RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Jeff Smith,M.D.)to execute on
behalf of the County, Contract Amendment Agreement #26-323-11 with CareerStaff Unlimited, a
corporation, effective April 15, 2006, to amend Contract #26-323-10, to increase the Contract
payment limit by $80,000, from $50,000 to a new total of$130,000, with no change in the original
term of January 1, 2006 through December 31,2006.
FISCAL IMPACT:
This Contract is funded 100%by Enterprise I.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):

On December 20, 2005, the Board of Supervisors approved Contract #26-323-10 with CareerStaff
Unlimited, for the period from January 1, 2006 through December 31, 2006, for the provision of
temporary' Occupational, Physical and Speech Therapists, Pharmacists and Pharmacy
Technicians, Medical Social Workers, Ultrasound Technologists and other ancillary
classifications, to cover specified peak loads, temporary absences, or emergency situations at
Contra Costa Regional Medical Center and Contra Costa Health Centers.
Approval of Contract Amendment Agreement #26-323-11 will allow the Contractor to provide
additional hours of services through December 31,2006.

SUBJECT: Request for Re troactive Contract Approval for Mental Health Management, Inc.,
(dba CanyonManor), Contract#74-190
STATEMENT OF NEED FOR RETROACTIVE CONTRACT
Attached is a request to amend the Mental Health Division's contract with Mental Health
Management, Inc. (dba Can}on Manor) to increase the total payment limit by $80,820,00. We
are requesting approval for alretroactive contract because we recently found it necessary to place
a client at this mental health rehabilitation center as admission of this client was denied by every
other facility with which we contract. There are insufficient funds in our current contract to
reimburse Canyon Manor for this client's care.
BENEFIT TO CONTRA COSTA COUNTY
Approval of this retroactive'contract will allow the Mental Health Division to continue to pay
this facility for placement of our client.
AUTHORIZATION OF SERVICES
These services were requested/authorized by the Mental Health Director, pursuant to the
California Welfare and Institutions Code, and under the Bronzon-McCorquodale Act, W&I Code
Section 5608.
