Amendment Agreement #22-428-15) with AIDS Community' Network for the period from March 1, 2001 through ' February 28, 2002 , to provide: support services for County residents with HIV disease, including case management services, benefit assurance counseling, and non-emergency: transportation services. This Contract meets the social needs of the population of the County by providing ' housing-related 'advocacy, referral, and placement services to County residents living' with HIV/AIDS to 'improve and/or maintain their health status and promote stability in their lives.
