RACIGROIAM
The Area Agency on Aging (AAA) provides services for older residents of the County under the Older
American'sAct (OAA) and Older Californian'sAct (OCA). The AAA has developed services for the Family
Caregiver Support Program, funded under OAA Title III E, providing Financial, Legal and Medical.
Education, Respite, Multicultural Connection Program, Assistive Technology, and Grandparent Caregiver
Support Program. The AAA continues to provide program development, coordination, administration, and
planning as outlined in the FY 2003-04 Update of the Title III E Area Plan Addendum for Planning and
Service.

GOAL #8
The Contra Costa County Area Agency on Aging is committed to
empowering and sustaining family caregivers, recognizing that
they are the backbone of our long term care system and that they
function under tremendous burdens. AAA will empower and
sustain family caregivers through the development and
enhancement of a comprehensive system of family caregiver
support services and by making this support system both visible
and accessible to them.
RATIONALE
Based on the Family Caregiver Needs Assessment and its analysis,
conducted by AAA and the FCSP Planning Group, AAA was
requested to provide an expanded system of support that
addresses caregivers needs for increased access to services.
OBJECTIVES
1. AAA staff and the Family Caregiver Support Program Planning Group
will seek input of family caregivers and other stakeholders regarding service
needs by conducting a comprehensive needs assessment by August 31, 2001.

2. AAA staff will assure ongoing participation in the Family Caregiver
Support Program planning process of service providers, funders, advocates,
family caregivers and other stakeholders through June 30, 2002. Completed
and Continued
3. AAA Staff and the Advisory Council on Aging will develop a timeline
and budget for FCSP implementation by September 1, 2001. Completed.
Additionally, FY 02-03 and 03-04 allocations and contracting has been
approved by the Advisory Council' and Board of Supervisors.
4. AAA Staff, in conjunction with the Advisory Council on Aging, will
develop FCSP evaluation components by October 15, 2001. Completed.
Evaluation and Monitoring tools have been developed and each
contractor and program has been monitored and evaluated.
5. AAA will work with Employment and Human Services and contract
staff to issue an RFP for service needs as identified in the FCSP Needs
Assessment and in compliance with the service categories outlined in the
FCSP Service Matrix by October 30, 2001. Completed
6. AAA,through its Request for Proposals process will require bidders to
include a plan for community outreach to assure that low income individuals,

ethnically diverse, limited English speaking individuals and persons identified
as being of greatest economic or social need are being served through June
30, 2002. Completed and Continued.
7. AAA will assure that programs and services provided with FCSP funds
meet the targeting mandates of the Older Americans Act through June 30,
2002. Completed and Continued.
8. AAA Staff and the Advisory Council on Aging will review and
recommend the awarding of bids for FCSP services by December 1, 2001.

9. AAA Staff will negotiate contracts for successful provision of FCSP
services by December 31, 2001. CompletedDecember 2001.
10.AAA Staff will provide technical assistance to successful bidders
through June 30, 2002. Completed and Continued.
11. Family Caregiver Alliance will provide a Community Education
Campaign targeted for medical groups, hospitals (discharge planners and
casemanagers), allied health services, social services, libraries, faith based
organizations with the goal of providing a multi-faceted approach to reaching
both the probable referral sources and caregivers themselves by June 2003.

12. Family Caregiver Alliance will conduct broad community outreach
through activities such as media, newsletter articles, flyers, and web site
byJune 2003. PD Completed and Continued.
13. Family Caregiver Alliance will make information and assistance
accessible to caregivers by live-voice telephone. Each contact will be sent a
package of materials and resource information tailored to the identified
caregiver need and be offered a quarterly newsletter, notice of workshops,
support groups and related events by June 2003. PD Completed and

14. Family Caregiver Alliance will provide comprehensive assessment
of caregiver functioning and assessment of the care recipient's functional
status, resulting in a plan of care for the caregiver with follow-up services
provided by June 2003. PD Completed and Continued.
15. Family Caregiver Alliance will provide caregivers individual and/or
family counseling and Caregiver Training Workshops by June 2003. PD
Completed and Continued.
16, Family Caregiver Alliance will offer an array of respite services to
family caregivers, which include on-going monthly respite, short-term respite,
in-home services, and day care slots by June 2003. PD Completed and

