RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Chris Grazzini) to execute on behalf of the County, Contract
#26-292-7 with West Contra Costa Transit Authority, in an amount not
to exceed $33, 000, to provide fixed route bus transportation
services between County' s Health Center in Richmond and Contra Costa
Regional Medical Center, for the period from May 1, 2003 through
April 30, 2004 .
FISCAL IMPACT:
This Contract is funded by Enterprise I in the Health Services
Department ' s Budget .
BACKGROUNDIREASON LS) FOR RECOMMENDATION(S) :
On April 23 , 2002, the Board of Supervisors approved Contract
#26-292-6, with West Contra Costa Transit Authority, for the period
from May 1, 2002 through April 30, 2003 , to provide non-emergency,
fixed route bus transportation services in order to transport County
patients between County Health Center in Richmond and Contra Costa
Regional Medical Center.
In accordance with the terms of the agreement, the County agrees to
indemnify and hold harmless West Contra Costa Transit Authority from
claims arising from the County' s negligence, in exchange for a
similar indemnification and hold harmless inuring to the benefit of
the County, which was previously approved by County Risk Management .
Approval of Contract #26-292-7 will allow the Contractor to continue
providing services through April 30, 2004 .

Camhi) , to execute on behalf of the County, Contract #27-291-5 with Bay Area
Retina Associates, a non-profit corporation, in an amount not to exceed
$60 , 000 , for the period from February 1, 2003 through January 31, 2004 , for
the provision of professional ophthalmology services for the Contra Costa
Health Plan..
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member
premiums . Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
BACKGROUND/REASON(u) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On April 9, 2002 , the Board of Supervisors approved Contract #27-291-4 with
Bay Area Retina Associates, for the period from February 1, 2002 through
January 31, 2003 .
Approval of Contract #27--2.91-5 will allow the Contractor to provide
professional ophthalmology services to Contra Costa Health Plan members,
through January 31, 2004 .

On June 18, 2002, the Board of Supervisors approved Contract #27---533 with Eileen Lo, O.D., for
the period from May 1, 2002 through April 30, 2003, for the provision of optician services to Health
Plan members.
Approval of Contract #27-533--1 will allow the Contractor to continue providing services through
April'30,2004.

on behalf of the County, Contract Amendment Agreement #26-452-1 with Bay Medic
Transportation,a corporation.,effective January 1,2003,to increase the total Contract Payment Limit
by $16,850 from$30,000 to a new total payment limit of$46,850.

On September 24, 2002, the Board of Supervisors approved Contract #26-452 with Bay Medic
Transportation, for the period from July 1, 2002 through June 30, 2003 to provide ambulance
transportation services to transport patients to and from Contra Costa Regional Medical Center and.
Contra Costa Health Centers to skilled nursing facilities and/or home.
At the time Contract#26-452 was negotiated, the payment limit was based upon targeted levels of
utilization. However, the utilization of services has exceeded the Department's projections.
Approval of Contract Amendment Agreement #26-452-1 will allow the Contractor to provide
additional ambulance transportation services through June 30,2003.

STATEMENT OF NEED FOR RETROACTIVE APPROVAL
On July 1, 2002, Bay Medic Transportation, Concord, CA, sgntralct ul. 26-542. began
providing nonemergency patient ambulance services. It was necessary to immediately
renew this contract to the next fiscal year to preclude interrtnptions of ambulance services
from CCRMC to Skilled Nursing Facility/patients home. Subsequent to this renewal, the
state budget deficits and deep budget reductions transpired forcing the Health Services
Department to enforce a 75%transportation budget cuts across the board. These cuts
necessitated a temporary suspension of contracts as ordered Clue primarily to lack of funds
to pay invoices. The delay is also caused by some of the invoices for December 2002 and
January 2003, being lost in tw sit and later showed up in finance for payment, The third
reason for the delay is the process of resolving the question of patients' eligibility for medi-
cal/medicare in order to save money. An oral agreement was therefore made by the MSS
Director with the contractor to minimize Interruption of transportation services to selected
patients, despite paucity of dollars.
Hence, retroactive approval of this contract amendment effectiveJam 1, 2003, is
requested in order to pay the old invoices still held by finance department, and to continue
providing appropriate transportation to deserving patients. Approval of this request will
result in a total county payment limit of $46,850.00.
The MSS Director who initiated the oral and the written contract has the legal authority to
negotiate, finalize and enter into the agreement.

On December 18, 2001, the Board of Supervisors approved Contract
#2 -385 (as amended by Contract Amendment Agreement #26-385-1)
with California Nurses Bureau, for the period from January 1, 2002
through December 31, 2002, with a payment limit of $300, 000, to
provide nursing registry services for Contra Costa, Regional.
Medical. Center. Services were requested and provided beyond the
payment limit and by the end of December 31, 2002, charges of
$310, 861. . 50 had been incurred, of which $300, 0700 had been paid and
$10, 861. 50 remains outstanding.
Due to an administrative oversight by both the County and
Contractor, use of the Contractor' s services exceeded the
authorized Limits.
The Department is requesting that the amount due to the Contractor
be 'paid. This can be accomplished by the Board of Supervisors
ratifying the actions of the County employees in obtaining
provision of nursing registry services of a value in excess of the
contract payment limit . This will create a valid obligation on
the part of the County, retroactively authorizing all payments
made by the Auditor-Controller up to now, and authorizing payment
of the balance.
