(#21-673-5) SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION: APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to EXECUTE an interagency agreement with Contra Costa County Office of Education for Foster Youth Educational Liaison Services with a payment limit of up to $75,000 for the period July 1, 2005 through June 30, 2006. (#21-673-5) FINANCIAL. IMPACT: 100% County Costs: $75,000. CHILDREN'S IMPACT STATEMENT: This contract directly supports three of the five community outcomes established in the Children's Report Card: 1) "Children Ready For and Succeeding in School"; 2) "Children and Youth Healthy and Preparing for Productive Adulthood"; and 5) "Communities that are Safe and Provide a High Quality of Life for Children and Families." ]BACKGROUND: Contra Costa County Office of Education(CCCOE), school districts, and Employment and Human Services Department (EHSD) collaborated and implemented educational advocacy services for all foster youth in Contra Costa County including support services for foster youth in group homes. The program focuses on literacy, academic, and life skills promoting academic and personal achievement of foster youth in their homes and at school and assists youth to successfully emancipate from foster care. The Department funds approximately 21% of the overall program budget with additional funds allocated from the California Department of Education. Two full time Educational Liaisons will work closely with EHSD Children and Family Services Bureau and Juvenile Probation to improve the educational accomplishments and opportunities for foster youth in Contra Costa County. Additionally, they will work jointly with EHSD to develop in-kind services and seek matching funds from outside sources. FINANCIAL.IMPACT: $60,000 Multi-Purpose Senior Service Program(MSSP): 100% State BACKGROUND: The Multi-purpose Senior Service Program(MSSP), Linkages, and Adult Protective Services(APS)provide case management on a countywide basis for Contra Costa County residents age 65+years who are eligible for MediCal and certifiable for skilled nursing facility care. The purpose of the program is to provide case management services, including but not limited to assessment, care planning, service arrangement, and client monitoring, so clients can safely stay at home and avoid institutionalization. Program services assist individuals to achieve independence, maintain self-sufficiency, and prevent placement in an acute care hospital, skilled nursing facility, or other 24-hour care facility. Affordable Home Care Agency will provide ongoing personal care,protective supervision,household chores, and respite services to MSSP, Linkages, and APS clients as referred by case managers. Services include chore services for the purpose of household support and household tasks;personal care to provide assistance to maintain bodily hygiene,personal safety, and activities of daily living;protective supervision which provide 24 hour supervision to persons in their own home who are very frail or may suffer a medical emergency; and in- home respite to relieve the client's caretaker and prevent a breakdown in th ' ormal s p system. FINANCIAL IMPACT: $805000 100% State, Older Californians Act (OCA), Community Based Service Programs BACKGROUND: The EHSD Aging and Adult Services Bureau,with Adult Area on Aging(AAA) contracts provide services for older residents of Contra Costa County. The FY 2005-2006 Area Plan proposes continued services to provide the Alzheimer's Day Care Resource services. The program provides services to individuals age 60 years and older to include supervision and activities for moderate to severe stages of Alzheimer's disease and related dementia and, provides respite, training, and supportive services to caregivers. BACKGROUND: YMCA of the East Bay is the fiscal agent for a group of non-profit agencies that address the needs of the Richmond/Coronado community through a range of services to youth and adults. Activities under this contract and under the proposed amendment support the Child Welfare Redesign and System Improvement Plan. BACKGROUND: The County's Employment and Human Services Department's (EHSD) Child and Family Services Bureau (CFS)provides services to children and families at risk for or already involved in the Child Welfare System (CWS). As the Parent Partner Coordinator,Ms. Knittel will serve as a voice and advocate for families and children who have been involved in the CWS. Services include assisting the System of Care Federal Grant Coordinator to develop the Family Mentor Program,recruiting, training, and supervising Family Outreach Mentors, participating in System of Care and Child Welfare Redesign workgroups and committees, developing a support network for families and child, and providing individual and system advocacy for families involved in the CWS. Ms. Knittel has extensive Child Welfare experience that includes direct service, training, technical assistance, program oversight, and program development.
