SUBJECT: APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to
EXECUTE a contract with California State University(CSU)Hayward to ACCEPT funding in
the amount of$91,000 for the Child Welfare Intern Partnership with CSU Hayward to provide
supervision to interns fulfilling Masters of Social Welfare program requirements for the period of

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to EXECUTE a
contract with California State University(CSU)Hayward to ACCEPT funding in the amount of$91,000 for the
Child Welfare Intern Partnership with CSU Hayward to provide supervision to interns fulfilling Masters of
Social Welfare program requirements for the period of July 1, 2004 through June 30, 2005. (#29-164-0)
FINANCIAL IMPACT:
County to receive $91,000 from California State University Hayward. (The $91,000 will defray cost of one (1)
Social Work Supervisor in the amount of$109,116 minus CSU Hayward contribution of$91,000 with a balance
of $18,116 to be funded with State and Federal IVE funding). No County Cost.
CHILDREN'S IMPACT STATEMF.NT:
The contract will support all five of Contra Costa County's community outcomes: (1) "Children Ready for and
Succeeding in School"; (2) "Children and Youth Healthy and Preparing for Productive Adulthood"; (3)"Families
that are Economically Self-Sufficient"; (4) "Families that are Safe, Stable and Nurturing"; and (5)"Communities
that are Safe and Provide a High Quality of Life for Children and Families"by advancing the education and
training of individuals to provide high quality services to children and families in need of assistance.
BACKGROUND:
EHSD has been in partnership with CSU Hayward in the career development of Master of Social Welfare
qualified staff for Child Welfare for several years. A part of the education process is an internship in the Child
Welfare division of a public social welfare agency. EHSD works closely with universities to expand and support
the internship programs. While in the internship program, interns must be supervised by an individual with a
Masters in Social Welfare. The CSU Hayward funding will be used to defray the costs of a Social Work
Supervisor to provide assistance and supervision to interns in the Masters of Social Welfare internship program
fulfilling requirements at an EHSD site.

Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD)to execute
on behalf of the County, Novation Contract #28-501-13 with Pleasant Hill Recreation and Park
District, a public Agency, to pay County an amount not to exceed $7,800, to provide congregate meal
services for County's Senior Nutrition Program, for the period from July 1, 2004 through June 30,
2005. This Novation Contract includes a three-month automatic extension through September 30,
2005, in an amount not to exceed$1,950.
FISCAL IMPACT:
No County cost.
BACKGROUND/REASON(S) FOR RECOMIVIENDATIONN.
This Contract meets the social needs of County's population in that it provides an average of 50
congregate meals per day, five days per week for senior citizens at the Pleasant Hill Senior Center.
On July 22, 2003, the Board of Supervisors approved Novation Contract #28-501-12, with Pleasant
Hill Recreation and Park District, for the period from July 1, 2003 through June 30, 2004 (which
included a three-month automatic extension through September 30, 2004) for thep rovision of
congregate meal services for County's Senior Nutrition Program.
Approval of Novation Contract #28-501-13 will replace the automatic extension under the prior
Contract and will allow the Contractor to continue providing services through June 30, 2005.

BACKGROUND/REASON(S) FOR RECOMMENDATIONN.
This Contract meets the social needs of County's population in that it provides an average of 120
congregate meals per day, five days per week for senior citizens at the Antioch Senior Center.
On July 15, 2003, the Board of Supervisors approved Novation Contract #28-540-9, with City of
Antioch— Leisure Services Department, for the period from July 1, 2003 through June 30, 2004
(which included a three-month automatic extension through September 30, 2004) for the provision
of congregate meal services for County's Senior Nutrition Program.
Approval of Novation Contract #28-540-10 will replace the automatic extension under the prior
Contract and will allow the Contractor to continue providing services through June 30, 2005.

unincorporated areas surrounding these cities.
On July 8, 2003, the Board of Supervisors approved Novation Contract #28-559-9 with West Contra
Costa County Meals on Wheels, for the period from July 1, 2003 through June 30, 2004 (which
included a three-month automatic extension through September 30, 2004) for the provision of
congregate meal services for County's Senior Nutrition Program.
Approval of Novation Contract #28-559-10 will replace the automatic extension under the prior
Contract and will allow the Contractor to continue providing services through June 30, 2005.

On December 16, 2003, the Board of Supervisors approved Novation Contract #28-644-3 with
Golden Rain Foundation, for the period from July 1, 2003 through June 30, 2004 (which included
a three-month automatic extension through September 30, 2004) for the provision of congregate
meal services for County's Senior Nutrition Program.
Approval of Novation Contract #28-6444 will replace the automatic extension under the prior
Contract and will allow the Contractor to continue providing services through June 30, 2005.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD) to execute
on behalf of the County, Novation Contract #28-530-11 with the City of San Ramon, a public
Agency, to pay County an amount not to exceed $11,000, to provide congregate meal services for
County's Senior Nutrition Program, for the period from July 1, 2004 through June 30, 2005. This
Novation Contract includes a three-month automatic extension through September 30, 2005, in an
amount not to exceed$2,750.
FISCAL IMPACT:
No County funds are required.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
This Contract meets the social needs of County's population in that it provides an average of 40
congregate meals per day, two days per week for senior citizens at the San Ramon Senior Center, and
15 home delivered meals per day to senior citizens in the San Ramon area.
On July 22, 2003, the Board of Supervisors approved Novation Contract#28-530-11 with the City of
San Ramon, for the period from July 1, 2003 through June 30, 2004 (which included a three-month
automatic extension through September 30, 2004) for the provision of congregate meal services for
County's Senior Nutrition Program.
Approval of Novation Contract #28-530-11 will replace the automatic extension under the prior
Contract and will allow the Contractor to continue providing services through June 30, 2005.

