RECOMMENDATION:
CONSIDER accepting report on the status of Vending Machine Policy implementation.
FISCAL IMPACT:
No direct fiscal impact. Staff costs associated with implementation of the policy and continued
compliance will be borne by departments as part of their cost of operations.
BACKGROUND:
On December 14, 2004, the Board of Supervisors approved a Vending Machine policy for the
County. That policy provided that the General Services Director, the Health Services Director
and the County Administrator create and disseminate healthy and nutritional food and
beverage standards for items served in all County vending machines with a requirement for all
machines to offer 50% products which comply with those standards; and to consolidate all
vending machines on County property under the management and control of the General
Services Department.
Progress of Implementation Plan
The policy was disseminated to Department Heads and to the Superior Court. Information
distributed included standards of healthy vending items, specifics on purchase of nutritious
vending machine products and contact information for General Services Department
Purchasing Division for those departments desiring to participate in a Countywide vending
contract.
A survey was conducted during the period December 2004 through May 2005 in all buildings
throughout the County, resulting in an inventory of vending machines. We have identified 148
snack and beverage vending machines in County buildings. Of these, 44 are in locations
where they serve members of the public.

SUBJECT: VENDING MACHINE POLICY UPDATE
The vending machine inventory was used by the Health Services Department to audit the
machines for compliance with nutrition standards as outlined in the Vending Machine Policy.
The inventory will be maintained by the General Services Purchasing Division for use by the
General Services Department in monitoring compliance with the facility requirements as
delineated in the Vending Machine Policy.
Current Status of Vending Machine compliance with Policy
INFORMATION DISSEMINATION AND EDUCATION: The Healthy Vending Policy has been
institutionalized as Administrative Bulletin 539, "Contra Costa County Vending Machine
Policy." Additionally, the Health Services Department has published the Vending Machine
Policy on their website, along with tips for healthy snacking. Columns have appeared in
several County employee publications, including County Clips (Spring 2005), the Human
Resources Department Employee Wellness Program Newsletter(April 2005). the Contra
Costa Health Services Director's Report(January 2005), and the Health Services Department
NEWS (Spring 2005), which are attached to this Board Order. Vending contractors doing
business with the County recently received individual letters advising them of the Healthy
Vending Policy and requiring compliance with the policy as a condition of continuing to do
business with the County.
HEALTHY SNACKS POLICY COMPLIANCE: The Community Wellness and Prevention
Division of the Public Health Department is funded to support food stamp eligible clients. Due
to budget constraints and personnel shortages, compliance audits are being performed only
on those vending machines that serve members of the public. All machines that serve the
public were audited for compliance with the healthy vending policy. The audit was completed
on June 28, 2005, and results are shown in the attachment "Public Access Vending Machines
with % of Approved Items." Of the 44 machines surveyed, eight were found to have at least
50% healthy items. An additional eight machines are in the 41%-49% range for compliance.
Those machines with at least 50% healthy items are:

COST RECOVERY.- There are currently 20 different vendors that provide vending machine
service to various County departments. County staff are in the process of collecting and
standardizing the contracting process with vending operators, with the objective of ensuring
that the County is receiving commissions that are consistent with the rest of the vending
industry. Vending operators have been asked to submit copies of their current contracts if
they exist. If the vending operator has no contract in place for their vending equipment, they
have been directed to contact County Purchasing so one can be put in place. Staff has also
disseminated information on the Healthy Vending Policy to these vendors. Those vendors
who do not respond or who do not wish to comply with the contracting request or the Healthy
Vending Policy will be told to pick up their disconnected machines after thirty days.

Contra Costa Regional Medical Center is used to new arrivals in its
Labor and Delivery Unit, but there.was an exciting arrival outside .

Darrell Williams, the proud "papa"as manager of the-Diagnostic
Imaging Department. Darrell, who started.with the-department in 1973,
said CCRMC and the health centers have had to refer well over 1,000
patients a year to other providers to get an MRI. By-the end of this
month, Darrell and staff will be doing MRIs on site in the 14-by-60-foot
new building just west of the Diagnostic Imaging Department.

Supervisors A rove Two Health Policies
The Board of Supervisors approved two new health policies last month. One requires all county vending-machines
to include provide healthy choices for at least 50 percent of all the food and beverage selections they offer.The
policy applies to vending machines located-in all county facilities, including courthouses, health centers, CCRMC and
community centers. Supervisor John Gioia sponsored the policy. Melody Steeples,from our Community Wellness&
Prevention Program, helped draft the standards for healthy foods that are included in the policy.They also identified
items that meet the standards and vendors who provide those foods.The second policy allows pharmacies to
sell hypodermic needles to people without a prescription. This measure is intended to help prevent the spread of
Hepatitis C and HIV. The policy applies to pharmacies in the cities and unincorporated areas of the county. Christine
Leivermann, director of our AIDS program, introduced the measure to the Board and provided compelling testimony -
about the relationship between dirty needles and the spread of HIV. -

Historic Donation to CCRMC
The Contra Costa Regional Health Foundation presented a$30,000 check last month to Dr. William Walker at a
Board of Supervisors meeting. Bette Felton, RN, DrPh, chairwoman of the Foundation's Board of Directors, said it
was"the first ever donation by the Foundation,.but will not be the last." Also present was Foundation Board Member
Doug Anderson. The donation was for equipment needs identified by CCRMC/Health Center staff. Executive
Committee members Bette Felton and Steve Roberti and Foundation staff Frank Puglisi are meeting with Drs. Jeff
Smith, Steve Tremain and Sonja Sutherland to get input for the Foundation's current strategic planning process.

End-of-Life Care Training for purses
Nurses spend more time with patients who are facing end-of-life than other health care providers, yet many nurses
feel they need more training to provide the comprehensive care needed by those who are at end-of-life. Our CCRMC _
Education and Training Department and the Hospice and Palliative Care of Contra Costa provided that training to
nurses and health workers on January 6 and 7 to improve end-of-life care.The training program was developed by
the End of Life Nursing Education Consortium (ELNEC)and consists of nine content modules addressing critical
aspects of end-of-life.The training will be offered again on May 19-20 and November 16-17, 2005.
