SUBJECT: Update on the Services Provided by the Mental
Health Division at the Crestwood Behavioral Health, Inc. .�
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
ACCEPT update from the County Administrator and the Department of Health Services,
Mental Health Division, regarding the management and security of the Crestwood facility
and the status of its operation.
FISCAL IMPACT:
No financial impact.
BACKGROUND:
This is the third quarterly report on the services provided, management and security of the
Crestwood Behavioral Health Center located in Pleasant Hili. The center opened and
received the County's first three consumers on October 10, 2003. As of the end of
November, there were fifteen consumers residing at the facility. There have been a total
of 48 consumers referred to Crestwood since it opened. As of May 25, 2004 there are a
total of 35 currently residing at the facility. Of these residents, 2 are in college and 2 in
vocational training, one working in the community, one who has received their high school
diploma and one who has gotten their GED. There have been +6 consumers discharged
back to their communities and 7 returned to a higher level of care.

Since we last reported to you, the following has occurred:
0 On May 10, 2004 at approximately 6:30 p.m. a Crestwood consumer walked away
from the facility. The egress bell was set, the door alarms were not and the
Crestwood staff was not alerted to the man's absence until the facility was
contacted by a Poets Corner neighbor stating that the Crestwood patient had come
to their house asking for a ride to the BART station. The residents spoke with the
man and then called a taxi to take him to the BART station. The neighbors called
thepolice to report the incident and followed up with a call to Crestwood. Total time
elapse between the encounter and the calls to Crestwood and the police
department was approximately 45 minutes. Neither Crestwood nor the Pleasant
Hill Police Department were able to intercept the patient at the BART station. The
patient was located and was placed in a facility offering a higher level of treatment.
0 On April 9, 2004, at approximately 7 p.m., a consumer left the building without
approval. He was seen at a nearby business and returned to the facility by two
staff members. The Pleasant Hill Police were notified by Crestwood. The
consumer was placed on a "one-to-one" status— monitored by staff at all times. On
April 11, 2004 at approximately 3 a.m., the consumer was getting a drink of water
when he ran out of the building. Crestwood staff was not able to catch him and
called for police assistance. The Pleasant Hill Police returned the consumer to the
Center approximately three hours later and was subsequently transferred to a
facility offering a higher level of treatment.
• On April 13, 2004, the Pleasant Hill Planning Commission received the second
quarterly report from the Zoning Administrator on the status of Crestwood regarding
their compliance with the conditions of the use permit approved by the City of
Pleasant Hill. The Zoning Administrator report that the "To date, the Crestwood
Healing Center is being operated in full compliance with the use permit conditions
of approval...
• The Neighborhood Oversight Committee has conducted an election and installed
new officers. The new Committee held their first meeting on April 8, 2004. Health
Services Department, Mental Health staff were invited and attended this meeting.
By the end of the meeting the Committee determined that they were 61... currently
satisfied that Crestwood has tried to meet, and in some cases, exceed its reporting
requirement to the Committee, the County and the Community. Their response has
uniformly been swift ...