BACKGROUND:
The County Homeless program is seeking funding through Target Community Giving to support Calli
House's Family Violence Prevention services. Calli House has provided outreach, short-term shelter,
goal-oriented counseling, educational and vocational opportunities to homeless and street youth, ages
14-21, in West County since April 2002. Since that time, Calli House has served over 200 unduplicated
youth in its temporary shelter and multi-service center. Approximately 40% of all youth entering Calli
House report some form of family violence or neglect.

BACKGROUND:
The County Homeless program is seeking funding through The San Francisco
Foundation to support a Street Youth Outreach program in Contra Costa County. This
program will target the County's most vulnerable homeless and runaway youth, those youth
ages 14-21 who are living on the streets or in encampments. These unaccompanied and
unprotected youth are subject to repeated sexual and other criminal predation. The County
Homeless program has identified the need for youth specific outreach through aggregate data
derived from the operation of the Homeless Outreach for People in Encampments (HOPE)
project. Outreached youth will be provided appropriate shelter, basic necessities/services, and
referrals.

A. APPROVE the Director of the Health Services Department or his designee to execute and
submit an application along with the necessary certifications and assurances to the US Department
of Health and Human Services Administration for Children and Families (ACF), Family and
Youth Services Bureau(FYSB), for funding for the operation of a Street Youth Outreach Program,
for youth ages 14-21, in Contra Costa County.
B. AUTHORIZE the Director of the Health Services Department or his designee to accept up to
$200,000 per year, for three years, in Family and Youth Services Bureau(FYSB) funding and
enter into a contract with US Department of Health and Human Services to perform all
responsibilities in relationship to receipt of the funding and contracted provisions for the operation
of the program for FY 2004-2047.
FISCAL IMPACT:
The funds requested from the Street Youth Outreach grant are necessary for the operation of a Street Youth
Outreach Program. Funds already allocated for the Single Adult Emergency Program will be used for the
(10%) applicant match required by the Family and Youth Services Bureau(FYSB) grant. In addition,
private sources of funding are being pursued. No additional County funds are required.

RECOMMENDATION:
A. Approve and Authorize the Director of Health Services Department or his designee to
execute and transmit an application along with the necessary certifications and assurances
to the Department of Housing and Urban Development (HUD) McKinney-Vento Continuum
of Care Homeless Assistance funding to support the services and operations of the FERST
multi-service centers for individuals and families.
B. Authorize the Director of Health Services Department or his designee, if awarded funding,
to accept up to $298,547 in HUD McKinney-Vento funding and enter into contract with
HUD to perform all responsibilities in relation to receipt of the funding and contracted
provisions for the operations of the FERST multi-service centers.
FISCAL IMPACT:
The funding requested of HUD will help to continue basic needs, counseling, and
information and referrals services for families and individuals throughout the County. The
20% match requirement will be met in-kind through Homeless Program projects that are
currently appropriated. No additional County funds required.
BACKGROUND:
The FERST program is critical to meeting the County's need for multi-service center (MSC)
entry points into the Continuum of Care. Originally awarded HUD McKinney-Vento funding In
1997 with St. Vincent de Paul, it became a project of the County Homeless program May 31,
2001.

Approve and authorize the Health Services Director, or his designee
(Donna Wigand, LCSW) to execute on behalf of the County, Standard
Agreement #29-500-10 (State #04-74020-000) with the State of California,
Department of Mental Health, for the period from July 1, 2004 through
June 30, 2005, in an amount not to exceed $2, 304, 817, to implement and
administer Managed Mental Health Care services for Medi-Cal eligible
residents of Contra Costa County.
FISCAL IMPACT:
Approval of this agreement will result in a total payment limit of
$2, 304, 817 from the State Department of Mental Health, for Managed
Mental Health Care services. No County funds are required.
REASONS FOR RECOrMENDATIONS/BACKGROUND:
On January 14, 1997, the Board adopted Resolution No 97/17, authorizing
the Health Services Department ' s Mental Health Division to assume
responsibility, for Fee-for Service Medi-Cal specialty mental health
services . On December 9, 2003, the Board of Supervisors approved
Standard Agreement #29-500-8 (as amended by Amendment Agreement #29-500-
9) with the State Department of Mental Health, for the period from July
1, 2003 through June 30, 2004 .
Approval of Standard Agreement #29-500-9 will allow the County to
continue to implement and administer Managed Mental Health Care services
for Medi-Cal eligible residents of Contra Costa County, through June 30,
2005.
Three sealed/certified copies of this Board Order should be returned to
the Contracts and Grants Unit for submission to the State Department of
Mental Health.
