The Multi-purpose Senior Service Program(MSSP)provides case management on a countywide basis for Contra Costa County residents age 65+years who are eligible for Medical and certifiable for skilled nursing care facility. MSSP provides case management services,which include assessment, care planning, provision of services, and client monitoring to assist clients to maintain independence and avoid instutionalization. Affordable Home Care Agency provides personal care,protective supervision services, and respite in-home services to individuals referred by case managers. Services support the clients' ability to remain independent by providing assistance with household tasks, non-medical services for daily living activities, and 24-hour supervision to frail in-home individuals preventing immediate placement in acute care hospital, skilled nursing facility, or other 24-hour care facility. Services under this contract are being increased to meet the ongoing demand for MSSP services. FINANC.IAI. IMPACT: $63,375 California Department of Social Services, Children and Family Services Division Grant funds: 100% State/Federal. CHII..DREN'S IMPACT STATEMENT: This action supports all five of the community outcomes established in the Children's Report Card: 1) "Children Ready for and Succeeding in School"; 2) "Children and Youth Healthy and Preparing for Productive Adulthood"; 3) "Families that are Economically Self Sufficient"; 4)"Families that are Safe, Stable and Nurturing"; and 5)"communities that are Safe and Provide a High Quality of Life for Children and Families." Consulting to the department's Children and Family Services Bureau, Mr. Hurley will plan and facilitate the California children and family services review(C-CFSR) and assist with the child welfare redesign effort. Services include developing and facilitating work plan,writing initial drafts for the County Self Assessment Plan and System Improvement Plan, completing final drafts for the C-CFSR, facilitating meetings of the Self Assessment Team, and providing support and training to the change management process. Mr. Hurley has prior experience assisting the department to implement the county's redesign plan. On August 5,2003,the Board of Supervisors approved Contact#26-403-6 with Surgical Nurse,LLC, dba'Travel Nurse Across America, for the period from July 1, 2003 through June 30, 2004, for the provision of Nursing Registry services at Contra Costa Regional Medical Center and Contra Costa Health Centers. Approval of Contract#26-403-7 will allow Surgical Nurse LLC,dba Travel Nurse Across America,to continue to provide registry services, through June 30, 2005.
