County SUBJECT: Approval of Grant Agreement#28-751 with the San Francisco Foundation - Z6 SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION RECOMMENDATION(S): Authorize the Health Services Director or his designee(Wendel Brunner,M.D), to execute, on behalf of the County, Grant Agreement #28-751 (C-2006-0668) with the San Francisco Foundation, to pay the County an amount not to exceed $120,000, for the Oral Health Access Project, for the period from April 1, 2006 through March 30, 2009. In accordance with the terms of the Agreement, County shall defend, indemnify and hold the San Francisco Foundation, its officers, directors, trustees, employees and agents, from and against any and all claims, liabilities, losses and expenses (including reasonable attorneys' fee and expenses) directly, indirectly, wholly or partially arising from or in connection with any act or omission of County, its employees or agents. FISCAL IMPACT: Approval of this Agreement will result in $120,000 from the San Francisco Foundation for the County's Oral Health Access Project. No County funds are required. BACKGROUND/REASON(S) FOR RECOMMENDATION(S): The goal of the Oral Health Access Project is to increase access to dental care for low-income Contra Costa County children by developing a centralized system for referrals, expanding the capacity of services of the Ronald McDonald Care Mobile Clinic and recruiting professionals to provide free or low- cost dental services. These grant funds will provide case management services to 240 children, create a database for tracking case management efforts, maintain and update the Denti-Cal referral list, recruit 25 new dentists and specialist to provide services and ensure at least 952 patients visits the Ronald McDonald Mobile Clinic, for the period from April 1, 2006 through March 30, 2009. In accordance with the terms of the Agreement, County shall defend, indemnify and hold the San Francisco Foundation, its officers, directors, trustees, employees and agents,from and against any and all claims, liabilities, losses and expenses (including reasonable attorneys' fee and expenses) directly, indirectly, wholly or partially arising from or in connection with any,act or omission of County, its employees or agents. Three certified and sealed copies of the Board Order should be returned to the Contracts and Grants Unit.
