SUBJECT: Amend Contract with The Perinatal Council for Cal Learn Case Management services
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION*
APPROVE and AUTHORIZE the Interim Employment and Human Services Director, or designee, to execute a
contract amendment with The Perinatal Council effective June 27, 2006, to increase the per case payment rate
from $163 to a new per case payment rate of$209.58 with no change in the payment limit of$264,060 and with
no change in the term of July 1, 2005 to June 30, 2006 to continue to provide Cal Learn Case Management
services. (Budgeted: No financial impact) (All Districts)

CHILDREN'S IMPACT STATEMENT:
This contract supports all five of the community outcomes established in the Children's Report Card: 1)
"Children Ready for and Succeeding in School"; 2) "Children and Youth Healthy and Preparing for Productive
Adulthood"; 3) "Families that are Economically Self Sufficient"; 4) "Families that are Safe, Stable and
Nurturing"; and 5) "Communities that are Safe and Provide a High Quality of Life for Children and Families,"
by providing assistance to teen parents.
RACKGROUND:
The Cal-Learn Program is designed to provide assistance to pregnant and parenting teens allowing them to
remain in school and obtain a high school diploma or equivalent. The Perinatal Council will provide case
management services to registered Cal-Learn Program participants who are referred by the County's
Employment and Human Services Department(EHSD). Services will include providing continuous
comprehensive case management services,providing program orientation and information, completing
educational assessments, counseling and mentoring,parenting assessments,providing child development skills
training, identifying child care, transportation, and ancillary needs for participant to attend and complete high
school, transitioning participants to independent living upon high school completion, referring participants to
community resources, and reviewing and monitoring school attendance, performance, and progress.
This board order seeks to adjust the per case per year reimbursement rate to reflect the California Department of
Public Social Services announcement that the per case per year rate has increased to $2,515. (21-178-17)