17. Family Caregiver Alliance will provide caregivers with access to legal
assistance, including but not limited to, legal advice, counseling, and
representation by an attorney or other acting under the supervision of an
attorney by June 2003. PD Completed and Continued.
13. Jewish Family Services of the East Bay will conduct
bilingual/bicultural outreach to family caregivers through 1) informal community
networks such as schools, community centers, clinics, churches and
synagogues, etc., 2) dissemination of information through newsletters and
brochures by June 2003. PD Completed and Continued.
19. Jewish Family Services of the East Bay will provide comprehensive
assessments to caregivers with multiple needs and determine the appropriate
supportive services. The assessment includes physical and mental health,
formal and informal support systems, activities of daily living, mobility, nutrition,
cognitive functioning physical environment and financial resources by June

20. Jewish Family Services of the East Bay will provide direct case
management assistance to caregivers experiencing diminished functioning
capabilities, personal conditions or other characteristics. Case managers will
provide service referral, service coordination, linkage, monitoring and advocacy
by June 2003. Completed and Continued.
21. Jewish Family Services of the East Bay program staff and
volunteers will help clients access transportation by June 2003. PD Completed
and Continued.
22. Jewish Family Services of the East Bay will provide caregiver
counseling at local sites and in-home to work with caregivers on issues, such
as, feelings of anger and guilt, changes in familial roles, and crisis situations by
June 2003. PD Completed and Continued.
23. Jewish Family Services of the East Bay will conduct caregiver
training with a focus on developing skills necessary to perform caregiving
activities, such as decision- making, problem solving, long term planning, use
and misuse of drugs for older adults, legal issues in eldercare, and family
dynamics in caregiving byJune 2003. PD Completed and Continued.
24. Jewish Family Services of the East Bay will conduct caregiver
support groups for socialization, education and peer support by June 2003. PD
Completed and Continued.
25. Jewish Family Services of the East Bay will provide home visiting to
family caregivers. For refugees and immigrants, volunteers may be trained to
tutor English-as-a-second language {ESL} or general life skills, so that the
caregiver is better able to function independently by June 2003. PD Completed
and Continued.
26. Jewish Family Services of the East Bay will create a Multilingual
Resource Library and website to include an estimated 50-70 books,
informational brochures, pamphlets and 20-40 videotapes on caregiver topics in
multiple languages of English, Spanish, Russian, Farsi, Bosnian, Vietnamese,
Cambodian and Lao. Information from the resource library will be widely
distributed throughout the target communities by June 2002. (This is an OTO
funding objective so the completion date is June 2002.) PD Completed
27. The Alzheimer's Association of the East Bay will provide outreach
services by individual contact, newsletters and meetings to promote, organize,
and establish Alzheimer's Disease resource network for identifying caregivers
by June 2003. PD Completed and Continued.
28. The Alzheimer's Association of the East Bay will provide community
education to family caregivers, to assist in the development of effective
caregiving skills and enhance caregiver well-being including but not limited to
Alzheimer's Beginning to Cope, legal and financial planning, dealing with
challenging behaviors, communication strategies, and caregiver wellness by
June 2003. PD Completed and Continued
29. The Alzheimer's Association of the East Bay will provide
comprehensive assessment to recruit, train and supervise volunteer"family
consultants" who will provide intensive emotional support and an expanded
follow-up component to family caregivers and intensive phone assistance by

30. The Alzheimer's Association of the East Bay will provide
counseling to determine the most appropriate type of caregiver support
necessary for individual families by June 2003. PD Completed and Ongoing.
31. The Alzheimer's Association of the East Bay will provide caregiver
support to assist individuals to better cope with their situation by June 2003.

