Approve and authorize the Health Services Director, or his designee (Richard Harrison), to execute
on behalf of the County, Contract Amendment Agreement#27-541-2 with Language Line, LLC (dba
Language Line Services), a Limited Liability Corporation, effective April 1, 2005, to amend Contract
#27-541-1 to increase the total Contract Payment Limit by $85,000 from $75,000 to a new total
Contract Payment Limit of$160,000 and with no change in the original term of the Contract through
August 31,2005.
FISCAI.IMPACT:
This Contract is funded 100%by Contra Costa Health Plan Member Premiums.
RACKGROUND/RF.ASON(S) FOR RECOMMENDATION,
For several years Language Line Services has provided foreign language interpretation services via
over-the-phone to Contra Costa Regional Medical Center (CCRMC). The Contractor shall recruit,
screen, train, supervise, and otherwise provide qualified interpreters to provide over-the-phone
foreign language translation services, with over 100 languages from Vietnamese to Portuguese, for
CCRMC and the Contra Costa Health Plan patients.
On September 14, 2004,the Board of Supervisors approved Contract#27-541-1 with Language Line,
LLC (dba Language Line Services) to provide imperative translation services for CCRMC and
Health Plan patient's who need access facilities and healthcare, when a translator is not physically
available, as requested by Department, for the period from September 1, 2004 through August 31,
2005.
Approval of Contract Amendment Agreement #27-541-2 will allow the Contractor to provide
additional over-the-phone interpretation services,through August 31,2005.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract #74-163-7 with Delta Personnel Services, Inc. (dba Guardian
Security Agency), a corporation, in an amount not to exceed $44,000, to provide Security Guard
services for the period from May 1,2005 through April 30,2006.
FISCAL IMPACT:
This Contract is funded 100%by Mental Health Realignment, offset by third-party revenues.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
In November 2004, the County Administrator approved, and the Purchasing Services Manager
executed Contract #74-163-6 with Delta Personnel Services, Inc. (dba Guardian Security Agency)
for the period from October 1, 2004 through April 30, 2005, for the provision of security guard
services, in order to safeguard equipment and furnishings, and to monitor unauthorized visitors at
County's Adult Mental Health Outpatient Clinic in Richmond.
Approval of Contract #74^-163-7 will allow the Contractor to continue providing services through
Apri 13 0,2006.

Approve and authorize the Health Services Director, or his designee (Jeff Smith, M.D.), to execute on
behalf of the County as follows:
1. Cancellation Agreement#26-907-6 with Laszlo Tamas,M.D., effective at the close of business on
April 30, 2005; and
2. Contract #26-907-7, with Laszlo Tamas, M.D., a self-employed individual, (Specialty:
Neurosurgery), in an amount not to exceed $975,000, to provide professional neurosurgery services
for patients at Contra Costa Regional Medical Center, for the period from May 1, 2005 through April
30,2008.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise I Funds. Cost to the County depends upon utilization. As
appropriate,patients and/or third party payors will be billed or services.
BACKCROITND/RFIASON(S)FOR RF.COMMENDATION(S):
On March 25, 2003, the Board of Supervisors approved Contract #26-907-5 with Laszlo Tamas, M.D.,
for the period from April 1, 2003 through March 31, 2006, for the provision of neurosurgery services,
including consultation, training, on-call services, clinic coverage, and medical and/or surgical procedures
for Contra Costa Regional Medical Center and Contra Costa Health Centers.
Due to the restructuring of the Neurosurgery Department to eliminate 24-hour on-call neurosurgery
coverage, the Department and Contractor have mutually agreed to renegotiate the Contract Payment
Provisions and redefine on-call duties. Therefore, in accordance with Paragraph 4. (Termination) of the
Contract, the Department and Contractor have agreed to a mutual cancellation of Contract #26-907-5.
Approval of Cancellation Agreement#26-907-6 will accomplish this termination.
Under Contract #26-907-7, Contractor will continue to provide professional Neurosurgery services to
patients at Contra Costa Regional Medical Center,through April 30, 2008.

RECOMMNDATION(S) :
Approve and authorize the Health Services Director or his designee
(Jeff Smith, M. D. ) to execute on behalf of the County, Contract
Amendment/Extension Agreement #26-360-8 with DELIVEREX, a corporation,
effective April 30, 2005, to amend Contract #26-360-3 (as amended by
Contract Amendment Agreements #26-360-4 through 426-360-7) , to increase
the total Contract Payment Limit by $135, 000 from $718, 000 to a new
total payment limit of $853, 000, and to extend the term of the Contract
from April 30, 2005 through August 31, 2005 .
FISCAL IMPACT:
Funding for this contract is included in the Department ' s Enterprise I
Budget.
BACKGROUND/REASON(S) FOR RECOMMNDAT I ON(S) :
On December 18, 2001, the Board of Supervisors approved Contract #26-
360-3 (as amended by Contract Amendment Agreements #26-360-4, through
#26-360-7) with DELIVEREX, for the period from November 1, 2001 through
April 30, 2005, to provide off-site medical records storage and
retrieval services for Contra Costa Regional Medical Center (CCRMC) and
various Divisions within the Health Services Department.
Approval of Contract Amendment/Extension Agreement #26-360-8 will allow
the Contractor to provide additional services through August 31, 2005,
as requested by the Department, due to a continuous increase in
utilization during the term of the agreement, which was actually higher
than originally anticipated, and allow the Department to continue
completion of the pending RFP process .

on behalf of the County, Contract Amendment Agreement #26-294-12 with Staff Care, Inc., a
Corporation, effective May 1, 2005, to amend Contract #26-294-8 (as amended by Amendment
Agreements #26-294-9 through #26-294-11), to increase the total Contract Payment Limit by
$125,000 from $975,000 to a new total Contract Payment Limit of$1,100,000 and with no change in
the original term of the Contract through December 31,2005.

RACKGROIJND/RFASON(S)FOR RF.COMMENDATI N 1:
For many years the County has contracted with registries to provide temporary qualified personnel to
assist the Department during pear workloads,temporary absences and emergency situations.
On December 17, 2002, the Board of Supervisors approved Contract #26-294-8 (as amended by
Contract Amendment Agreements #26-294-9 through #26-294-11) with Staff Care, Inc., for the
period from January 1, 2003 through December 31, 2005, to provide locum tenens physicians to
work as temporary employees to provide vacation, sick leave, and extended leave relief for County-
employed physicians at Contra Costa Regional Medical Center and Contra Costa Health Centers,
including the specialty of Radiology services.
Approval of Contract Amendment Agreement #26-294-12 will allow the Contractor to provide
additional physician services, especially in the Radiology, Orthopedic, ENT and Psychiatric Clinics
due to a back-log of patients needing appointments,through December 31, 2005.
