No County costs. To secure funds under the Older Californians Act and Titles III, III E and VII of the Older
Americans Act, the State must approve the Contra Costa County Area Agency on Aging Area Plan Update
for FY 2003/2004.

Under the Acts, the Area Agency on Aging (AAA) must develop and submit an update to the four-year plan
to the State to receive funding. The State will determine funding levels and inform the local areas based on
their local plans.
The AAA provides services for older County residents under Titles III, III-E & VII of the Older Americans
Act and under the Older Californians Act. Under the 2003-2004 Area Plan Update, the AAA provides
through contract Congregate Meals, Dome-Delivered Meals, Legal Assistance, Home Visiting, In-Home
Services Registry, Transportation, Adult Day Care, Errand & Escort Services and Family Caregiver Provider
program services. Funding from the Older Californians Act allows the AAA to provide through contract the
Alzheimer's Day Mare Resource Center Program. Additionally, the AAA directly provides Information and
Assistance, Preventive Health Care and Disease Prevention, Linkages Programs, Respite Purchase of
Services, Health Insurance Counseling and Advocacy Program (HICAP) and perform the program
development, coordination, administration and planning responsibilities of the Area Agency on Aging.

This 2003-2004 update of the 2001-2005 Area Plan for PSA 7 is hereby submitted
to the California Department of Aging for approval. The Contra Costa County Board of
Supervisors supports the development of community based systems of care and
recognizes the responsibility within each community to establish systems in order to
address the care needs of older individuals and individuals with functional impairments,
their families and caregivers.

AREA AGENCY on AGING
Area Plan Update -- Fiscal Year 2003 -- 2004
1. Contracted Service:
A. Nursing Home Ombudsman - provides countywide complaint investigation and
resolution services to elderly residents of nursing and residential care facilities
through a carps of trained volunteers.
B. Congregate Nutrition - provides nutritionally balanced meals in a group setting at
21 locations throughout the county.

C. Horne Delivered Nutrition - provides nutritionally balanced home delivered meals
to homebound older persons from the following delivery points serving all
communities in Contra Costa County.

D. Horne Visiting - provides a countywide network of volunteers who visit
homebound, isolated alder persons offering reassurance and comfort,
E. In Home Services Registry - countywide project maintains lists of qualified in
hors service workers and refers them to alder persons in need of home chore and
other in home service assistance.
E. Transportation - arranges transportation for congregate nutrition participants to
and from meal sites.
G. Legal Services -provides a countywide program offering information, counseling,
consultation, advice, administrative representation and advocacy and legal
representation services by attorneys and legal support staff'to older persons with
certain law-related problems.
H. Family_Caregiver Provider Pro rare -provides services addressing the needs of
family members who provide cage to an elderly relative and for grandparents who
are providing custodial care for their grandchildren.

I. Assisted Transportation (Errand & Escort)— Expansion of errand and escort
services to East County. Volunteers are matched with individuals 60 and aider
who are unable to drive or use public transportation alone, and therefore, would
otherwise be unable to get to Doctor appointments, pick up groceries, medications
or ether items necessary for daily living.
J. Adult Day Care (Social Day Care) - Provision of personal care for dependent
adults in a supervised, protective, congregate setting during some portion of a
twenty-four hour day. Services offered typically include social and recreational
activities, training, counseling, meals and assistance with activities of daily living.
K. Trilpgy Integrated Resources (Network of Care Website)- A comprehensive
Internet based community resource for seniors, family members, people with
disabilities, caregivers, service.providers and community members.
L. Senior Coalitions (East, West, and Central)-Provide organized activities and
advocacy on issues affecting seniors in the community.
M. Alzheimer's Day Care Resource Center- provides specialized day care services to
persons with Alzheimer's disease or other dementia disorders and also provides
support services to families and caregivers. It serves as a Resource and Education
Center to the Community.
11. Area Agency on Aging Direct Services (AAA);
A. Information and Assistance - the countywide Aging and Adult Information and
Assistance program performs the following functions:
1. Maintains current information on the resources, programs and services
available to people 18 years of age and over with up to date computerized data
base.
2. Provides telephone counseling to senior callers, dependent adults age 18 and
over regarding service programs and resources which would be of assistance
to their}, and provides arrangements or appointments to assure they get needed
service.
. Provides comprehensive assessments to callers or family members where there
are multiple needs and determines the necessary supportive or other
appropriate services to meet those needs.
4. Employs a specially trained staff to provide information and assistance.

