Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Novation
Contract #29-335-15 with the City of El Cerrito, to provide
congregate meal services for County' s Senior Nutrition Program, for
the period from July 1, 2002 through June 30, 2003 . This Contract
.includes a three-month automatic extension through September 30,
2003 .
FISC" IMPACT:
No County cost is included.
BACKGROUND/REASON(S) FOR RECflMMZjAT10N(S>)
This Contract meets the social needs of County's population in that
it provides an average of 50 meals per day, five days per week, for
senior citizens in El Cerrito.
On October 2, 2001, the Board of Supervisors approved Novation
Contract #29-335-14 with the City of El Cerrito, for the period from
July 1, 2001 through June 30, 2002 (which included a three-month
automatic extension through September 30, 2002) for the provision
of congregate meal services for County' s Senior Nutrition Program.
Approval of Novation Contract #29-335-15 will replace the three-
month automatic extension under the prior Contract and will allow
the Contractor to continue providing services through June 30, 2003 .

Approve and authorize the Health Services Director, or his designee '
(Donna Wigand) to execute on behalf of the County, Contract
Amendment Agreement #24-933-17 with Crestwood Behavioral Health,
Inc . , a corporation, effective May 1, 2003, to amend Contract
#24-933-17, to increase the total Contract Payment Limit by
$1, 029, 129 from $5, 652, 362 to a new total payment limit of
$6, 681,491 and to increase the six-month automatic extension amount
by $514, 565 to a new total six-month extension amount of $3 , 340, 745 .
FISCAL IMPACT:
This Contract is 100% Mental Health Realignment funded.
BAQKGRQUND/RZASgE($) FOR RECOMMENDATION W :
On October 22, 2002, the Board of Supervisors approved Contract
#24-933-16 with Crestwood Behavioral Health, Inc. , to provide
Subacute skilled nursing care services for County's Seriously and
Persistently Mentally III (SMPI) and Neurobehavioral clients, for
the period from July 1, 2002 through June 30, 2003, which includes
a six-month automatic extension through December 31, 2003 .
Approval of Contract Amendment Agreement #24-933-17 will allow the
Contractor to provide additional services to County' s clients
through June 30, 2003 .

Approve and authorize the Health Services Director, or his designee,
(Steve Tremain, M.D. ) to execute on behalf of the County, Contract
Amendment Agreement #26-985-3 with Suzanne Ishii, D.P.M. ,
(Specialty: Podiatry services) , a self-employed individual,
effective March 1, 2003, to amend Contract #26-985-2, to increase
the total Contract Payment Limit by $6, 944 from $103 , 056 to a new
total Contract Payment Limit of $110, 000 .
FISCAL IMPACT.
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services.
BACKGRQUND/REASON>(S) FOR RECC7NMENDATIONS:
On March. 18 , 2003, the Board of Supervisors approved Contract #26-
985-2 with. Suzanne Ishii, D.P.M. , for the period from March 1, 20`03
through February 28, 2006, to provide Podiatry services to patients
at Contra Costa Regional Medical Center.
Due to the mutual mistake of the County and the Contractor, the
Contract payment provisions did not accurately reflect the intent of
the parties, which was that the Contractor would provide additional
4-hoar clinicsessions at a rate of $2803 per session, in addition to
her regularly scheduled clinic coverage, as set forth in the
Contract .
Approval. of Contract Amendment Agreement #26-985-3 will reform the
Contract, to make the formal Contract consistent with the oral
agreement, which was negotiated with the Contractor.

April 18, 2003, to amend. Contract #26-993 (as amended by Contract
Amendment Agreement #26-993-1.) , to increase the total Contract
Payment Limit by $4, 000 from $75, 000 OU to a new total payment limit of
$79, 000 .

For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and clinics
On June 4, 2002, the Board of Supervisors approved Contract 26-993
(as amended by Contract Amendment Agreementw. #26-993-1) , with Russell
Schulze, M.D. , to provide orthopedic Surgery services, including,
but: not limited to, consultation, clinic coverage, training, and
medical and./or surgical procedures, for Contra Costa Regional
Medical Center and Contra Costa Health Centers, for the period from
May 20, 2002 through May 31., 2003 .
Approval of Contract Amendment Agreement #26-993-2 will allow
Contractor to provide additional Orthopedic services through May 31,
2003, due to the leave of one orthopedic Surgeon.

behalf of the County, Contract Amendment Agreement #27-534-1 with ePOCRATES, Inc., a
corporation, effective June 1, 2003 to amend Contract#27-534 to increase the Contract payment
limit by $35,500, from $35,000 to a new total of $70,000 and extend the Contract terra through
December 31, 2006.

The Contra Costa Health Plan (Health Plan) has been continually initiating cost reduction measures
to promote affordable healthcare in Contra Costa County. Recent years have seen a dramatic rise in
the cast associated with drug formularies. tinder this Contract,the Contractor provides an electronic
drug formulary to be downloaded to providers' handheld computers. This software reduces drug
costs'by allowing providers to select approved medications,from the Health Plan's Drug Formulary,
by selecting generic versus costly name brand drugs.
On June 11,2002, the Board of Supervisors approved Contract#27-534 with ePOCRATES, Inc. for
the period from June 1, 2002 through December 31, 2003, for the provision of softwareand
administration services for the Health Plan's Drug Formulary.
Approval of Contract Amendment Agreement #27-534-1 will allow the Contactor to continue
providing,services through December 31,2006.
