RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Milt Camhi) to execute on
behalf of the County, Contract #27-4.37-5 with Dominic Biunno, Jr., a self-employed individual,
in an amount not to exceed $74,041, to provide consultation and technical assistance to the
Department with regard to Contra Costa Health Plan(Health Plan)provider contracts for the period
from June 1, 2004 through May 31,2005.
FISCAL IMPACT:
This Contract is funded 100%by Health Plan member premiums.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
On May 6, 2003, the Board of Supervisors approved Contract#27--437-4 with Dominic Biunno,Jr.,
for the period from June 1, 2003 through May 31, 2004, for the provision of consultation and
technical assistance with regard to Health Plan provider contracts, including developing requests for
proposals, negotiating with prospective contractors, developing measurable outcome-oriented
objectives,and monitoring regulatory requirements and Health Plan compliance.
Approval of Contract #27-437-5 will allow the Contractor to continue providing services through
May 31,2005.

Approve and authorize the Health Services Director,or his designee(Milt Camhi), to execute on behalf of the
County, Contract #27-503-3 with Elizabeth Milnes, Psy.D., a self-employed individual, in an amount not to
exceed $35,000, for provision of professional outpatient psychotherapy services for Contra Costa health Plan
members, for the period from May 1, 2004 through April 30, 2006.
FISCAL IMP& T:
This Contract is funded by Contra Costa Health Plan member premiums. Costs depend upon utilization. As
appropriate, patients and/or third party payors will be billed for services.

The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and group Health Plan membership contracts with the County.
In April 2003,the County Administrator approved and the Purchasing Services Manager executed Contract#27-
503-2 with Elizabeth Milnes,Psy.D., for the period from May 1, 2003 through April 30, 2004.
Approval of Contract #27-503-3 will allow the Contractor to continue to provide professional outpatient
psychotherapy services to Health Plan members, through April 30, 2006.

RECOMMENDATIUN(S):
Approve and authorize the Health Services Director,or his designee(Donna Wigand)to execute on behalf of the
County, Contract Amendment Agreement #74-083-9 with La Cheim School, Inc., a non-profit corporation,
effective April '1, 2004, to amend Contract #74-038-8 to increase the total Contract Payment Limit by $75,000
from$96,696 to a new total payment of$171,696 and to increase the six-month automatic extension by$37,500
from$48,348 to $85,848.
FISCAL IMPACT:
This Contract is funded 50%by Federal Medi-Cal, 48% by State EPSDT, and 2% Mental Health Realignment
Funds.
BACKGROUND/REASON(S)FOR RECOMMENDATION($):
The State Department of Mental Health has been working in collaboration with the County's Mental Health
Division to establish a mandated program to provide Therapeutic Behavioral Services(TBS)to children ages 0-
21 years of age, who participate in the Early and Periodic, Diagnosis, and Treatment (EPSDT) Supplemental
Specialty Mental Health services. This program will provide services to eligible Medi-Cal beneficiaries under
21 years of agewho meet the Mental Health Plan medical necessity criteria, who are members of the certified
class and who meet the criteria for needing these services.
4n September 23, 2003, the Board of Supervisors approved Novation Contract #74-083-8 with La Cheim
School, Inc. for the period. from July 1, 2003 through June 30, 2004, (which included a six-month automatic
extension through December 31, 20043)for the provision of Therapeutic Behavioral Services (TBS) to County
referred clients ages 0-21 years of age.
Approval of Contract Amendment Agreement#74-083-9 will allow the Contractor to provide additional units
of services through June 30,2004.

BACKGROUND/REASON(S)FOR.RECOMMENDATIO S .
On 'January 6, 2004, the Board of Supervisors approved Contract #74-206—I with Bay Area
Community Resources for the period from January 1, 2004 through December 31, 2004, for the
provision of drug abuse prevention services in West County for the Rodeo Project.
Approval of Contract Amendment Agreement#74--206-2 will reform the Contract to make technical
adjustments mandated by the State ADAP, and will allow the Contractor to continue providing
services through December 31, 2004.

RE: SDFSC Contract Amendment

The purpose of this letter is to request a contract amendment for the two Safe and Drug School
Free Schools and Communities (SDFSC) Contracts: #74-206-1 and #74-207-1. Both contracts
were approved by the Board of Supervisors last January based on fee for service, to deliver AOD
primary prevention services to the residents of Bay Point and Rodeo under the No Gild Left
Behind Act. The two contracts are 100% Federally funded. No county funds are required, no
changes in payment limit, and there are no changes on program delivery.
The terms and conditions of the SDFSC County Grant program, estipulate reimbursement of the
federal funds allocated for the services in arrears, for actual allowable costs incurred under this
grant program. The State Alcohol and Drug Programs (ADP) has requested that we amend both
contracts from a fee for service to cost reimbursement effective January 1, 2004.
