Approve and authorize the Health Services Director, or his designee (Jeff Smith, MD)to execute on
behalf of the County, Contract #26-203-23 with Nor-Cal Medical Temps, a corporation, in an
amount not to exceed $320,000, to provide temporary help (pharmacists and pharmacy technicians)
at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from April
1,2005 through March 31,2006.
FISCAL IMPACT:
This Contract is funded 100%by Enterprise I Budget.
BACKGROUND/REASON(S)FOR RECOMMENDATIONS
On March 9,2004,the Board of Supervisors approved Contract#26-203-21 (as amended by Contract
Amendment Agreement #26-203-22) with Nor-Cal Medical Temps, for the period from April 1,
2004 through March 31, 2005, for temporary help during vacations, sick leave, and temporary
absences of County pharmacists and technicians, as well as during periods when permanent positions
were vacant.
App roval 'of Contract #26-203—23 will allow the Contractor to continue providing services through
March 31,200"6.

Approve and authorize the Health Services Director, or his designee(Jeff Smith,M.D.)to execute on
behalf of the County, Contract #26-518 with Alexander Gorodetsky, M.D., a self-employed
individual, in an amount not to exceed $387,180, to provide professional outpatient psychiatric
services, for theP eriod from April 1, 2005 through March 31,2007.
FISCAL IMPACT:
This Contract is funded by Enterprise I Budget. Cost to the County depends upon utilization. As
appropriate,patients and/or third party payors will be billed for services.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S�:
For a number of years the County has contracted with Medical,Dental and Mental Health Specialists
top rovide specialized professional services that are not otherwise available in its Hospital and Health
Centers.
On May 4, 2004, the Board of Supervisors approved a Mental Health Specialist Contract#24-991-6
(as amended by Amendment Agreement #24-991-7) with Alexander Gorodetsky, M.D., for the
period from April 1, 2004 through March 31, 2005, for the provision of professional psychiatric
services for the Mental Health Division.
Approval of Contract #26-518 will allow the Contractor to provide psychiatric services for Contra
Costa Regional Medical Center and Contra Costa Health Centers,due to an organizational restructure
at the Hospital,through March 31,2007.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee, (Jeff Smith,MD.)to execute on
behalf of the County, Contract Amendment Agreement #26-977-5 with Pacific Urology, a
Corporation, effective May 1, 2005, to amend Contract #26-977-3 (as amended by Amendment
Agreement #26-977-4) to increase the Payment Limit by $100,000 to a new total payment limit of
$175,000 and to extend the term from October 31, 2005 to October 31, 2006, to provide additional
urology services to patients at Contra Costa Regional Medical Center.
FISCAL,IMPACT:
100% Enterprise I Funds. Cost to the County depends upon utilization. As appropriate, patients
and/or third party payors will be billed for services.
BACKGROITND REASON(S)FOR RECOMMENDATIONS:
For a number of years the County has contracted with Medical and Dental Specialists to provide
specialized professional services which are not otherwise available in its hospital and clinics.
On October 28, 2003, the Board of Supervisors approved Contract #26-977-3 (as amended by
Amendment Agreement #26-977-4) with Pacific Urology, for the period from November 1, 2003
through October 31, 2005, to provide Urology services at Contra Costa Regional Medical Center,
including consultation,training, and medical and/or surgical procedures.
Approval of Contract Amendment Agreement #26-977-5 will allow Contractor to provide an
additional urology services at Contra Costa Regional Medical Center,through October 31,2006.

through March 31, 2004.
Approval of Contract#26-983-4 will allow Contractor to continue providing services through March
31,2008.

Approve and authorize the Health Services Director, or his designee (Richard Harrison) to
execute on behalf of the County, Contract #27-253-6 with California Eye Clinic, a partnership, in
an amount not to exceed $200,000, to provide professional ophthalmology services for the Contra
Costa Health Plan for the period from May 1, 2005 through April 30, 2007.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan(Health Plan)member premiums. Costs depend
upon utilization. As appropriate,patients and/or third party payors will be billed for services.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
The Health Plan has an obligation to provide certain specialized professional health care services for
its members under the terms of their Individual and Group Health Plan membership contracts with
the County.
On May 25, 2004, the Board of Supervisors approved Contract #27-253-5 with California Eye
Clinic for the period from May 1, 2004 through April 30, 2005, for the provision of professional
ophthalmology services.
Approval of Contract #27-253-6 will allow the Contractor to continue providing services through
April 30,2007.

Approval of Contract #27-583-1 will allow the Contractor to continue providing services through
April 30,2007.
