Approve and authorize the Health Services Director,', or his designee
(Jeff Smith, M.D. ) , to execute on behalf of the County, Contract #26-
315-1 with The Results Group, a partnership, in an amount not to
exceed $60, 000, for the period from March 1, 2003 through February 28,
2005, to provide Consultation and training to designated staff at
Contra Costa Regional Medical Center and Health Centers .
FISCAL IMPACT:
This contract is included in the Health Services Department Bntprprise
I budget .
REASONS FOR RECOMMENDATIONSIBACKGROUND:
Under Contract #26-315-1, Contractor will provide professional
consultation, and training to County selected staff with regard to
organization development including conflict resolution, interpersonal
communication and team building, for Contra Costa ;Regional Medical
Center and Contra Costa Health Centers, through February 28, 2005 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee,
(Jeff Smith, M.D. ) to execute on behalf of the County, Contract #26-
983-3 , with Thuy-Trang Dang, M.D. , (Specialty: Infectious Disease) ,
a self-employed individual, in amount not to exceed a total. of
$61, 752, for the period from April 1, 2003 through March 31, 2005,
to provide professional Infectious Disease services, including
clinical coverage and consultation for patients at Contra Cost
Regional Medical Center.
FISCAL IMPACT:
Funding for this Contract is included in the Department ' s Enterprise
I Budget . Cost to the County depends upon utilization. As
appropriate, patients and/or third party payors will be billed for
services .
BACKGROUND/REASON(S) FOR RECOMMENDATIONS:
On March 19, 2002, the Board of Supervisors approved Contract #26-
983-1 with Thuy-Trang Dang, M.D. , for the period from April 1, 2002
through March 31, 2003, to provide professional Infectious Disease
services, including clinical coverage and consultation for patients
at Contra Costa Regional Medical Center.
Approval of Contract #26-983-3 will allow Contractor to continue
providing services through March 31, 2005 .

Pauline Velez, M.D.

RECOMMENDATION W :
Approve and authorize the Health Services Director, or his
designee, (Jeff Smith, M.D. ) to execute on behalf of the County,
Contract Amendment Agreement #26-912'-13, with Pauline Velez, M.D. ,
a self-employed individual, (specialty: General and Vascular
Surgery) , effective April 1, 2003 , to amend Contract #26-912-12,
to increase the total Contract Payment Limit by $70, 000 from
$155, 000 to a new total payment limit of $225, 000 .
FISCAL IMPACT:
cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services.
BACKGROUND/REASON(S) FOR RECOMMENDATION W :
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and clinics.
On December 3 , 2002, the Board of Supervisors approved Contract
#26-912-12 with Pauline Velez, M.D. , for the period from January
1, 2003 through December 31, 2004, to provide professional General
and Vascular Surgery services, including, but not limited to,
consultation, on-call coverage, training, medical and/or surgical
procedures, for the Contra Costa Regional Medical Center.
Approval of Contract Amendment Agreement #26-912-13 will allow
Contractor to provide additional General Surgery on-call coverage
services through December 31, 2004 .

Approve and authorize the Health Services Director to execute on
behalf of the County, Contract Amendment Agreement #23-332-1 with
Dulin Advertising, Inc. , effective March 1, 2003, to amend Contract
#23-332, to increase the Contract payment limit by $40, 000, from
$175, 000 to anew total of $215, 000 .
FISCAL IMPACT-.
This Contract is funded by County General Fund and is included in
the Department ' s Enterprise I budget .

In order to recruit qualified individuals to fill vacant positions,
particularly in the area of direct patient care, the Department must
place advertisements in such media as newspapers and professional
journals . I
On July 16, 2002, the Board of Supervisors approved Contract #23-332
with Dulin Advertising, Inc. , for the period from July 1, 2002
through June 30, 2003, to provide advertising services including but
not limited to, providing consultation and technical assistance with
regard to media research and planning, developing and producing
camera-ready recruitment ads and arranging for insertion of ads in
appropriate newspapers, professional journals and internet web
pages .
Approval of Contract Amendment Agreement #23-332-1 will allow the
Contractor to continue to provide services through June 30, 2003 .

Approve and authorize the Health Services Director, or his designee (Lewis
Pascalli, Jr. ) to execute on behalf of the County, Contract Amendment
Agreement #22-855-2 effective December 1, 42002 , to amend Contract #22-855
(as amended by Contract Amendment Agreement #22-855-1) with Robert A. Katin,
dba Katin Engineering Consulting (Formerly: Bob Katin) , a sole proprietor,
to accurately reflect the correct business name of the Contractor with no
change to the original Contract Payment limit of $34, 700 .
FISCAL IMPACT:
This Contract is funded 100; by Industry fees .
BACKGROUND/REASON(S) FOR RECOMMEMA`I`ION(5)_:
On November 12, 2002, the Board of Supervisors approved Contract #22-855 (as
amended by Contract Amendment Agreement #22-855-1) with Bob Katin for the
period from December 1, 2002 through May 31, 2003, for consultation and
technical assistance to the Department with regard to the Industrial Safety
Ordinance (ISO) .
Due to the mutual mistake of the parties, the Contract did not accurately
reflect the correct business name of the Contractor. The Contractor' s
official business name should read Robert A. Katin, dba Katin Engineering
Consulting, instead of Bob Katin, a self-employed individual .
Approval of this Contract Amendment Agreement #22-855-2 will reform the
Contract to remedy the mutual mistake of the parties, allowing the
Contractor to continue to provide services under the correct business name
of Robert A. Katin, dba Katin Engineering Consulting.

Stress Center

RECOMMENDATIONW :
Approve and authorize the Health Services Director, or his designee (Chuck
Deutschman) to execute on behalf of the County, Contract Amendment Agreement
#74-180-1 with Family Stress Center, a non-profit Corporation, effective
July 1, 2002, to modify the Payment Provisions, with no change to the
original Contract Payment Limit in an amount not to exceed $140, 004 .
FISCAL IMPACT.
This Amendment will modify the Prevention portion of the Contract to reflect
an increase in the rate per service unit ,from $38 . 89 to $55 . 01, allowing the
Contractor to provide services to additional unduplicated high-risked youth
participants and their families .
This Contract is 100%s Federally funded.
$ACKGRfltT /REASON(S) FOR RECOMMENDATION(S) :
Can July 16, 2002, the Board of Supervisors approved Contract #74-180 with
Family Stress Center, for the period from July 1, 2002 through June 30,
2003 , to provide drug abuse prevention services in Bay Point, Concord, and
Richmond clinics .
Due to a mutual mistake of the parties, the fee schedule of Payment
Provisions, Paragraph 1 . (Payment Amounts) , Subparagraph d.; (2) of Contract
#74-180 did not accurately reflect the intent of the parties . It was the
intent of the parties to pay Contractor at a rate of $55.01 per each
Prevention unit of services, instead of $38.89 .
Approval of this Contract Amendment Agreement #74-180-1 will reform Contract
#74-180, to make the formal. Contract consistent with the oral agreement,
which was negotiated with the Contractor.
