SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
"COMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD) to
execute on behalf of the County, Contract Amendment Agreement #22-401-26 with Pittsburg Pre-
School and Community Council, Inc., a non-profit corporation, effective May 1, 2005, to amend
Contract #22-401-25, to decrease the Contract payment limit by $13,314, from $122,364 to a new
total of$109,050, with no change in the original term, from March 1, 2005 through February 28,
2006.
FISCAL IMPACT:
This Contract Amendment Agreement reflects a reduction in Federal funds. This contract is funded
100% by Federal funds from the Ryan White CARE Act, Title 1, through an inter-governmental
agreement with Alameda County,who is the Grantee of these funds. No County funds are required.
BACKGROUND/REASON(S)FOR RECOMMENDATIO :
On February 8, 2005, the Board of Supervisors approved Contract #22-401-25 with Pittsburg Pre-
School and Community Council, Inc. for the period from March 1, 2005 through February 28,2006,
top rovide support services for County residents diagnosed with HIV disease, including case
management services,benefits assurance counseling,and non-emergency transportation.
Funding for this Contract was based on anticipated funding from the Ryan White Care Act. The
actual funding received was less than expected, resulting in the necessity to reduce the amount of
services provided by this Contractor. The specific reductions were determined by priorities
established in the Department's federally-mandated community planning process. This reduction
will result in fewer specialty services to be provided to County's clients.
Approval of Contract Amendment Agreement #22-401-26 will allow the Contractor to continue
providing services through February 28, 2006.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee(Wendel Brunner)to execute on
behalf of the County, Contract Amendment Agreement#22-403-23 with Desarrollo Familiar, a non-
profit corporation, effective June 1, 2005, to amend Contract #22-403-22, to increase the Contract
payment limit by$19,662, from $143,513 to a new total of$163,175, with no change in the original
term, from March 1,2005 through February 28, 2006.
FISCAL IMPACT:
This Contract is funded 100% Federally funded, by the State, through the Ryan White CARE Act,
through an intergovernmental agreement with Alameda County, who is the Grantee of these funds.
No County funds are required.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On February 8, 2005, the Board of Supervisors approved Contract #22-403-23 with Desarrollo
Familiar, for the period from March 1, 2005 through February 28, 2006, for the provision of
support services, including Case Management and Mental Health services to West Contra Costa
County residents with HIV disease.
Approval of Contract Amendment Agreement #22-403-23 will allow the Contractor to provide
services to additional clients through February 28, 2006.

execute on behalf of the County, Contract Amendment Agreement #22-491-26 with Diablo Valley

decrease the Contract payment limit by $2,495, from $148,175 to a new total of$145,680, with no
change in the original term, from March 1,2005 through February 28,2006.

100% by Federal funds from the Ryan White CARE Act, Title I, through an inter-governmental
agreement with Alameda County,who is the Grantee of these funds. No County funds are required.

execute on behalf of the County, Contract #22-746-8 with New Connections, a non-profit
corporation, in amount not to exceed$43,200,to provide outreach and referral services to HIV/AIDS
clients in West County,for the period from July 1,2005 through June 30,2006.

This Contract is 100% Federally funded through the Ryan White CARE Act, Title III. No County
funds are required.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On June 225 2004,the Board of Supervisors approved Contract#22-746-6 (as amended by Contract
Amendment Agreement #22-746-7) with New Connections, for the period from June 30, 2004
through June 30, 2005, for the provision of outreach and referral services to HIV/AIDS clients in
West County.
Approval of Contract #22-746-8 will allow the Contractor to provide services to additional County
clients and to continue providing services through June 30,2006.

execute on behalf of the County, Contract #22-874-2 With Nightingale of Contra Costa, LLC, a
Limited Liability Company, in an amount not to exceed $110,000, to provide temporary in-home
health care for AIDS patients,for the period from July 1, 2005 through June 30, 2006.
FISCAL IMPACT:
This Contract is 100%federally funded. No County funds required.
BACKGROUND/REASON(S)FOR RECOM MNDATION(S):
On April 27, 2004, the Board of Supervisors approved Contract #22-874-1 With Nightingale of
Contra Costa, LLC, will provide temporary in-home health care for County-referred AIDS patients
including, but not limited to, intermittent skilled nursing care, attendant care, homemaker services,
nutritional and psychosocial counseling, physical and occupational therapy, and medical adherence
services, for the period from July 1, 2004 through June 30,2005.

Approval of Contract#22-874-2 will allow the Contractor to continue providing temporary in-home
health care for AIDS patients,through June 30,2006.
