(dba Senior Outreach Services of Contra Costa),a non-profit corporation,in an amount not to exceed $151,593, to provide congregate meal services for the Senior Nutrition Program, for the period from July 1, 2004 through June 30, 2005. This Contract includes a four-month automatic extension through October 31,2005,in an amount not to exceed$50,531. F#1scAL IMPACT: This Contract is 100% federally funded under Title III-C(1) of the Older Americans .Act of 1965. No County funding is required. BAcKGRC uND/RE,AS(lN )FOR RECOMMENDATION(S): This Contract meets the social needs of County's population in that it provides meals on 250 searing days, at seven sites throughout the County, to an average of 225 senior citizens, to ensure they receive at least 113 of their daily nutritional requirements. On;August 5,2003, the Board of Supervisors approved Novation Contract#22433-44 with Family and Community Services, Inc. (dba Senior Outreach Services of Contra Costa) for the period from July 1, 2003 through June 30, 2004, (which included a four-month automatic extension through October 31,2004) for the provision of congregate meal services for the Senior Nutrition Program. Approval of Novation Contract #22-033-45 replaces the automatic extension under the prior Contract and allows the Contractor to continue providing services through June 30,2005. BACKGRouND/RgASON(S)FoR REcommFNDATiQN(s): This Contract meets the social needs of County's population in that it provides home-delivered meals on 250 serving days, to an average of 540 senior citizens, to ensure they receive at least 1/3 of their daily nutritional requirements. On August 5, 2403, the Beard of Supervisors approved Novation Contract##22-137-32 with Family and Community Services, Inc. (dba Senior Outreach Services of Contra Costa) for the period from July 1, 2403 through June 30, 2004, (which included a four-month automatic extension through October 31,2004)for the provision of home-delivered meals for the Senior Nutrition Program. Approval of Novation Contract 422-137-33 replaces the automatic extension under the prior Contract and allows the Contractor to continue providing services through June 30,2005. SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on behalf of the County, Contract #74-017-5 with STARS, Inc., a non-profit corporation, in an amount not to exceed $375,000, to provide mental health services for Seriously Emotionally Disturbed(SED)Children in West County for the period from July 1, 2004 through June 30, 2005. FISCAL IMPACT: This Contract is funded 50%by Federal Medi-Cal, and 50%by State EPSDT funds. CHILDREN'S IMPACT STATEMENT: This Children's Mental Health Program will support the following Board of Supervisors' community outcomes: "Children Ready For and Succeeding in School"; "Families that are Safe, Stable, and Nurturing"; and "Communities that are Safe and Provide a High Quality of Life for Children and Families". Expected program outcomes include an increase in positive social and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS)and Youth Self Report(YSR). BACKGROUND/REASON(S)FOR RECONEMENDATION(S): This Contract meets the social needs of County's population in that it provides mental health services for high-risk children and adolescents with emotional and behavioral problems to address issues of physical, sexual, emotional, and substance abuse; gang involvement; academic skills; and family issues, and to improve school performance, reduce unsafe behavioral practices, and reduce the need for out-of-home placements. On August 5, 2003, the Board of Supervisors approved Contract #74-017-4 with STARS, Inc. for the period from July 1, 2003 through June 30, 2004, for the provision of mental health services for Seriously Emotionally Disturbed(SED)Children.
