Hargrave II, M.D. , (Specialty: Gastroenterology) , a self-employed individual, in an amount not to exceed $345, 000, for the provision of Gastroenterology services, effective on the close of business on February 29, 2004 ; and 2 . Contract #26-971-15 with Thomas B. Hargrave, M.D. , (Specialty: Gastroenterology) , a self--employed individual, for the period from March 1, 2004 through January 31, 2005, in an amount not to exceed $25, 000, for the prevision of professional Gastroenterology services for patients at Contra Costa Regional Medical Center. FIS ALI ACT. This Contract is funded by Enterprise I Budget. Cost to the County depends upon utilization. As appropriate, patients and/or third party payors will be billed for services . On April 9, 2002, the Board of Supervisors approved Contract #26-911-13 with Thomas B. Hargrave III, M.D. , to provide Gastroenterology services, including clinic coverage, training, and medical and/or surgical procedures, for patients at Contra Costa Regional Medical Center, for the period from February 1, 2002 through January 31, 2005 . Based upon an increased in volume and utilization of Gastroenterology patients at Contra Costa Regional Medical Center, Contractor has agreed to provide additional clinic sessions . Therefore, the Department and the Contractor have mutually agreed to renegotiate the Contract Payment Provisions and the term, to reflect the intent of the parties. Therefore, in accordance with Paragraph 3 . (Term) of the Contract, the Department and Contractor have agreed to a mutual cancellation of Contract #26-911-13 . Approval of Cancellation Agreement #26-911-14 will accomplish this termination. Under Contract #26-911-15, Contractor will provide professional Gastroenterology services, including, consultation, training, and medical and/or surgical procedures, through January 31, 2005 . Reasons for Recommendations and Background: Centra Costa County entered into an agreement with Blankinship and Associates, Inc.,May 20, 2002, to prepare an East Bay Regional Pesticide Monitoring Program for the Statewide NPDES Permit for discharges of aquatic pesticides. The NPDES permit requires the development and implementation of a water quality monitoring plan. Applicants have the choice to partner with other agencies to develop regional monitoring plans. Contra Costa County joined forces with Alameda County PublicWorks Agency, City of Antioch, City of Concord, and Zone 7—Alameda County Flood Control and Water District to develop a regional monitoring program. The agencies are sharing the cost of the development and the execution of the regional water quality monitoring plan equally through the development of Joint Exercise of Powers Agreement.The amendment to the Consultant Services Agreement in the amount of $95,000 is required to fund continuing services for monitoring,testing, and report writing in support of the East Bay Regional Pesticide Monitoring Program through January 31, 2005. IV. Consequences of Negative Action: If the contract amendment is not approved with Blankinship and Associates,Inc.,the County will not be in compliance with the Statewide General National Pollutant Discharge Elimination System permit. DATE: March 23, 2004 PAGE: 2 III. Reasons for Recommendations and_Backeround: The community of Knightsen does not have a municipal sewer system, relying instead on septic tank systems. Since the residents rely on wells for drinking water, a concern by some residents is that flooding events can over flood septic tanks, mix with floodwaters, and contaminate the drinking water supply. Further potential contamination of Rock Slough waters can occur via seepage through levees. During flooding events,property owners typically pump their storm waters:from their properties to nearby facilities i.e,ditches,East Contra.Costa Irrigation District canals,thereby mer spreading the contaminated waters to downstream property owners. IV. Consequences of Negative Action: Delays will impact progress towards a healthy solution for Knightsen. If no opportunity for a CSD exists, further implied discussions with the Knightsen Town Advisory Council and the community would be hampered as to the benefits of what a CSD can do,and to affirm what the responsibilities of the district will be.
