Chertkow and told to contact Deputy Gilbert regarding personal property that had been accidentally damaged at the screening post 4. At about 1030 hours I contacted Deputy Gilbert and she told me the following. Gilbert was standing at the post 4 x-ray screening machine in the Bray Criminal Building when Victim Opong'Mensah came into the building. As per the policy of the Sherifrs Office and the Court, he placed a large leather bag on the machine conveyor belt so it could be x-ray scanned for potential weapons. At the time the machine was off. When Gilbert started it, the belt moved backwards 'a few inches for some unknown reason. Thi; caused the ba `0 g to fall off the machine and land on the floor about 30 inches bela0i Opong'Mensah immediately yelled at Gilbert accusing her of breaking his computer. -Gilbert had him remove the laptop computer from the bag and turn it on to see if it was damaged. It was turned on and apparently was working fine. Opong'Mensah then took the computer and bag into the Law Libra*ry. A few minutes later he returned with his computer and showed it to Gilbert, stating the mouse.controller was no longer working. Gilbert confirmed this, however she wasn't sure if the fall caused it to be inoperative. asked Gilbert why the conveyor had moved backwards. Shb*had no idea. It occurred on September 26,2003,Dr. Richard Gurley,M.D. pronounced my son dead at approximately 10:11 a.m, of a possible Seizure Syncopal Disorder. 2. Where did the Damage or Injury? In the County of Contra Costa at the Contra Costa Regional Medical Center, 2500 Alhambra Avenue, Martinez, CA 94553. 3. How did the damage or injury occur? On September 21,2003 my son was picked up by the police on a 51/50 but was later released into my custody because the police said he knew where he lived and was coherent. My son asks that he be taken to the hospital. I and my husband took him straight to Contra Costa Regional Medical Center and soon after our arrival Brian was admitted to the Psychiatric Ward. Brian's case worker Robert Martinez went to visit him on September 23,2003 and was concerned about his condition. He advises that we contact the doctor in charge of Brian's treatment because there was some confusion about the medications he's taken. I contacted the Psychiatric Ward immediately;the doctor in charge of Brian's treatment was not available; so I ended up speaking with a Dr.Robyn Draper,PHD who was familiar with Brian's course of treatment. I spent quite some time on the phone with her given her all the names of the medications,dates and amounts to be taken. My husband went to visit Brian on September 25, 2003 to take him some clean clothes. Brian was walking around and talking with his father. On September 26,20031 received a phone call from Sylvia Luz, R.N.the Director of Psychiatric Nursing at C.C.R.M.C., she said my son was having a medical crisis and that I needed to get there. I arrived about 20 minutes later and was told by Sylvia Luz that my son didn't make it I was taken to a room where Dr. Richard Gurley,M.D. arrived to inform me and family members that he didn't know what had caused my son's death. Dr.Richard Gurley,M.D. said my son died suddenly and that it was unusual for something like this to happen in someone so young and healthy;he recommended an autopsy be done do to this reason. We finally agreed to an autopsy being performed and were told by Dr.Richard Gurley, M.D. that our son's death would be considered a coronet's case. We called the Coroner's office on September 29,2003 to find out when Brian's body was going to be released and to our StjjWIZ4,-and discouragement we were told Brian's body never reached their office. Deputy Brinks of the Contra.Costa Coroner's office informed me that Dr. Richard Gurley, M.D. called him back that after noon of September 26,2003 and told him Brian's death was no longer considered a Coroner's case. Dr.Richard 2 Exhibit 11 Continue Questions 3-4 Gurley,M.D.had stated he was going to sign off on the death certificate. I put in a request for a copy of my son's medical records and autopsy report, I finally received them and after careful review I find that on September 25,2003 Dr.Truman Wong ordered 8mg of Risperdal along with two doses I 00mg of Zoloft be administered in the a.m.; and again in the afternoon he was given another 8mg of Risperdal. Also,on that day he was given 30 ma of Zyprexa(10 mg)each. My son was pronounced dead the next day. After reviewing the Autopsy and Toxicology report no where is it stated there was any sign of seizure,brain stem a or hewt failure in the pwlir i-inary reports and or any conclusive reports. On December 29,2003 1 wrote a letter to Dr. Richard Gurley,M.D. to request that additional lab tests be performed on Brian's remaining tissue and blood samples;reason being from reading the medical records and autopsy report no levels were ever given or taken on any of the medications Brian received during his treatment at C.C.R.M.C. I believe my son's death was caused by a drug overdose of the medications administered to him during the course of his treatment. The medications caused the seizure disorder which resulted in torsades de pointes and or sudden cardiac death. 4. Other particular acts would be that of the following medications listed that were Iministered during the course of Brian's.&---d— rt on the Psychiatric ward. I. Risperidone(Risperdal)—Side effects hypeipyrexia,muscle rigidity,altered status,autonomic instability(i.e.,irregular pulse or BP,tachycardia, diaphoresis,and cardiac dysrhythmia). CV: Prolongation of the QT interval that might lead to ton des de pointes and sin death. Overdose management symptoms: Exaggeration of known effects,especially drowsiness,sedation,tachycardia,hypotension and extra . sympkwm Drug indications: Clozapine/Risperidone clearance following chronic use of clozapine. Dosage: Adults initial I mg b.i.d.once daily. Can be increased by 1 mg b.i.d.on the second and dtird days as tolerated. Furdw w- creases m dose should occur at intervals of about I week. 11.
