Approve and authorize the Health Services Director, or his designee,
(Steve Tremain, M.D. ) to execute on behalf of the County, Contract
#26-993-3 with Russell Schulze, M.U. , (Specialty: Orthopedics) , a
self-employed individual, in an amount not to exceed $79, 000, for
the period from June 1, 2003 through May 31, 2004, to provide
professional Orthopedic Surgery services to Contra Costa Regional
Medical Center.
FISCAL IMPACT.
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services.
BACKGR4M/RVASON(5) FOR RECOMM ATIOAiS :
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and clinics.
On June 4, 2002, the Board Of Supervisors approved Contract #26-993
(as amended by Contract Amendment Agreements #26-993-1 and #26-993-
2) , with Russell Schulze, M.U. , to provide Orthopedic Surgery
services, including, but not limited to, consultation, clinic
coverage, training, and medical and/or surgical procedures, for
Contra Costa Regional Medical Center and Contra. Costa Health
Centers, for the period from May 20, 2002 through May 31, 2003 .
Approval of Contract ##26-593-3 will allow Contractor to continue to
provide Orthopedic services through May 31, 2004 .

Approve and authorize the Health Services Director, or his designee(Milt Camhi), to execute on behalf of the
County, Contract #27-304-5 with Daniel Zimmerman, M.D., a self-employed individual, in an amount not
to exceed $50,000, for the period from July 1, 2003 through June 30, 2004, for the provision of professional
OB/GYN services for the Contra Costa Health Plan.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon
utilization. As appropriate, patients and/or third party payors will be billed for services.
BACKGROUNID/ ASON(S) FOR.RECOMMENDATION(S):
The Health Plan has an obligation to provide certain specialized professional health care services for its
members under the terms of their Individual and Group Health Plan membership contracts,with the County.
Approval of Contract#27-304-5 will allow the Contractor to provide professional OB/GYN services to Contra
Costa Health Pian members, through June 30, 2004.

corporation, effective June 30, 2003, to amend Contract, ##26-426, to
increase the total Contract Payment Limit by $1.25, 000 from $75, 000 to
a new total payment limit of $200, 000 .

BACKt ROUND/REASON(S) FOR RECQMMENDATION(S) :
On December 10, 2002, the Board of Supervisors approved Contract ##26-
426 with Intelistaf Flying Nurses Corp. , to provide temporary licensed
nursing personnel and other classifications to assist Contra Costa
Regional Medical Center and Contra Costa Health Centers during peak
loads, temporary absences and emergency situations in critical areas,
for the period from January 1, 2003 through December 31, 2003 .
At the time of negotiations, the payment limit was based upon target
levels of utilization. However, the utilization during the term of
the agreement was actually higher than originally anticipated.
Approval of Contract Amendment Agreement #26-426-1, will allow
Contractor to provide additional temporary help registry services to
Contra Costa Regional Medical Center, through December 31, 2003 .

In November 2002, the County Administrator approved and the Purchasing
Services Manager executed. Contract #26-443 with A 2 Z Printing and
Translations, to provide patient interpretation and translation
services for the Contra, Costa Regional Medical Center for the period
from July 1, 2002 through June 30, 2003 .
Approval of Contract Amendment Agreement #26--443-1, will allow
Contractor to provide additional services to patients at Contra Costa
Regional Medical Center, through June 30, 2004 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Kathryn Grazzini) , to execute on behalf of the County, Contract
Amendment Agreement ##26-458--1 with Access Nurses, a Corporation,
effective June 1, 2003, to increase the total Contract Payment Limit
by $100, 040 from $100, 000 to a new total payment limit of $200, 000 .
FISCAL IMPACT:
This contract is included in the Health Services Department Enterprise
I budget .
REASONS FOR RECOMMENDATIONS/BACKGROUND:
For several years the County has contracted with registries to provide
temporary medical, professionals to assist Contra. Costa Regional
Medical Center and Contra Costa Health Centers during peak loads,
temporary absences, and emergency situations . There is a nationwide
nursing and healthcare staff shortage, and in spite of persistent
efforts to recruit nurses and other healthcare staff, the Department
has experienced great difficulty in filling any vacant positions .
Therefore, the Department has had to rely heavily on the registry
services in order to provide quality nursing care for patients.
On September 17, 2002, the Board of Supervisors approved Contract #26-
458 with Access Nurses to provide temporary nursing services, as
requested by the Department, for the period from September 1, 2002
through August 31, 2003 .
Approval of Contract Amendment Agreement ##26-458-1 will allow the
Contractor to provide additional nursing coverage services through
August 31, 2003 .

execute on behalf of the County, Standard Agreement #74-205 (##02-72236-000) with the State
Department of Mental Health, in an amount not to exceed $964,856, for' the purchase and
usage of state hospital beds, for the period from July 1, 2002 through June 30, 2003.
FISCA ZDIPACT
This agreement is 1.00* funded by Mental Health Realignment Trust Fund. No cost to the
County.

In previous years, the agreement to purchase State hospital beds was a part of the Mandated
County Performance Contract, as required by Section 5650 of the Welfare and Institution
Code. Recently the Dental Health Division was notified by the State, that beginning with
Fiscal Years 2002/2003, the purchase of state hospital beds will be separated from the
Performance Contract, making it a stand-alone agreement. On October 1, 2002, the Board
of Supervisors approved the Fiscal. Year 2002/.2003 Performance Contract, without the bed
purchase agreement language, However~, the State did not issue the Agreement until April
2003. The State is requiring that the Mental Health Department use their Standard
Agreement (#02-72236-000) , for approval. .
Under Standard Agreement #74-208, the County shall compensate the State and the State
agrees to provide bed usage, including staffing, facilities, equipment, and supplies
through June 30, 2003.
Seven seal/certified copies of the Board order should be returned to the Contracts and
Grants Unit for submission to the State Department of Mental. Health.
