This Mental ' Health Program will support the fallowing Board of Supervisors '
community out "Children Ready For and Succeeding inSchool" ;
"Families that are Safe, Stable, and Nurturing" ; and "'Communities that are

Expected grogram outcomes include an increase in positive social and
emotional development as measured by the Child and Adolescent. Functional
Assessment Scale (CAF'AS) and placement at discharge at a lower level of
care.

This Contract meets the social needs of County"s population in that: it
provides Juvenile Court-mandated intensive day treatment' mental i health
services, and 'medication support for SED children with emotional and

intensive day treatment program through June 30, 2002 .

NEW CONTRACT-Sunny Hills Children's Garden Group Hames "
and Day Treatment Program
STATEMENT OF NEED FOR RETROACTIVE CONTRACT
Children and Families Services has been ordered by the Juvenile Court to place a Contra Costa dependent
youth in this group home as the Judge is impressed by the program and would like to see the County use it
more frequently. We do not currently have a contract with;Sunny Hills. The youth ordered'into the program
has exceeded his 90-day stay at Rose Manning Emergency Shelter and will be placed ASAP in Sunny Hills
Children's Garden.
There are two other Contra Costa dependents already resitting at Sunny Hills and receiving day treatment
services. Since it is known that Judge Haight intends to order other children into the program, the contract is
written to include the two existing clients,the child in process of being,placed, and an extra slot for any
additional children that may require this placement. Payment will be made only for day treatment services
actually rendered; there is no provision for dedicated beds.
Retroactive approval of this contract request is required to allow Child and Family Services to comply with
a court order and to provide day treatment services to the disturbed Contra Costa dependent youth placed in
the program, as mandated by PL 94-142..
BENEFIT OF CONTRA COSTA COUNTY:
Approval of this retroactive contract request will allow provision of a stable placement and day treatment
services to Contra Costa dependent youth. It also allows Children and Family Services to comply with an
order of the Juvenile Court and allows both CFS and the Mental Health Division to respond the Court's
requests in a timely manner.
AUTHORIZATION OF SERVICES;
These services were;requested/authorized by the Children's Program Chief who is granted this authority by
the Mental Health Director,pursuant to the California Welfare and Institutions Code, under the Bronzon-
McCorquodale Act,W&I Section 5608.'

On 'December' 4 , 2001, the Board of Supervisors approved Novation
Contract #24-754-38 with Alcohol and Drug Abuse Council of Contra
Costa, Inc. , for the period .from July 1, 2001 through June 30, 2002,
for the 'provision of 'primary prevention services, including "'Friday
Fight Live and. "Club Live" in order to provide drug-free and
alcohol free alternative;. youth activities.
Due' to the 'mutual mistake of the parties, the Contract did not
accurately reflect the intent of the parties;, which was for the
Contractor to be paid at the rate of $43 . 37' for each Prevention unit
of 'service, instead of the rate of $32 . 93 stated: in the Payment
Provisions.
Approval of Contract Amendment Agreement #24-754-39 will reform the
Contract to remedy the mutual mistake of the County and Contractor,
and to :accurately reflect the intent of the parties, so that the
Contractor can be properly reimbursed for primary' prevention
services, including "Friday light Dive" and. "Club Live"

The purpose of this letter is to request a contract amendment to Contract#24-754-3..
During the process of the contract,the contractor mistakenly provided wrong figures and
he did not report the error until January 2442,when the contract had already been signed.
Although, the correct figures do"`not alter the payment limit of the contract, revisions
indicate a change in the number of units of services and the cost per unit of service.
Attached please find documentation reflecting the revisions made to the Alcohol and
Drug Abuse Council Contract#24-754-38 and paperwork requesting to amend the
contract. The new figures reflect correct numbers.

provided services to the Department with: regard to evaluation of
services and grant writing.
The Homeless Outreach Project to Encampments (HOPE) Grant from the
Federal Department of Health and Human Services, Substance Abuse and
Mental. Health Services Administrations (SAIHSA) mandates evaluation
of services provided under' the grant. Approval of Contract
#25-037-2 will allow the Contractor to provide those evaluation
services through September 30, 2002 .
