SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
Approve and authorize the Health Services Director, or his designee(Steve Tremain, M.D.)to execute
on behalf of the County, Contract#26-993.5 with Russell Schulze, M.D., (Specialty: Orthopedics), a
self-employed individual, in an amount not to exceed$93,000, for the period from June 1, 2004
through May 31, 2005, to provide professional Orthopedic Surgery services to Contra Costa Regional
Medical Center.

Cost to the County depends upon utilization. As appropriate, patients and/or third party payors will
be billed for services.

For a number of years the County has contracted with Medical and Dental Specialists to provide
specialized professional services which are not otherwise available in its hospital and clinics.
On June 24, 2003, the Board of Supervisors approved Contract#26.993.3 (as amended by Contract
Amendment Agreement#26.993.4),with Russell Schulze, M.D., to provide Orthopedic Surgery
services, including,but not limited to, consultation, clinic coverage, training, and medical and/or
surgical procedures, for Contra Costa Regional Medical Center and Contra Costa Health Centers, for
the period from June 1, 2003 through May 31, 2004.
Approval of Contract#26-993-5 will allow Contractor to continue to provide Orthopedic services
through May 31, 2005.

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION($):
Approve and authorize the Health Services Director, or his designee (Milt Camhi) to execute on
behalf of the County, Contract #27-528-2 with CorVel Corporation, a corporation, in an amount
not to exceed$300,000, to provide claims processing and negotiation services for the Contra Costa
Health Plan(Health Plan) for the period from May 1, 2004 through April 30, 2005.
FISCAL IMPACT:
This Contract is funded by Health Plan member premiums. The Contractor will be paid for its
services on a contingent fee basis at a rate not to exceed 30% of the amount that County saves due
to the Contractor processing the claim.
BACKGROUND/REASON(S)FOR RECOMMENDATIONN:
The Health Plan is obligated to provide its members with emergent, urgent and specialty
healthcare services, which may require healthcare outside of the Health Plan's Provider Network.
On May 13, 2003, the Board of Supervisors approved Contract #27-528-1 with CorVel
Corporation for the period from May 1, 2003 through April 30, 2004, to provide claims
processing and negotiation services, on behalf of the County, to secure discounted rates for out-
of-network provider claims. The Contractor is paid on a commission basis for any amount that
County saves as a result of negotiation services.
Approval of Contract #27-525-2 allows the Contractor to continue providing services through April
30, 2005.

RECOMMENDATIONN:
Approve and authorize the Health Services Director, or his designee (Milt Camhi) to execute on
behalf of the County, Contract #27-577 with Mercer Human Resource Consulting, Inc., a
Corporation, in an amount not to exceed $150,000, to provide actuarial consulting services for the
period from March 1,2004 through February 29, 2005.
FISCAL IMPACT:
This Contract is funded 100%by Contra Costa Health Plan member premiums.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
The Health Services Department and the Contra Costa Health Plan have an obligation to provide
certain State contracts with professionally certified actuarial documents. In April 2003, the
County Administrator approved and the Purchasing Services Manager executed Contract 423-347
(as amended by Contract Amendment Agreements #23-347-1 and #23-347-2) with Mercer
Human Resource Consulting for the Department's Office of the Director, for the period from
March 1, 2043 through February 29, 2004.
Approval of Contract #27-577 will allow the Contractor to provide actuarial consulting services for
the Contra Costa Health Plan,through.February 28, 2045.

Subject; REQUEST FOR RETROACTIVE DATE
1. Request to make retroactive the effective date for the proposed contract between
Contra Costa County(CCHP) and Mercer Human Resource Consulting.
2. The Health Plan has been utilizing the professional actuarial services of Mercer
Human Resource Consulting under Contract 23-347-1. An oversight during the
contract period resulted in the contract not being put into the renewal sequence for the
Health Plan. Subsequently,the contract lapsed on February 29,2004 without a
pending contract. This oversight has been corrected. The renewal contract
accompanies this memo.
3. Because a contract was anticipated,work was provided by Mercer Human Resource
Consulting since March 1, 2004. Your approval will allow the Health Plan to have a
contract in place that will cover this time period.
4. Based on the above,request your support and approval to proceed with the contract
having an effective period from March 1, 2044 through February 28,2005.