5. Provides outreach to isolated and underserved older adults through visits to the
community, educational presentations, publicity, posters, and participation on
committees and groups.
o. Evaluates and follows up to determine the quality and effectiveness of service
provided to clients and determine gaps in service.
7. Provides literature, fact sheets, housing lists, resource library and many other
specialized resource materials.
S. Health Promotion and Disease Prevention - in conjunction with the
Information and Assistance program disseminates health promotion and
disease prevention information to maintain or improve the physical, mental or
nutritional health of older persons.
a. Produces Senior Information Journal, a cable television program., which
airs on local cable channels throughout Contra Costa County providing
health promotion and disease prevention information.
b. Health Page provides health promotion and disuse prevention information
in each issue of the Senior Information newsletter.
c. Provides community health education presentations for older persons.
d. Provides staff support for committees working on health issues in the
community.
B. Health InsuranceCounseling and Advocacy Program. HICAP - provides
Medicare beneficiaries and those imminent of becoming eligible for Medicare with
counseling and advocacy regarding Medicare, private health insurance and related
health care coverage plans through the efforts of both a-corps of highly skilled
volunteers and professional staff`. This program provides objective counseling to
assist consumers in choosing appropriate health insurance coverage. This program
provides assistance with Medicare problems, Medicare Supplemental Insurance,
Health Maintenance Organization (HMO) plans, long-term care insurance, medical
billings, and claims and Medicare appeals.
C. Linkages- provides case management and information and assistance to
individuals age 18 and older in order to prevent premature institutionalization.
D. Respite :purchase of Service Program- funds are used for purchasing respite
services for primary unpaid caregivers of functionally impaired individuals.

III. Area A�ency on Arming, Other Activities:
A. Prouarn.Development - AAA seeks to establish new services and the
improvement, expansion or integration of existing services for older persons.
B. Coordination - the AAA brings together programs, services and organizations into
interconnected community systems serving older persons in order to promote a
community based system of care.
C. Administration - the AAA performs the ongoing planning and management tasks
related to the development and administration of the Area Plan. These activities
include: need assessments, plan writing, contracts management, monitoring,
evaluation and Request for Proposals (R.F.P.'s). Also included is advocacy, which
is defined as: to plead and/or support the concerns common to many older persons
to seep change in laws, institutions or conditions to better meet their needs (e.g.,
staffing of advisory council committees, task forces, etc. which advocate on behalf
of older persons).
IV. Outreach/Targeting
The AAA through its Request For Proposals (R.F.P.'s) requires bidders to include a
plan for community outreach to assure that low-income individuals, minorities, limited
.English-speaking individuals and persons identified as being of greatest economic or
social need aged 60 years or older are being served. If an outreach plan is not
included in the proposal, the proposal may be deemed non-responsive.
Contracts with service providers specify the minimum percentage or number of low-
income and minority clients that must be served.
As part of the County Employment and Human Services Department, the AAA has
access to bilingual and translator staff to assist non-English speaking clients in all
regions of the County.
The AAA actively participates in concert with task farces, focus groups, etc., on
multi-cultural issues. AAA works with other agencies to assist in identifying low-
income,minority seniors who may be at risk.
The .AAA is a member of the Southeast Asian Senior Access Collaborative Project
(SEASAC). SEASAC helps isolated Laotian seniors in West County overcome
language and cultural barriers to access quality health care and human services.
SEASAC provides comprehensive, multilingual services including community
outreach, needs assessment, case management, and advocacy. Health education,
social and cultural activities, ESL,classes, and information and referral services are
also provided.

Coordination of Tax Counseling for the Elderly program.
InfoVan, an information center on wheels, travels throughout the county to provide
information about services, how to access them, and contact information for health
and social service providers. The van is equipped with shelves that allow the
transportation and display of brochures, and other materials. Some informational
materials are available in many languages including, Laotian, Russian, Spanish,
Chinese, and Vietnamese.
The Senior Information newsletter is distributed every two months-to over 13,000
organizations and individuals.
An Active Aging Community Task Force (AACTF) has been formed this year.
AACTF plans to increase public awareness about the benefits of physical activities for
older adults and to implement community--based physical activity programs, which
include strength-training exercises, aimed at maintaining functional fitness and
reducing the risk of chronic disease and falling in alder adults. Many senior centers
and senior housing complexes that serve the low-income and diverse populations
already have programs available and the task force is working with these groups to
enhance their programs.
The AAA is in the process of administering the Key Informant Survey. The survey
objective is to increase AAA's awareness of the needs of aider adults. The survey
participants are drawn from all areas of the county. The survey will be done in
English, Spanish, Chinese, and Laotian.
V. Area Flan Goals
Gaal Statement #I:
The AAA is committed to assisting older individuals (who may be in danger of losing
their independence) so they can lead meaningful, dignified, and independent lives in
their own homes and communities as long as possible. AAA will accomplish this by
promoting the availability of an accessible range of service options through
collaborative efforts and decision making among public, private, voluntary and
fraternal organizations and coalitions, for continuing the development and
enhancement of a comprehensive and coordinated home and community based system
of care.
Goal Statement#2:
AAA will provide services to older individuals with the greatest economic and social
needs, including low income diverse population groups through continued service
expansion, development and collaboration with corninunity groups, service providers,
and elected officials, in order to provide health, social, nutrition and legal services for
older individuals who may be at risk of abuse or neglect.

