SUBJECT: Update on the Services Provided by the Mental County
Health Division at the Crestwood Behavioral Health, Inc. A
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
ACCEPT update from the County Administrator and the Department of Health Services,
Mental Health Division, regarding the management and security of the Crestwood facility
and the status of its operation.
FISCAL IMPACT:
No financial impact.
BACKGROUND:
Crestwood's Pleasant Hill Mental Health Rehabilitation Center received the County's first
three consumers on October 10, 2003. As of the end of November, there were fifteen (15)
consumers residing at the facility. When we reported to you on December 9, 2003, there
had been no negative incidents at the facility, and no reports of negative impact on the
neighborhood.
Since it opened in October 2003, there have been 35 consumers referred to the Center.
There are currently 26 residents, with two in college and three in vocational training. There
have been nine people discharged, three to board and care homes, four to out-of-county
locked facilities and two to home.

To date, staff at Crestwood Behavioral Health, Inc. decided to hospitalize a consumer
under Health and Welfare Code 5150 one time and has removed three others from the
facility.
• On January 20, 2004 a consumer failed to return to the designated meeting location
following an outing to a bookstore. Crestwood staff conducted a search of the
immediate vicinity but was unable to locate the consumer. The staff then notified
their management staff at the Pleasant Hill facility and was instructed to return the
group to the facility without the missing consumer. The facility contacted the
Pleasant Hill Police Department. The consumer subsequently returned to the facility'
on his own six hours later. A toxicology screening was immediately performed as
required. Can January 26, 2004 the consumer was subsequentlysent to another
care facility and the State Department of Mental Health was notified. The
Neighborhood oversight Committee was informed of the incident.
• On February 14, 2004 two consumers got into a verbal argument in a room they
shared. Crestwood staff was unable to calm down one of the consumers. Pleasant
Hill Police were called by the Crestwood staff and responded to the facility. The
consumer was removed from the facility under Health and Welfare Code section
5150. This was an "in-facility" incident. Because of this, a report to the State
Department of Mental Health is not required. Reports of incidents inside the facility
are not routinely reported to the Neighborhood Oversight Committee. The
Committee was informed of and agreed to this in December 2003.
• On February 18, 2004 a newly arrived consumer decided he wanted to leave the
facility the day after he was admitted. A staff member saw him leave and followed
him in her car while he walked to a local restaurant. Before approaching the
consumer the staff member decided to request the assistance of the Pleasant Hill
Police. The consumer was returned to the facility and later voluntarily returned to his
previous placement. Crestw000d's understanding is that the consumer was not
AWOL as he was never out of the line of sight of a staff member, However,
Crestwood decided to request a determination from the State Department of Mental
Health on whether an incident of this type should be considered an AWOL.
Because Crestwood does not consider the consumer AWOL, the Neighborhood
Oversight Committee has not been notified. If the State determines that the incident
should be treated as an AWOL, the Committee will be notified.
One consumer was returned to the previous facility without incident. The Crestwood
staff and the consumer agreed that returning to the previous facility was in the
consumer's best interest.
Crestwood is notifying the County of all incidents. Crestwood is also making an effort to
inform the Neighborhood Oversight Committee and the Pleasant Hill Police Department as
appropriate. The Pleasant Hill Police Department is also keeping the Pleasant Hill city staff
informers. I believe that the staff at the Crestwood Pleasant Hill Healing Center is making a
concerted effort to meet the intent of their agreement with the residents.'
As you know, consumers who are referred to the Crestwood facility have already
undergone treatment for several months at a higher level of care and are ready to reenter
their community. Licensed mental health staff carefully review potential candidates to
determine if they are appropriate for placement in this facility (for example, clean and
sober, medication compliant, no recent history of assaultive behavior, and some success in
developing daisy living skills).
There has been one meeting of the Neighborhood Oversight Advisory Committee.
Crestwood and County staff were present at this meeting. The next meeting is scheduled
for mid-March 2004. Representatives from the Mental Health Division continue to be
available to the Neighborhood Committee when requested.
