SUBJECT: Approval of Award Agreement#28-735 from The California Endowment
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECCOMMENDATION(S)
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.) to
execute on behalf of the County, Award Agreement#28-735 from the California Endowment, to pay
County in an amount not to exceed $150,000, for the Public Health Outreach, Education and
Collaborations(PHOEQ Program, for the period from March 1,2005 through February 28,2007.
FISCAL,IMPACT:
This Award Agreement will result in an amount not to exceed $150,000 from the California
Endowment for the Public Health Outreach, Education and Collaborations (PHOEQ Program. No
County funds are required.

Under Award Agreement #28-735, the California Endowment will provide funding to the County's
Health Services Department for the Public Health Outreach, Education and Collaborations (PHOEQ
Program with regard to developing comprehensive community-based public health programs
including,but not limited to, developing a community ladder of participation, conducting training for
County staff and other local health departments and making presentations at regional, state and
national conferences, for the period from March 1,2005 through February 28, 2007.
Three certified copies of the Board Order should be returned to the Contracts and Grants Unit.

SUBJECT: Approval of Agreement #29-666 with Regents of the University of California, on behalf of the
University of California San Francisco
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee(Jeff Smith,M.D.)to execute on behalf of the
County, Agreement #29-666 with the Regents of the University of California, on behalf of the University of
California San Francisco, to pay County in an amount not to exceed $22,711, for the Leading Organizational
Change-Advancing Quality Through Culturally Responsive Care Project,for the period from February 1,2005
through July 31, 2005.
The Regents of the University of California,on behalf of the University of California San Francisco has requested
to be held harmless and indemnified as part of their standard for approval of the Agreement with the County. In
the view of County Risk Management this indemnification will create a minimal potential loss. The Health
Services Department desires to expedite approval of the Agreement as written.
FISCAL IMPACT:
Acceptance of this Agreement will result in$22,711 from Regents of the University of California on behalf of the
University of California San Francisco. No County funds are required.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
The goal of the Leading Organizational Change-Advancing Quality Through Culturally Responsive Care Project
is to focus on the prenatal needs of approximately 1500 women in East Contra Costa including,but not limited to
providing training for 40 Registered Nurses, 20 other nursing staff and 15 physicians to provide care for this
group of women in a culturally competent manner, through July 31, 2005.
Three certified copies of the Board Order should be returned to the Contracts and Grants Unit.