Gaal Statement#3:
AAA will provide public information to agencies and community persons age 60+ and
their families regarding resource availability, educational opportunities,.
demographics, health care insurance counseling, and funding opportunities.
Goal Statement #4:
The AAA will work toward an efficient, cost effective, user friendly integrated
approach to deliver Aging and Adult services, including but not limited to
Multipurpose Seniors Service Project, Adult :protective Services, L.PS
conservatorship, In-Home Support Services, Older California Act programs (Linkages,
Alzheimers Day Care and. Older Americans Act Services. The AAA will continue to
provide direct HICAP services. The adult service delivery system will be monitored
to ensure quality responsiveness to the community based system of care for the
elderly.
Goal Statement#5:
The AAA will continue to be an advocate for all older adults 60+ by analyzing public
policy issues, legislation, programs, hearings, and by participating in public forums,
community activities, and advocating for older adults rights, as appropriate.
Goal Statement#+6
The AAA will enhance the administrative efficiency of its contracting and service
delivery operations for the purpose of improving the responsiveness of the community
based system of care for the elderly in Centra Costa County to the changing needs of
the older papulation.
Goal. Statement #7:
AAA will maintain a preventive health care program that will provide both health care
insurance counseling and advocacy and health promotion and disease prevention
information to the older population of Contra Costa County by means of a variety of
community outreach. activities.
Goal Statement#8:
The Contra Costa County Area Agency on Aging is committed to empowering ;and
sustaining family caregivers,recognizing that they are the backbone of our long-term
care system and that they function under tremendous burdens. AAA will empower
and sustain family caregivers through the development and enhancement of a
comprehensive system of family caregiver support services and by making this
support system both visible and accessible to them..

ADEQUATE PROPORTION
The Federal Government in accordance with the Older Americans Act section 306 (a) (2)
requires the Area Agency on Aging to expend an adequate proportion of the amount allocated
for Title IIIB of the Older Americans Act in the following service areas: Access, Legal
Services and In Home Services. The AAA will provide the following minimum percentages
in Access, Legal, and In-Home Services.

COMMUNITY SERVICE AREAS & FOAL POINTS
The California Department of Aging consistent with Older Americans Act regulations
encourages area agencies on aging to identify, develop, and.implement a community-based
system of care for specific communities in its planning and service area.. Area agencies on
aging have the responsibility to establish criteria for and identification of community service
areas including designation of community focal points within each community service area, as
appropriate.
The Contra Costa County Area Agency on Aging has established three community service
areas - Fast, Central, and 'Vest County.
The East County Community Service Area includes the cities of Pittsburg, Antioch, Oakley,
and Brentwood and the unincorporated communities of Bay Point, Bethel Island, Byron,
Knightsen, Discovery Bay and adjacent areas.
The Central County Community Service Area includes the cities of Martinez, Concord,
Pleasant Hill, "Walnut Creek, Lafayette, Moraga, Orinda, Danville, San Ramon and Clayton and
the unincorporated communities of Pacheco, Alamo, Blackhawk, and adjacent areas.
The Fest County Community Service Area includes the cities of El Cerrito, Richmond, San
Pablo, Pinole, and .Hercules and the unincorporated communities of Kensington, East
Richmond Heights, North Richmond, El Sobrante, Rodeo, Crockett, Port Costa, and adjacent
areas.
The AAA intends to support the continued development of coalitions in East, Central, and
West County. Each coalition includes older persons, service provider agency representatives,
business representatives, professionals interested in serving older persons, local committee on

aging representatives, and other appropriate persons. These coalitions serve as regional focal
points for assisting the AAA in its planning, program development, and coordination
responsibilities.
