: Rudy Barroga(Provider), District V. In April 2006, there was one appointment: Mike Fernandes, Consumer under 60. In November 2006, there was one appointment: Ida Miles, (Provider), District I. At the end of December 2006 there were the following vacancies: Consumer 60 or older— Seat 3 District Appointments—II and V The February and May Advisory Committee meetings had to be cancelled due to a lack of a quorum. The Advisory Committee voted to not meet in November 2006. Recruitment for replacement members was ongoing throughout 2006. Each issue of the Advisory Committee Newsletter, Living Independently, mailed to all 6,000 +IHSS Consumers and 6,000+providers in the county included an appeal for interested persons. Presentations were made at the Central, East and West County Senior Coalitions. Fliers were mailed to every IHSS social worker, Senior Center and Senior Residential program - 2 - in the county. The Advisory Committee also announces any vacancies at each monthly training session for consumers and providers. IV Training/Certification The Public Authority Advisory Committee received training or attended presentations on the following topics: • Medicare Part D • Brown Act • AB 1234 • Legislative Advocacy • Contra Costa Every Generation Plan • Acute and Long Term Care Integration • Statewide Quality Assurance Implementation/Hourly Task Guidelines • Transportation(Coordinated Leadership Conference) • CICA Conference V Proposed Work Plan • Monitor and evaluate the Rapid Response Program • Continue legislative advocacy for In-Home Supportive Services • Research the issue of criminal background checks for In-Home Supportive Services providers • Continue participation in the Long Term Care Integration Program development • Continue to work collaboratively with In-Home Supportive Services and other county departments Prepared by: Ethel Dotson, Chair John Cottrell, Executive Director Fran Smith, Program Manager - 3 -
