SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
1. OPEN and conduct a public hearing on the closure of the Contra Costa Regional Medical
Center Operating Room#3 in accordance with Health and Safety Code Section 1442.5
(Bielenson Act).
2. CONSIDER public testimony and the staff report on the closure of Operating Room#3.
3. DECLARE that the Board of Supervisors has also considered public testimony and the staff
report on the closure of Operating Room#3 at the April 20,2004 budget workshop.
4. RECOGNIZE that the Health Services Department is facing a$26.4 million shortfall for
fiscal year 2004/2005, and that closure of Operating Room#3 will result in $500,000
savings to the General Fund.
5. AUTHORIZE the Health Services Director to initiate the closure of the Contra Costa
Regional Medical Center Operating Room#3, as recommended by the Health Services
Director.
6. DIRECT the Health Services Director to arrange for appropriate scheduling of patients being
served by Operating Room#3.
7. RECOGNIZE that the Health Services Director will ensure that all appropriate State and
federal laws and regulations are complied with in closing the Operating Room.
FISCAL IMPACT:
Closure of Contra Costa Regional Medical Center Operating Room#3 will result in a savings of
$500,000 to the General Fund.

On this date the Board closed a public hearing on the closure of the Contra
Costa Regional Medical Center Operating Room #3 in accordance with
Health and Safety Code Section 1442.5 (Bielenson Act);
The Board discussed the history of the issue and requested
clarifications of various relative items from staff.
The Chair invited comments from the public, and no one appeared to speak
After farther discussions the Board took the following action:
• DECLARED that the Board of Supervisors consider public testimony
and the staff report on the closure of Operating Room#3 on April 20,
2004;
• RECOGNIZED that the Health Services Department is facing a $26.4

Dear Supervisor Gioia:
What would you think if you saw a headline in the Times that read: "Contra Costa
County Budget Proposal: How to save $100K at the cost of $1.2M"?.

.ra.7,, . Or,Impossible one might say. Yet,that is exactly what line item No. 27 of the
proposed programs cuts' spreadsheet by CCHS will do if the Board were to approve it.
A proposal to the Board of Supervisors to be reviewed for final approval by Mid-May,
2004 states that the impact from the cuts"eliminates volunteer recruitment and
community outreach for non-paid support for hospitaL Reduction in available
volunteer hours(quantity not specified). Reduces pardc1pation in career to work
programs(school to work progrann)." In layman's terms, the Volunteer Program,
considered discretionary, is on the chopping block to save the annual salary of the single
individual employee of the department. The Coordinator.
When Dr. Smith presented the original budget reduction list in mid April, and was asked
to clarify the Volunteer program status in particular,he indicated that the program was
"not specy1cally on the list because its loss would be a minimal financial savings.
However, if the coordinator is lost,I doubt that the Program would stay."That made
sense at the time. Since then however, it appears a decision was made to dismantle the
program, eliminate the coordinator's paid position,and add what is left of the program(if
anything)to someone else's responsibilities.
Without going into lengthy statistical data,I will succinctly delineate the basis for the
"headline"above.
Official County statistics reported during the recent National Volunteer Week revealed

for the calendar year 2003,the Contra Costa Health Services Volunteer Services
ces
Program, excludina the Auxiliary segment of the Department, provided 28,000 hours of
free labor and services to the County. This constitutes 93% of the total time worked by
volunteers. The Auxiliary provided the re wining 2,000 hours or 7%of the total. The
nature of Auxiliary volunteer time is different,thus the distinction. Additionally,the
Auxiliary program does not appear to be in jeopardy,whereas ours is.
The formula to figure the value of volunteer time to the County is rather simple and
straight forward: Take the 28,000 hours, apply the dollar value of volunteer time at
$16.54 per hour,, as set by the Bureau of Labor Statistics. Add 12%for fringe, and the

result is that the Volunteer program at CCHS provided the w6rk of 13.8 FTEs(Full Time
Equivalent workers). The total annual dollar value and savings to the County was
$518;694. .In-addition, if any of these services provided by the volunteer program were
cut or.clirninated,the cost of replacing that free volunteer labor(at all levels)would have
a significant monetary impact on the nursing staff and departments-dependent on
volunteer support. By quantifying that impact,we arrive to an aggregate dollar value in
the neighborhood of $1.2M!
A core group of volunteers have chosen me to advocate for and speak on behalf of
maintaining*the integrity of the Volunteer Program at Contra Costa County Health
Services. Encouragement has also come from the community at large: residents,
taxpayers,.and concerned citizens. Driving the scrutiny of the County's budget cats is the
increasing anger and outrage in the community about the handling of such: First was
Education, now is Health Services. What next? To quote a recent bumper sticker "If
you aren't completely appalled,then you haven't been payn�a attention"
We.can certainly provide much more detail if needed. For the purposes of this letter
however, suffice to say that a decision to in any way cut, reduce, or obliterate an already
short-staffed albeit, successful program,makes no fiscal sense whatsoever.
Consequently, we urge you and all the members.of the Board to act prudently when
reviewing this stern in the County's proposal. No one would life to see our name
associated with that"headline" in the Times. I mention it because there has been a
newsnaner--retorter interested in the very subject who has been asking questions. I
believe that sooner than later answers will have to be provided.

NOTICE OF PUBLIC HEARING
The Board of Supervisors of the County of Contra Costa will conduct a PUBLIC
HEARING pursuant to Health and Safety Code Section 1442.5, on the elimination of
and/or reductions in-the level of medical services provided by the County, and/or the
closing of County health facilities. The hearing will be held at the following time and
place:
Tuesday, May 25, 2004 at 3:00 p.m.
Board of Supervisors Chambers
Room 107, Administration Building

At the hearing, the Contra Costa County Board of Supervisors will consider the impact
on the health care needs of the County's indigents of the implementation of the changes
set forth in the attached Notice.
The Board does not consider all of the program changes in the attached Notice to be
within the required coverage of Health and Safety Code Section 1442.5. Its scheduling
of this Hearing, giving this Notice of Public Hearing, the holding of a Hearing, and the
making of any findings, shall not be construed as an admission by the County of Contra
Costa, or the Board of Supervisors, that any of the actions included in the attached
notice are subject to the hearing provisions of Health and Safety Code Section 1442.5.
