Costa DATE: June 19, 2001 County SUBJECT: APPROVING THE FOURTH AMENDMENT TO THE CONSULTING SERVICES AGREEMENT FOR REMODELING THIRD FLOOR, PHASE II, 50 DOUGLAS DRIVE, MARTINEZ (WH351F) SPECIFIC REQUESTS OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION I. RECOMMENDATION A. APPROVE Fourth Amendment authorizing extra work under the Consulting Services Agreement dated June 22, 1999 with Interactive Resources, Inc., for architectural services for Remodeling Third Floor, Phase II, 50 Douglas Drive, Martinez for the Department of Child Support Services (formerly District Attorney Family Support). The Fourth Amendment increases the scope of services and increases the payment limit, as modified by the First, Second and Third Amendments, from $67,343.00 to $70,313.00, an increase of$2,970.00. B. AUTHORIZE the Director of General Services to execute the Fourth Amendment to the consulting services agreement and to issue written authorizations for extra work, in addition to the authorization for the Fourth Amendment, provided that the cumulative total extra cost for such additional authorizations shall not exceed $7,000.00. H. FINANCIAL IMPACT Sufficient funds are available in the Department of Child Support Services' budget to cover this increase in the Consultant's.payment limit. III. REASONS FOR RECOMMENDATIONS/BACKGROUND A. The First Amendment increased the scope of services to provide additional design for mechanical and electrical related items to meet program needs. B. The Second Amendment increased the scope of services to provide Fire Alarm System revisions necessary to interface with Phase I changes required by Fire Marshal's site inspection and current County requirements. The Third Amendment increased the scope of services to provide design revisions to combined Break Room 3009 and Conference Room 3007 into a single room and to convert Transaction Room 3002 into office space. D. The Fourth Amendment as outlined in Consultant's proposal dated May 15, 2000 increases the scope to provide additional architectural and consulting services for design changes and expanded project scope. Scope includes: a. Prepare and issue additional construction change directives. b. Modifications to door openings at private office. c. Provide solutions for correction of deficiency at corridor walls. d. Additional data terminal at private office. e. Additional site visits. File: 250-9840/A.I.1 FOURTH AMENDMENT TO CONSULTING SERVICES AGREEMENT FOR REMODELING THIRD FLOOR,PHASE 11,50 DOUGLAS DRIVE,MARTINEZ FOR THE DEPARTMENT OF CHILD SUPPORT SERVICES (FORMERLY DISTRICT ATTORNEY FAMILY SUPPORT) (WH351 F) I. Effective Date and Parties: Effective June 19, 2001, Interactive Resources, Inc.(herein called "Consultant"), a California Corporation, and the County of Contra Costa(herein called"County"),a political subdivision of the State of California,mutually agree as follows: 2. Purpose: On June 22, 1999, the parties entered into a contract entitled "Consulting Services Agreement," referred to as the "Agreement," which covers architectural services for Remodeling Third Floor, Phase 11, 50 Douglas Drive, Martinez for the Department of Child Support Services (formerly District Attorney Family Support). The Agreement has previously been amended as follows: First Amendment dated May 12, 2000, Second Amendment dated June 9,2000 and Third Amendment dated March 5,2001. The parties desire to further amend the Agreement to expand the scope of service and to increase the payment limit accordingly. 3. Amendments to Agreement: A. In the Agreement, Section 1(c),change the Payment Limit from$67,343.00 to$70,313.00,an increase of$2,970.00. This change shall apply to charges from and after May 15,2001. B. In accordance with Section 14 of the Agreement,provide the following extra services: I. Additional architectural and consulting services for design changes and expanded project scope. Scope includes: a. Prepare and issue additional construction change directives. b. Modifications to door openings at private office. C. Provide solutions for correction of deficiency at corridor walls. d. Additional data terminal at private office. C. Additional site visits. 4. Effect: Subject to the revisions made by this and any prior amendments,the Agreement shall remain in full force and effect. 5.
