RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD) to
execute on behalf of the County, Contract Amendment Agreement#25-044-3 with Mental Health
Consumer Concerns, a non-profit corporation, effective June 30, 2004,to amend Contract#25-044-2,
to increase the Contract payment limit by $11,106, from $94,925 to a new total of$106,031 and
extend the Contract term through September 30, 2004.
FISCAL IMPACT:
This Contract Amendment is funded 100%by Federal Substance Abuse and Mental Health Services
Administration(SAMHSA)Funds.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
On September 23, 2003, the Board of Supervisors approved Contract #25-044-2 with Mental Health
Consumer Concerns, for the period from August 15, 2003 through June 30, 2004, for the provision
of peer-counseling services to homeless clients involved in the Health, Housing and Integrated
Services Network (HHISN)
Approval of Contract Amendment Agreement #25-044-3 will allow the Contractor to continue
providing services through September 30,2004.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director,William Walker,M.D.,to execute on behalf of the County,
Contract #27-056-12 with Robert A. Turcios, O.D., a self-employed individual, in an amount not to exceed
$501,000,for provision of professional Optometry services for Contra Costa Health Plan members,for the period
from July 1, 2004 through June 30, 2005.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums. Costs depend upon utilization. As
appropriate, patients and/or third party payors will be billed for services.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County.
On July 15,2003,the Board of Supervisors approved Contract#27-056-11 with Robert A.Turcios,O.D.,for the
period from July 1, 2003 through June 30, 2004, for professional Optometry services for Contra Costa Health
Plan members.
Approval of Contract #27-056-12 will allow the Contractor to continue to provide professional Optometry
services to Health Plan members, through June 30, 2005.

FISCAL IMPACT:
This Contract is 100% funded by Mental Health Realignment.
BACKGROUND/REASON(S) FOR RECOMMNDAT ION(S) :
The Department ' s residential support program, called Supported
Adult Group Enterprises (S.A.G.E. ) provides Augmented Board and
Care Services designed to meet the needs of the seriously
persistently mentally ill (SPMI) adults, who currently reside in
locked facilities and state hospitals . Hardaway Family Care Home
is one of the few board and care facilities that was able to meet
the S.A.G.E. Program requirements under the Department ' s Request
For Proposal.
On May 13, 2003, the Board of Supervisors approved Contract
#24-887-8 with Willie Hardaway (dba Hardaway Family Care Home) ,
for the period from July 1, 2003 through June 30, 2004, for the
provision of augmented board and care services for County-referred
mentally disordered clients .
Approval of Contract #24-887-9 will allow the Contractor to
continue providing six (6) dedicated beds for use by County-
referred clients, through June 30, 2005 .

care services for County-referred mentally disordered clients, for the
period from July 1, 2004 through June 30, 2005 .

This Contract is 100% funded by Mental Health Realignment .
BACKGROUND/REASON(S) FOR RECObMNDAT ION(S)
This Contract meets the social needs of the County' s population in that it
provides augmentation of room and board, and twenty-four hour emergency
residential care and supervision to eligible mentally disordered clients,
who are specifically referred by the Mental Health Program Staff and who
are served by County Mental Health Services. Fred Williams (dba William' s
Board and Care) is one of the few board and care facilities in the County
that is large enough to accommodate the number of beds and the level of
care necessary for this project.
On May 20, 2003, the Board of Supervisors approved Contract #24-681-20 (6)
with Fred Williams (dba William' s Board and Care) , for the period from
July 1, 2003 through June 30, 2004, for Augmented Board and Care Services
for mentally disordered adults, as part of the Department ' s strategy to
reduce the County' s patient census in Institutions for the Mentally
Diseased (IMD' s) and at Napa State Hospital.
Approval of Contract #24-681-20 (7) will allow the Contractor to continue
providing services through June 30, 2005.

