Contra DATE: June 29 2001 Costa ------ County SUBJECT: Approval of Contract Amendment Agreement #22-611-6 with New Connections SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S) : Approve and authorize the Health Services Director or his designee (Wendel Brunner, M.D. ) , to execute on behalf of the County, Contract Amendment Agreement #22-611-6 with New Connections, effective July 1, 2001, to amend Contract #22-611-5, to increase the payment limit by $42 , 558, from $230, 692 to a new Contract Payment Limit of $273 , 250 . FISCAL IMPACT• This contract is funded 100. by Federal funds from the Ryan White CARE Act , Title I , Supplemental Grant, through an intergovernmental agreement with Alameda County, who is the Grantee of these funds . No County match is required. BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : On April 2 , 2001, the Board of Supervisors approved Contract #22-611-5, with New Connections, for the period from March 1, 2001 through February 28 , 2002, to provide support services for County residents diagnosed with HIV disease. Approval of Contract Amendment Agreement #22-611-6 will allow Contractor to provide additional services through February 28 , 2002 .
