By: Jacqueline Pigg, Contracts Administrator
DATE: April 12, 2006 J County
SUBJECT: Approval of Standard Agreement#74-208-4 with the State Department of Mental Health
I
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Standard Agreement#74-208-4 with the State Department of Mental Health, a
government agency, in an amount not to exceed $1,046,119, for the purchase and usage of state
hospital beds, for the period from July 1, 2005 through June 30, 2006. The County is agreeing to
indemnify and hold the State harmless for claims arising out of the County's performance under the
Contract.
FISCAL IMPACT:
This Contract is funded 100%by Mental Health Realignment. No cost to County.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
In previous years, the agreement to purchase State hospital beds was a part of the Mandated
County Performance Contract, as required by Section 5650 of the Welfare and Institution Code.
Since Fiscal Year 2002/2003, the purchase of state hospital beds has been separated from the
Performance Contract, making it a stand-alone agreement.
On January 18, 2005, the Bogard of Supervisors approved Standard Agreement #74-208-2 with the
State Department of Mental Health, for the period from July 1, 2004 through June 30, 2005, for the
purchase and usage of state hospital beds.
Approval of Standard Agreement 474-208-4 will allow County to continue to compensate the State
and the State agrees to provide bed usage, including staffing, facilities, equipment, and supplies
through June 30, 2006.
Seven seal/certified copies of the Board Order should be returned to the Contracts and Grants Unit
for submission to the State Department of Mental Health.

SUBJECT: Approval of Contract Amendment Agreement#25-062-1 with Kimberly Baello
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND IUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee(Wendel Brunner, M.D.), to execute
on behalf of the County, Contract Amendment Agreement #25-062-1 with Kimberly Baello, a self-
employed individual, effective April 1, 2006 to amend Contract #25-062, to increase the contract
payment limit by $2,400 from $73,028 to a new total of$75,428 with no change in the original term of
January 1, 2006 through December 31,2007.
FISCAL IMPACT:
Funding for this Amendment is 100% Federal McKinney-Vento Funds, through a Housing and Urban
Development(HUD) Supportive Housing Program Grant.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S)
On January 24, 2006, the Board it f Supervisors approved Contract 425-062 with Kimberly Baello to
provide consultation and technical assistance with regard to the Homeless Management Information
System, for the period from January 1,2006 through December 31,2007.
Due to an administrative oversight, provisions to reimburse Contractor for actual allowable expenses
incurred while traveling on behalf of County was omitted from the initial Contract. It was the intent of
the County that the Contractor beJreimbursed for these expenses,therefore this amendment will increase
the Payment Limit and modify the Payment Provisions to add language to reimburse Contractor for
miscellaneous allowable expenses.
Approval of Contract Amendment Agreement #25-062-1, will allow Contractor to provide additional
services including traveling on behalf of the County,through December 31,2007.
