with PHFE Management Solutions(dba Public Health Foundation Enterprises, Inc.)
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD) to
execute on behalf of the County, Contract Amendment/Extension Agreement#22-806-6 with PHFE
Management Solutions (dba Public Health Foundation Enterprises, Inc.), a non-profit corporation,
effective February 1, 2005, to amend Contract #22-806-5, to increase the Contract payment limit by
$420,000, from$150,144 to a new total of$570,144 and extend the Contract term through March 31,
2006.
FISCAL IMPACT:
This Contract is funded 91.4%by a Federal Bioterrorism Grant, and 8.6%by County GRIP funds.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On April 20, 2004, the Board of Supervisors approved Contract #22-806-5 with the Contractor for
the period from April 1, 2004 through March 31, 2005, for the provision of consultation and
technical assistance with regard to community health promotion for the Partnership for Public Health
(PPH) Program including organization services for the West County Health Collaborative,
Monument Community Partnership and Bay Point Community Partnership Programs, leadership and
public health advocacy training and participation with PPH Program's Action Plans.
Approval of Contract Amendment/Extension Agreement #22-806-6 will allow the Contractor to
continue providing services through March 31, 2006.

for the provision of specialized geropsychological and neuropsychological testing services for mental health
patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from
April 1, 2005 through March 31, 2008.

On March 26, 2002, the Board of Supervisors approved Contract #26-364-3 with Margaret A. Thayer,
Ph.D., for the period from April 1, 2002 through March 31, 2005, for the provision of geropsychological
and neuropsychological testing services for mental health patients at Contra Costa Regional Medical Center
and Contra Costa Health Centers.
Approval of Contract #26-3644, will allow Margaret A. Thayer, Ph.D., to continue providing services,
including group therapy sessions for the Family Practice Clinic, through March 31, 2008.

execute on behalf of the County, Contract #26-473-2 with Supplemental Health Care, a
corporation, in an amount not to exceed $500,000, to provide nursing registry temporary
help services for Contra Costa Regional Medical Center, for the period from April 1, 2005
through March 31, 2006.
FISCAL IMPACT:
This Contract is funded in the Health Services Department Enterprise I Budget.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
For many years the County has contracted with registries to provide temporary qualified personnel to
assist the Department during peak workloads,temporary absences and emergency situations.
On March 16, 2004, the Board of Supervisors approved Contract #26-473-1 with Supplemental
Health Care for temporary nurses for the Contra Costa Regional Medical Center, for thep eriod from
April 1, 2004 through March 31,2005.
Approval of Contract #26-473-2 will allow the Contractor to continue to provide registry nursing
temporary help services,through March 31, 2006.

Contract#26-474-3 with Frank J. Puglisi, Jr. a self-employed individual, in an amount not to
exceed $45,000, to provide professional consultation services to the Health Services
Director with regard to the Contra Costa Regional Health Foundation Board, for the period
from April 1, 2005 through September 30, 2005.

This Contract is included in the Health Services Department Enterprise I Budget.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On February 24, 2004, the Board of Supervisors approved Contract #26-474-2 with Frank Puglisi,

Jr., for the period from April 1, 2004 through March 31, 2005, to provide professional
consultation and technical assistance to the Health Services Director with regard to the Contra
Costa Regional Health Foundation Board. Services included, but were not limited to, staffing
Board meetings, developing fund-raising goals, developing policies and procedures, and attending
workshops and conferences on behalf of the County.
Approval of Contract #26-474-3 will allow the Contractor to continue to provide services, through
September 30, 2005.
