SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION REC22MMATI ON(S) Approve and authorize the Health Services Director or his designee (Kathryn Crazzini) to execute on behalf of the County, Contract #26-878-8 with Concord Yellow Cab, Inc. , a corporation, in an amount not to exceed $75, 000, to provide taxicab transportation services to transport patients to and from Contra Costa Regional Medical Center and Contra Costa Health Centers and other health facilities, for the period from July 1, 2004 through June 30, 2005. BAC 9RQ=/REAS0N(S) EUR REQQNffX j Tj©N(S) Taxicab services are utilized when a clinical condition could be adversely affected by lack of transportation for needed medical care, including but not limited to, physical disabilities which make it unsafe for patient to travel on public transportation, patient has a verifiable seizure disorder, or patient has received mediation which has or could impair the mobility or coordination of the patient. On June 24, 2003, the Board of Supervisors approved Contract #26- 378-2, with Concord Yellow Cab, Inc. , for the period from July 1, 2003 through June 30, 2004, to provide taxicab transportation services to transport patients to and from Contra Costa Regional Medical Center and Contra Costa Health Centers and other health facilities. D. ) to execute on behalf of the County, Contract Amendment Agreement #26-443-4 with A 2 Z Printing and Translations, a Corporation, effective June 30, 2004, to amend Contract, #26-443 (as amended by Contract Amendment Agreements #26-443-1, #26-44.3-2, and #26-443-3) , to increase the total Contract Payment Limit by $25, 000 from $80, 000 to a new total payment limit of $105, 000 and to extend the term of the Contract through September 30, 2004 . FISCAL IMPACT: This Contract is funded by the Health Services Department' s Enterprise I . BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : In November 2002, the County Administrator approved and the Purchasing Services Manager executed Contract #26-443 (as amended by Contract Amendment Agreements #26-443-1, #26-443-2, and #26-443-3) with A 2 Z Printing and Translations, to provide patient interpretation and translation services for the Contra Costa Regional Medical Center for the period from July 1, 2002 through June 30, 2004 . The purpose of this Contract Amendment Extension is to continue the Contract so that services are not disrupted, pending the Department' s outcome and completion of current RFP process for language assistance. Approval of Contract Amendment/Extension Agreement #26-443-4, will allow Contractor to provide additional translations services for patients at Contra Costa Regional Medical Center, through September 30, 2004 .