32. The Alzheimer's Association of the East Bay will provide dementia
education forums and caregiver training, which offers support group attendees
in-depth training and education by skilled professionals by June 2003. PD

33. FamiliesFirst, Inc. & The Family Enhancement Collaborative will
match grandparents and other relative caregivers with a peer Kinship Mentor
for short term, one-on-one support and case management services by June

work with the family to identify needs and develop a plan to address those

provide an intensive home-visitation component with meetings and/or phone

resources and will address short-term needs with direct emergency assistance

accompany grandparents and relative caregivers to doctor's appointments,
meeting with social service agencies, and parent teacher conferences to
provide support during stressful periods and facilitate communication and
interaction with the health care and school systems by June 2003. PD

38. FamiliesFirst, Inc. & The Family Enhancement Collaborative will
assist grandparents and relative caregivers with information regarding legal
options and provide support coping with the loss of a child and caring for the
children's children by June 2003. PD Completed and Ongoing.
39. FamiliesFirst, Inc. & The Family Enhancement Collaborative will
provide for recreational activities, respite days, and support groups for
caregivers by June 2003. PD Completed and Ongoing.
40. Independent Living Resource will provide information and
assistance to connect individuals in caregiving roles with current technology
and services in the community by June 2002. (This is an OTO funding objective
o the completion date is June 2002.) PD Completed.
41. Independent Living Resource will assess individual needs through
one-to-one meetings with caregivers, will conduct at-home site evaluations and
link caregivers with appropriate sources for assistance by June 2002. (This is
an OTO funding objective so the completion date is June 2002.) PD

42. Independent Living Resource will provide training to family
members and Others providing care for older persons on such topics as
effective use of technology and/or services, problem-solving techniques, aid
and personal care activities such as bathing, exercising, transferring and
preparation of meals for special diets by June 2002. (This is an OTO funding
objective so the completion date is June 2002.) PD Completed.
43. Independent Living Resource will research and purchase
assistive living devices and equipment for demonstration and for the lending
library by June 2002. (This is an OTC) funding objective so the completion date

44. Independent Living Resource will provide a benefits specialist to
assess, provide information and advocate for caregivers to access programs
that can provide the needed technology and service by June 2002. PD
Completed and Ongoing.
45. The AAA will design a training program specifically addressing the
needs of an ethnically diverse caregiver population in Contra Costa County.
This program is funded under the Title Ill E Family Caregiver Support Program.
AAA will contract with Jewish Family and Children's Services (JFCS), acting as
the lead agency, to provide the development and design of a program
addressing the training and educational needs of caregivers who exhibit cultural
barriers to accepting assistance in the care of a family member. Caregivers
from different ethnic cultures bring with them cultural stigmas about care giving
responsibilities. They also lack specialized training and counseling on how to
deal with the daily stresses of caring for a relative as well as lack information
about the resources available to them.
A coalition of agencies, coordinated by JFCS, will coordinate
outreach in the following.communities: Laotian, Vietnamese, Latino,
African American, Russian, Afghan and Philippine. Focus groups will be
assembled to discuss the specific cultural needs for caregivers from these
cultures. Based upon information gathered from these focus groups, a
curriculum will be designed to address the needs assessed. This phase will
be completed by June 30, 2003. Completer)
The Implementation phase (identification of caregivers and training
provided) will be completed by June 30, 2004.
46. The AAA will participate in a project sponsored by the )National
Association of Area Agencies on Aging (n4a) entitled: Making the Link.
This program is designed to work with area physicians to identify
caregivers and link them with available services provided by the Human
Service organizations of Contra Costa County. All phases of this project
will be completed by June 30, 2004.
Narrative
Title III E Area Plan Addendum Fiscal Year 2003-04
In fiscal year 2002-2003, PSA 7 (Contra Costa County Area Agency on Aging)
contracted with Jewish Family and Children's Services of the Greater East Bay to
develop, in conjunction with the Family Caregiver Alliance, a Caregiver Training
Curriculum addressing the particular cultural barriers and diversity found in Contra Costa
County. Using One Time Only funds,Jewish Family Services developed a caregiver-
training curriculum addressing the particular cultural aspects of the Lao,Russian, Afghan
communities. In fiscal year 2003-04,Jewish Family Services will implement the training
curriculum for those three ethnic groups.
Contra Costa County has not encountered any particular challenges in the implementation
of the Title III E program. Contra Costa County AAA was aware that there was a need to
provide caregiver training to caregivers from specific ethnic cultures where cultural
barriers prevented those caregivers from receiving complete services under the Title III E
Family Caregiver Provider Program. To this end, Contra Costa County AAA has
contracted with Jewish Family and Children's Services to develop culturally specific
caregiver training curriculum and to implement that program by providing training
sessions to caregivers from the Lao, Russian and Afghan communities.
