Lecky SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Randall Sawyer) to execute on behalf of the County, Contract Amendment Agreement #22-847-5 with Jayne H. Lecky, a self- employed individual, effective February 10, 2006, to amend Contract #22-847-4, to increase the Contract payment limit by $1,500, from $27,160 to a new total of$28,660, with no change in the original term, from July 1, 2005 flu ough June 30, 2006. FISCAL IMPACT: This Contract is funded 100%by Green Business Program Partners. BACKGROUND/REASON(S) FOR RECOMMENDATION(S): On June 28, 2005, the Board of Supervisors approved Contract#22-847-4 with Jayne H. Lecky, for the period from July 1, 2005 through June 30, 2006, for the provision of consultation and teclulical assistance to the Deparhnent with regard to the Green Business Program to provide assistance to the Hazardous Materials Division in the certification process including conducting site visits of 40 new business in Contra Costa and coordinating inspections and audits needed for certification of new businesses. Approval of Contract Amendment Agrecment #22-847-5 will allow the Contractor to provide additional travel on behalf of the County, through June 30, 2006.