BACKGROUND/REASON(S) FOR RECOMMNDAT ION(S) :
This Contract meets the social needs of the County' s population in that it
provides augmented board and care to young, at-risk mental health clients,
who are specifically referred by the Mental Health Program Staff, and who
are served by Contra Costa County Mental Health services . Providing this
augmented board and care keeps these young adults clients out of higher
levels of care institutions, including State Hospitals .
On May 20, 2003, the Board of Supervisors approved Contract #24-681-52 (3)
with Nenita and Edgardo Dacoron (dba Agape Family Care Home) , for the
period from July 1, 2003 through June 30, 2004, to provide 24-hour
augmented board and care services for County-referred mentally disordered
clients.

period from July 1, 2003 through June 30, 2004, for the- provision of
augmented board and care services for County-referred mentally
disordered clients.
Approval of Contract #24-681-24 (11) will allow the Contractor to

BACKGROUND/REASON(S) FOR RECOMMNDAT I ON(S) :
On May 20, 2003, the Board of Supervisors approved Contract #24-793-12
with Pine Tree Gardens, Inc. , for the period from July 1, 2003 through
June 30, 2004, to provide room and board and twenty-four hour emergency
residential care and supervision, to severely and persistently mentally
ill adults who are specifically referred to Facility Operator for service.
Approval of Contract #24-793-13 will allow the Contractor to provide
residential board and care services through June 30, 2005 .

BACKGROUND/REASON(S) FOR RECONlMNDATION(S) :
This Contract meets the social needs of the County' s population in that it
provides augmentation of room and board, and twenty-four hour emergency
residential care and supervision to eligible mentally disordered clients,
who are specifically referred by the Mental Health Program Staff and who
are served by County Mental Health Services.
On May 20, 2003, the Board of Supervisors approved Contract #24-681-15 (9)
with Diane Anglin (dba Scenic View) , for the period from July 1, 2003
through June 30, 2004, to provide augmented Residential Board and Care
Services for County-referred mentally disordered clients .
Approval of Contract #24-681-15 (10) will allow Contractor to continue
providing services, through June 30, 2005 .

BAC ON(S) FOR RECObMNDATION(S) :
On May 18, 2004, the Board of Supervisors approved Contract #24-984-8 with
Pradeep Kumar, M. D. , for the provision of professional outpatient
psychiatric services for the County' s Mental Health Department. The Board
Order reflected an incorrect payment limit. The correct payment limit
should read $107, 400 . The purpose of this Board Order is to correct the
payment limit to reflect the intent of the parties and allow the
Contractor to continue providing psychiatry services through May 31, 2005 .

to execute on behalf of the County as follows:
1 . Cancellation Agreement #26-830-25 with Paul Reif, M.D. , (Specialty:
Gastroenterology) , a self employed individual, effective on the close of business
on June 30, 2004; and
2 . Contract #26-830-26 with Paul Reif, M.D. , Inc. ('Specialty: Gastroenterology) , a
professional corporation, in an amount not to exceed $750, 000, for the provision
of professional Gastroenterology services for patients at Contra Costa Regional
Medical Center, for the period from July 1, 2004 through June 30, 2007 .
FISCAL IMPACT:
This Contract is funded by Enterprise I Budget. Cost to the County depends upon
utilization. As appropriate, patients and/or third party payors will be billed for
services.
BAC KGROUND/REASON(S) FOR RE C O14KENDAT I ON(S) :
On October 8, 2002, the Board of Supervisors approved Contract #26-830-24 with Paul
Reif, M.D. , to provide Gastroenterology services, including consultation, training, on-
call, medical and/or surgical procedures, and regularly scheduled weekly GI clinic
coverage, for patients at Contra Costa Regional Medical Center, for the period from
October 1, 2002 through September 30, 2005.
In consideration of Contractor' s agreement to continue providing services, both parties
have agreed to redefine the scope of services under the Contract. This will allow the
Contractor to be paid per clinic session instead of a monthly rate and to change the
legal capacity from a self-employed individual to a professional corporation. The
Department and the Contractor have mutually agreed to renegotiate the Contract Payment
Provisions and the term to reflect the intent of the parties. Therefore, in accordance
with Paragraph 3. (Term) of the Contract, the Department and Contractor have agreed to
a mutual cancellation of Contract #26-830-24. Approval of Cancellation Agreement #26-
830-25 will accomplish this termination.
Under Contract #26-830-26, Contractor will provide professional Gastroenterology
services, including, consultation, training, and medical and/or surgical procedures,
through December 31, 2006.
