Approve and authorize the Health Services Director, or his designee (Jeff Smith, MD), to execute on
behalf of the County, Contract Amendment Agreement #26-446-2 with JLG Medical Transcription
Services, a Corporation, effective June 30, 2005, to amend Contract #26-446 (as amended by
Amendment Agreement #26-446-1), to increase the Payment Limit by $107,000 from $1,620,000 to a
new total Payment Limit of$1,727,000 and extend the term of the contract from June 30, 2005 through
August 31,2005.

REASONS FURRF.0 OMMFNDATIONSlI3ACKGROTIND:
On July 9, 2002, the Board of Supervisors approved Contract #26-446 (as amended by Contract
Amendment Agreement #26-446-1) with JLG Medical Transcription Services, for-the period from July
1, 2002 through June 30, 2005, to provide dictation and medical transcription services, 24-hours per
day, seven days per week,upon request for patients at Contra Costa Regional Medical Center.
Approval of Contract Amendment Agreement#26-446-2 will allow the Contractor to provide additional
services and extend the term of the contract through August 31,2005.

Health Outpatient Psychiatric Unit, for the period from July 1, 2005 through June 30, 2006.

On June 22, 2004, the Board of Supervisors approved Contract #74-185..2 (as amended by
Amendment Agreement #74-185-3) with Windrix Transcription, Inc., to provide expert medical
transcription services necessary, to meet the ADA accommodations for staff with disability, to provide
medical transcription services for designated psychiatrists and therapists working in Psychiatric
Outpatient Services, and to address the problem of illegibility on the part of professionals for which
Contra Costa Mental Health has been criticized in State Department of Health Services, Medi-Cal
audits., for the period from July 1, 2004 through June 30, 2005.
Approval of Contract#74485-4 will allow Contractor to continue to provide transcription services for
the Mental Health Outpatient Psychiatric Unit, through June 30, 2006.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute
on behalf of the County, Contract #74-105-5 with Karen Shuler, a self-employed individual, in an
amount not to exceed $40,750, to provide professional consultation and technical assistance to the
Mental Health Commission, for the period from July 1, 2005 through June 30, 2006.
FISCAL IMPACT:
This Contract is funded 100%by Mental Health Realignment.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
The Mental Health Commission is mandated by the State, and by law, the Health Service
Department's Mental Health Division is required to facilitate the function of this body.
On June 8, 2004, the Board of Supervisors approved Contract #74-105-4 with Karen Shuler for the
provision of professional consultation and technical assistance to the Mental Health Commission
including, but not limited to, attending monthly meetings and acting as a liaison between the
commission and mental health administration, for the period from July 1, 2004 through June 30,
2005.
Approval of Contract #74-105-5 will allow the Contractor to continue providing services through
June 30, 2006.

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATIONS :
Approve and authorize the Health Services Director, or his designee (Pat Godley) to execute on
behalf of the County, Contract #23-074-24 with Foley & Lardner, for provision of temporary legal
services with regard to Medi-Cal audit appeals and other Departmental fiscal, administrative and
litigation matters,for the period from July 1,2005 through June 30,2006.
FISCAL IMPACT:
Contractor shall be paid a fee of not less than $75.00 per hour nor more than $500.00 per hour in
accordance with the Contractor's usual and customary hourly billing rates for its services, as
approved in advance by the Health Services director, and for miscellaneous expenses actually
incurred in the provision of services under the Contract.
This Contract is funded 100% by Enterprise I fiends included in the Health Services Department's
budget.
BACKGROUND/REASON(S)FOR RECOMM ENDATION(S):
Since 1985,this Contractor,which specializes in health care law,has been providing temporary legal
services in the form of consultation, research, opinion, advice and representation for the Health
Services Director with regard to Medi-Cal audit appeals and other Department fiscal, administrative
and litigation matters.
On August 17, 2004,the Board of Supervisors approved Contract#23-074-23 with Foley&Lardner,
for the period from July 1, 2004 through June 30,2005. Approval of Contract#23-074-24 will allow
the Contractor to continue to provide temporary legal services, as requested by the Health Services
Director,through June 30,2006.

RACKGROUND/RFASON(S)FOR RECOMMENDATION(S).

This Contract meets the social needs of the County's population in that it provides augmentation of
room and board, and twenty-four hour emergency residential care and supervision to eligible mentally
disordered clients, who are specifically referred by the Mental Health Program Staff and who are
served by County Mental Health Services.
On June 22, 2004, the Board of Supervisors approved Contract #24-681-59(4),, with Concord Royale,
now known as Berg Senior Living, LLC (dba Concord Royale), to provide augmented board and care
services for County-referred mentally disordered clients, for the period from July 1. 2004 through June
30112005.
Approval of Contract #24-681-59(5) will allow Berg Senior Living, LLC dba Concord Royale) to
provide services through June 30,2006.
