Approve and authorize the Health Services Director, 'or his designee (Kathryn Grazzini)to execute
on behalf of the County,Contract Amendment Agreement #26-440-2 with Moving Solutions, Inc.,
a corporation, effective February 1, 2003, to amend Contract #26-440 (as amended by Contract
Amendment Agreement#26-440-1),to increase the total Contract Payment Limit by$14,370 from
$186,402 to a new total payment limit of$200,772.
FISCAL IMPACT:
Funding for this Contract is included in the Department's Enterprise I budget for FY 2002/2003.
BACKGROUND- E-ASOPi(S)FOR RECOMMENDATION(S):
On June 4, 2002, the Board of Supervisors approved Contract #26-440 (as amended by Contract
Amendment Agreement#26-440-1) with Moving Solutions, Inc, for the period from May 7, 2002
through June 30, 2003 to provide furniture and equipment moving services, storage and relocation
services required for occupancy of the new Ambulatory Care facility at Contra Costa Regional
Medical Center,and to track and tag shipments of new furniture and coordinate placement at the new
facility.
Approval of Contract Amendment Agreement #26-440-2 will allow the Contractor to provide
additional storage and moving services through June 30, 2003.

Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract Amendment Agreement #24-859-10 with Edgewood Center for
Children and Families, a non-profit corporation, effective January 1, 2003 to amend Contract
#24-859-9 to modify the scope of services and payment provisions with no change in the original
Contract Payment Limit of$284,385.
FISCAL:IMPACT:
None, there is no change in the Contract Payment Limit. This Contract is funded 45% by Federal
Medi-Cal(FFP),45%by State EPSDT,and 10%Q by Mental Health Realignment.
BACKGROUNDAREASQN(S)FOR RECOMMENDATIQN(S):
On July 23, 2003, the Board of Supervisors approved Contract#24-859--9 with Edgewood Center
for Children and Families for the period from July 1, 2002 through June 30, 2003, for the
provision of Day Treatment services for severely emotionally disturbed children.
Approval of Contract Amendment Agreement #24-859-10 will allow the Contractor to provide
additional services, including mental health services and medication support services through June
30,2003.

RECOMMENDATION W :
Approve and authorize the Health Services Director, or his designee,
(Jeff Smith, M.D. ) to execute on behalf of the County, Contract
Amendment Agreement #26-994-1 with Torsten Jacobsen, M.D. ,
(Specialty: Orthopedic Surgery) , a self-employed individual,
effective March 15, 2003, extend the term of the Contract fpm June
30, 2003 through June 30, 2005 and to increase the total Contract
Payment Limit by $205, 000 from $80, 000 to a new total payment limit
of $285, 000 .
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services.
BACKGROV= REASON(S) FOR RECOMMENDATIONS:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and clinics .
On June 25, 2002, the Board of Supervisors approved Contract #26-994
with Torsten Jacobsen, M.D. , to provide Orthopedic Surgery services,
including, but not limited to, on-call coverage, consultation,
clinic coverage, training, and medical and/or surgical procedures,
for Contra Costa Regional Medical Center and Contra Costa Health
Centers, for the period from June 17, 2002 through June 30, 2003 .
At the time of negotiations, the payment limit was based upon target
levels of utilization. However, the utilization during the term of
the agreement was actually higher thin originally anticipated and
the performance of Orthopedic procedures has exceed the expectation
of the Division. Approval of Contact Amendment Agreement #26-994-1
will allow Contractor to continue providing services through June
30, 2005 .

502-1 with Bruce R. London, M.D. , Inc. , to provide Ancillary Radiology
physiciati services for Contra Costa Regional Medical Center and Contra
Costa Health Centers, including, but not limited to, consultation, on-
call coverage, and clinic coverage, for the period from January 1, 2003
through December 31, 2003 .
In accordance with Paragraph 4 . (Termination) of the Contract, the
Department and Contractor have agreed to a mutual cancellation of
Contract #76-502 . Approval of Cancellation Agreement #76-502-1 will
accomplish this termination.

Approve and authorize the County Administrator, or his designee, to execute on behalf of the
County, Administrative Amendments, as specified in the attached lists (Exhibit A and Exhibit B), to
increase the salary and administrative cost components by 5% for Fiscal Year 2002-2003, to .make
modifications in corresponding payment and budget provisions, and to incorporate State or County
required changes in other provisions (if any).
FISCAL IIVCF'ACT:
These Amendments provide a % cost-of-living increase to the salary and administrative cost
components of the Contracts on the attached lists, and increase the FY 2002-2003 Contract Payment
Limits based on the attached calculations. Furthermore, these increases are also reflected in the
Payment Limits for the corresponding automatic six-month contract extensions in those contracts that
contain this provision.
BACKGRt_3'C ND/REASON(S)FOR RECO M NDATIONN:
These Administrative Amendments carry out the directives of the Board of Supervisors regarding
cost-of-living increases for certain contract programs which are providing a vital and important part
of the County's continuum of care and services for residents of Contra Costa County. The cost-of-
living increase for each contract is based on the relative size of personnel and administrative costs of
each contract payment limit(excluding any ane-time-only amounts).
