Per California Department of Aging Agreement(MS-0405-27) for the Multipurpose Senior Service Program
(MSSP), County to receive funding in the amount of$631,057. No County match required.
BACKGROUND,
The California Department of Aging funding is yearly funding accepted by the Department on behalf of the
County to provide case management on a countywide basis for Contra Costa County residents age 65 and over,
Medi-Cal eligible, and certifiable for skilled nursing facility care.
The Multipurpose Senior Service Program (MSSP)provides case management, assessment, care planning,
service arrangement, and client monitoring through a team of health and social service providers. The provision
of case management services to this senior population allows individuals who may otherwise require
institutionalization, to remain safely in their own residences.

$4"7,373 from the State Department of Health Services-Tuberculosis (TB) Control Branch,
for the Tuberculosis Consortium Task Order 47, for the period from March 1, 2404 through
December 31, 2004.
FISCAL SACT:
Acceptance of this Award will result in an amount not to exceed $47,373 from the State
Department of Health Services-Tuberculosis Control Branch. (No County funds are
required)

Contra Costa County maintains programs to control Tuberculosis for all residents of the
County who need this service. The Tuberculosis Control staff works in the Communicable
Disease Control Unit. Funding for this program will allow the unit to monitor the
incidence of tuberculosis, follow cases and their contacts and test people at high risk,
through clinics, field services and coordination with private providers. The goal of the
Tuberculosis Control Program is to prevent the occurrence and transmission of
tuberculosis through early diagnosis and treatment of disease and recognition/treatment
of contacts to active cases.
Three sealed and certified copies of the Board should be returned to the Contracts and
Grants Unit.

California, Department of Alcohol and drug Programs

RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee (Haven Fearn) to execute, on
behalf of the County, Standard Agreement (Amendment) #29-502-12 with the State Department of
Alcohol and Drug Programs, to amend Standard Agreement #29-502-6 (as amended by subsequent
amendments x29950247 through #29- 502-11) effective July 1, 2003, to reflect a decrease by the
State and. Federal Drug Medi-Cal funding, in an amount not to exceed $2,348,547 and an increase
Federal Block Grant funding in an amount not to exceed $661,082 for Fiscal Year 2003-20104,
witi a new payment limit of 810,632,848.
FINANCIAL IMPACT:
Approval of this (Amendment) agreement will decrease the funds by $1,687,465 from the State and
Federal Drug/Fled--Cal as indicated below:

BACKGR.C7TtTMLIIEASO N(S) FOR. RECOMMENDATION(S),:
On July 24, 2001, the Board of Supervisors approved Standard Agreement ##29-502-6 (as
amended by subsequent amendments ##29-502-7 through #29--502-11) with the State Department
of Alcohol, & .Drug Programs, for the period from :July 1, 2001 through June 30, 2005
(expending Fiscal Years 2003-2004 Federal funds is extended through June 30, 2005) , for
the provision of Substance Abuse services.
Approval of this Standard. Agreement (Amendment) #29-502-11 will guarantee that the County
will continue to receive the full allocation amount included in the Agreement and allow
the Department to continue providing needed Substance Abuse Services through June 30,
2005.
Three sealed and certified copies of this Board Order should be returned to the Contracts and
Grants ' nit for submission to the State.

RECDED ACTION:
Approve and authorize the 1-ieaith Services Director or his designee (Maven Fearn) to
execute, on behalf of the County, Standard Agreement #29-502--13 with the State Department
of Alcohol and Drug Programs, in an amount not to exceed $10,713,056: for the period from
July 1, 2004 through June 30, 2008, to receive State General Funds, Federal Block Grant
allocations and D'rUg/Medi-Cal monies to fund approximately 72W of Alcohol and other Drug
Services.
FINANCI.AI -IMPACT:
Funding for this agreement is based on the Governor's proposed budget :for Fiscal Year
2004-2005 in an amount not to exceed $10,713,056 for the first year of a four year
agreement. :Expending of Fiscal Year 2006-2007 Federal funds is extended through June 30,
2008. The following is a summary of the ,State and Federal funding covered under this
Aareement:

This Standard Agreement #29-502 is a combined Negotiated Net Amount (NNA) and. Drug/Medi-
Cal contract, Bike the NINA Agreement (effective July 1, 2001 through ,,;rune 30, 2005) ,
this new Agreement requires counties to provide Drug/Meda.-Cal services up to their full.
State General. Fund allocation for Drug/Medi-Cal match. 1f the required services exceed
the allocation, counties`may access a Drug/Medi-Cal reserve set aside for this purpose.
Approval of this Standard. Agreement #29-502-13 will guarantee that t-he County will
continue to receive the full allocation amount included in the Agreement and allow the
Department to continue providing needed Substance Abuse Services.
Three sealed and certified copies of this Board order should be returned to the Contracts
and Grants Unit for submission to the State.

APPROVE and AUTHORIZE the Director of the Building Inspection Department or
Designee, to execute a contract with C A. Cola General Enaineerinq, to demolish the
property located at, 1114 Court Street, Martinez, CA in an amount not to exceed
$27,144.00 for the period of May 25, 2004 to May 25, 2005.
FISCAL IMPACT
Funding of $50,000.00 is found in budget unit #0030, Non-Capital Improvements.
BACKGROUND/REASONS FOR RECOMMENDATIONS
The building to be demolished is a County owned building. It is a 3375 square foot, two-
story residence including foundations, hazardous materials, utilities and site
appurtenances. The site will be restored to the original condition to mala preparation for a
future parking lot. On April 13, 2004, the Building Inspection Department solicited bids
from thirty-two (32) contractors to perform the abatement and received bids from four (7)
contractors. C. A. Cola General Engineering was the lowest responsible bidder. C.A. Cala
General Engineering is a local business.
