"COMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD) to
execute on behalf of the County, Contract #25-056-2 with Garden Park Apartments Community; a
non-profit corporation, in an amount not to exceed $214,,162, to provide supportive housing services
for Homeless families in Central County, for the period from July 1, 2005 through June 30, 2006.
FISCAL IMPACT:
This Contract is 100%federally finided by the Department of Housing and Urban Development.
CHILDREN'S IMPACT STATEMENT:
This program supports the following Board of Supervisors' community outcome: "Communities
that are Safe and Provide a High Quality of Life for Children and Families". Expected program
outcomes include access to support and educational services for children in homeless families.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S).
This Contract meets the social needs of County's population in that it provides permanent supportive
housing and case management services for families with special needs. Families residing in the
apartments will receive case management services to help them access job training and mainstream
health and human services and to obtain long-term permanent housing.
On July 20, 2004, the Board of Supervisors approved Contract #25456-1 with Garden Park
Apartments Community for the period from July 1, 2004 through June 30, 2005, to renovate a 28-
unit apartment building and provide permanent housing for homeless families.
Approval of Contract #25-056-2 will allow the Contractor to continue providing services through
June 30, 2006.

Approve and authorize the Health Services Director, or his designee(Jeff Smith,M.D.)to execute on
behalf of the County, Contract#26-400-1 with Vicki A. Gallagher, a self-employed individual, in an
amount not to exceed $40,000, to provide consultation and technical assistance to the Contra Costa
Regional Medical Center and Health Centers with regard to the Total Joint Project, for the period
from June 1.2005 through May 31,2006.
FISCAL IMPACT:
This Contract is funded by 100% Enterprise I Budget. Cost to the County depends upon utilization.
As appropriate,patients and/or third party payors will be billed for services.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
Under Contract #26-400-1, Contractor will provide consultation and technical assistance to the
Contra Costa Regional Medical Center and Health Centers with regard the Total Joint Proj ect
including, but not limited to, quality improvement,, patient safety and other operational
procedures,through May 31, 2006.

corporation, in an amount not to exceed $450,000, to provide professional radiologist services at
Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from July 1,
2005 through June 30,2007.

BACKGROUND/REASON(S)FOR RECOMMENDATIO :
For a number of years the County has contracted with Medical, Dental and Mental Health Specialists
to provide specialized professional services that are not otherwise available in its Hospital and Health
Centers.
Under Contract #26-445, Contractor will provide professional Radiology services, including
consultation, on-call, supervision and interpretation of CT Scan, MRI, Ultrasound, invasive
procedures, and plain films, through June 30, 2007.

On November 30, 2004, the Board of Supervisors approved Contract #26-447-2 with the Melody L.
Haworth, to provide professional Ophthalmology assistance to Ophthalmologists in the specialty clinics,
including but not limited to, gathering information regarding medical history, performing diagnostic
tests, conducting anatomical, optical, and functional ocular measurements, and caring for instruments,
as requested by County, for the period from January 1, 2005 through June 30, 2005.
Approval of Contract#26-447-3 will allow the Contractor to continue to provide services through
December 31, 2005.

Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Standard Agreement (Amendment) #74-208-3, with the State Department of
Mental Health, to amend Standard Agreement #74-208-2 (#04-74149-000) to increase the Contract
payment limit by $1,539,268, from $915,354 to a new total of$2,454,622, with no change in the
original term, from July 1, 2004 through June 30,2005.
FISCAL IMPACT:
This agreement is 100% finided by Mental Health Realignment Trust Fund. No County funds are
required.
BACKGROUND/REASON(S)FOR RECOMMIENDATIO
On January 18, 2005, the Board of Supervisors approved Standard Agreement #74-208-2
(#04-74149-000) with the State Department of Mental Health, for the period from July 1, 2004
through June 30, 2005, to allow County to compensate the State, and for the State to provide bed
usage, including staffing, facilities, equipment, and supplies.
Approval of Standard Agreement (Amendment) #74-208-3 will allow County to pay for the actual
beds and services required during fiscal year 2004/2005.
Five sealed/certified copies of this Board Order should be returned to the Contracts and Grants Unit
for submission to the State.

1. RECOMMENDATION:
APPROVE and AUTHORIZE the Sheriff, or designee, to execute a contract on behalf of
the County with Jean C. Hastings for the period July 1, 2005 through June 30, 2006, in an
amount not to exceed $48,840.00, to provide Library Services to inmates at Contra Costa
County Detention Facilities.
Il. FINANCIAL IMPACT:
NONE. This contract is fully paid from the Inmate Welfare Fund.
III. BACKGROUND:
Maintaining a Detention library that can provide an adequate supply of reading material for
the recreational enjoyment of inmates is an important component of the mission of the Inmate
Welfare Fund. Qualified personnel with knowledge of library practices and policies are critical
to the success of the program.
