FISCAL IMPACT: TvTonne, This Contract is funded 100% by Federal "Drug Free Schools" through the California Department of Alcohol and Drug Programs(ADAP). BACKGROUND/REASON(S))FOR RECOMMENDATIONN: On January 6, 2004,the Board of Supervisors approved Contract##74-207--1 with Center for Human Development for the period from January 1, 2044 through December 31, 2004; for the provision of Alcohol and Drug Abuse Primary Prevention Program Services to County referred residents. Approval of Contract Amendment Agreement#74-207-2 will reform the Contract to make technical adjustments mandated by the State ANAP, and will allow the Contractor to continue providing services through December 31,2004. Haven Fearn 0 Alcohol and tither Drugs Services, director RE: SDFSC Contract Amendment DT: March 24, 2004 The purpose of this letter is to request a contract amendment for the two Safe and Drug Schaal Free Schools and Communities (SDFSC) Contracts: #74-2.06-1 and #74-2.07-1. Both contracts were approved by the Board of Supervisors last January based on fee for service, to deliver AOD primary prevention services to the residents of Bay Point and Rodeo under the No Child Left Behind Act. The two Contracts are 100% Federally funded. No county funds are required, no changes in payment limit, and there are no changes on program delivery. The terms and conditions of the SDFSC County Grant program, estipulate reimbursement of the federal funds allocated for the services in arrears, for actual allowable costs incurred under this grant program. The State Alcohol and Drug Programs (ADP) has requested that we amend both contracts from a fee for service to cost reimbursement effective January 1, 2004. D. , a self-employed individual, (specialty: Gastroenterology) , effective May 1, 2004, to amend Contract #26-911-15, to increase the total Contract Payment Limit by $75, 000 from $25, 000 to a new total of $100, 000, with no change in the original term of the Contract through January 31, 2005. On March 23, 2004, the Board of Supervisors approved Contract #26- 911-15 with Thomas B. Hargrave III, M. D. , for the period from March 1, 2004 through January 31, 2005, to provide professional Gastroenterology services, including clinical coverage, teaching, and medical and/or surgical procedures. BACKMOT TI ' IREAA,, ON(S) OR-REC�''y€M NT7A f T(CN411g): On January 27, 2004, the Board of Supervisors approved Contract#27.277-4 with Kaiser Foundation Health Plan, Inc., for the period from October 1, 2003 through September 30, 2004, for the provision of health care services for Medi-Cal recipients enrolled in Kaiser Foundation Health Plan. Kaiser Foundation Health Plan has been a traditional provider of health care services to Medi,Cal beneficiaries in Contra Costa County. With the implementation of the Local Initiative for Medi, Cal managed care, Kaiser, in order to continue to enroll Medi-Cal members, must affiliate with the Local Initiative. The affiliation arrangement allows Medi-Cal recipients to choose Contra Costa Health Plan as their managed care plan, and designate Kaiser as their provider. Approval of Contract Amendment Agreement#27.27 75 will allow the Contractor to provide services to additional Medi-Cal recipients, through September 30, 2004. March.23, 2004 County SUBJECT: Approval of Contract Amendment Agreement#27528--3 with CorVel Corporation /W1 SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Milt Camhi) to execute on behalf of the County, Contract Amendment Agreement #27-528--3 with CorVel Corporation, a corporation, effective April 1, 2004 to amend Contract#27-528-1 to increase the Contract payment limit by $75,000, from $200,000 to a new total of$275,000 with no change in the Contract term, through.April 30,2044. FISCAL IMPACT: This Contract is funded by Health Plan member premiums. The Contractor will be paid for its services on a contingent fee basis at a rate not to exceed 30%of the amount that County saves due to the Contractor processing the claim. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): On May 13, 2003, the Board of Supervisors approved Contract #27-528-1 with CorVel Corporation for the period from May 1, 2003 through April 30, 2004, to provide claims processing and negotiation services, on behalf of the County, to secure discounted rates for out- of-network provider claims. The Contractor is paid on a commission basis for any amount that County saves as a result of negotiation services.
