Fiscal Impact: None Background: CCFuture created a new framework for identifying, implementing and financing services which improve the well-being of children and families. With foundation support, the research-based, client- focused programs Welcome Home Baby and Spirit of Carina have tested the application of the CCFuture children's prevention strategy investment tools. The CCFuture Investment Strategy has demonstrated the efficacy of collaborative results-based, family-oriented early intervention/prevention services in improving family functioning and reducing the high cost of dependency. CLOSE out CCFuture as a referral to the Family& Human Services Committee. Backgroundaeasons for Recommendations: Contra Costa County has a longstanding commitment to improving the well-being of children and families. In the early 1990s, the Board of Supervisors adopted Healthy Families 2000, which articulated the need for expanded early intervention/programs that revolved around the needs of children and families. The publication of the County's first Children's Report Card in 1997 infused results-based accountability into the County's approach with the adoption of community outcomes and measurable indicators of the well-being of children and families. Contra Costa Future{CCFuture}investment Strategy brought these concepts together in a comprehensive systems approach characterized by: 3> Continuums of Care Community and family focused services Integrated and collaborative services Results accountability Data driven service delivery planning and design Funding that is maximized, leveraged and aligned This new approach recognized the County's fiscal and regulatory constraints: Rigid,fragmented crisis services- State and federal funds are primarily directed at crisis, not prevention., early intervention or remediation. With little discretion over eligibility, services or service parameters, the County has little discretion on how to best meet client needs. Family needs not paramount- Unfortunately, most families in county systems face multiple barriers to independence. Consequently, they need services tailored to meet their individual family needs. Limited County resources- Since most County revenues are used to meet state and federal mandates, very little is left for local priorities. Consequently, allocations must be strategic. CCFuture created a new framework for identifying, implementing and financing services. With foundation support, two research-based, client-focused programs have tested the application the CCFuture children's prevention strategy investment tools. These pilots are Welcome Rouge Baby -In-home visitation to all first-time parents. Program goals. increased well baby care; higher immunization rates; enrollment in a primary health home; improved parenting skills; and reduced abuse and neglect, thereby reducing out-of-home placement. ,Spirit o,f Caring—Wraparound services for children and adolescents with serious emotional and behavioral difficulties. Program goals: improved ability of youth to function in society, succeed in school and remain at home, thereby reducing the need for/length of stay in out-of-home placement. Each of the pilots has aligned its program with the CCFuture parameters. Each installed program performance software to monitor, document and guide program activities and successes. Each program also focused their efforts on leveraging discretionary County dollars to maximize all eligible, categorical funds from the state and federal government. As a result, Welcome Horne Baby is now able to access targeted case management federal Binding through the Health Services Department, The Spirit of Caring is currently in the process of applying for a state SB 163 Child Welfare Waiver, which will allow it to draw down state and federal dollars normally available only to the Child Welfare System. Welcome Home Baby and Spirit of Caring will be reporting on the status of their programs, including their results in improving client functioning, satisfying client needs 2 and reducing out-of-home placements together with data on estimated cost savings to the County. Those reports are attached as Power Point presentations. Program performance software utilized by Welcome Home Baby and Spirit of Caring have helped them identify specific service gaps and needs for their client base. On a larger scale, the Data Archive will guide new service development/expansion within communities. Using real client data from various human service programs which is then stripped of all identity and made anonymous, the Data Archive will be able to help identify targeted interventions that align services within individual community needs. Currently, the Data Archive contains Children's Mental Health data from 1995 to 2004, Probation and CaIWORKs data through 2001 and Mt. Diablo and West Contra Costa School Districts data through 2000. Over the past year, the Data Archive was"moved" to Children's Mental Health for oversight and management. Carryover funds from the Stuart Grant are being used to support development of the data refreshing function and for a more user-friendly configuration. Over the past four years, CCFuture has been financially supported with grants from the S. H. Cowell Foundation, East Bay Community Foundation and Stuart Foundation. On March 31, 2004, the County submitted its final report for grant funding. We were very pleased to report success in development of CCFuture, demonstrating the value of results-based early intervention and prevention programs in improving the lives of children and families and reducing County costs for crisis/safety net services. With the conclusion of these grants, it would be an appropriate time to close out this referral to the Family and Human Services Committee. 3 ;.: Wel C.C.C.
