Contra � DATE: June 6, 2001 - a�° Costa County SUBJECT: Approve Grant Agreement (Amendment) #28-647-1 with the Public Health Institute SPECIFIC REQUEST(S)OR RECOMMENDATION(S)a BACKGROUND AND JUSTIFICATION I RECOMMENDED ACTION: I Approve and authorize the Health Services Director or his designee .(Wendel Brunner, M.D. ) to execute, Ion behalf of the County, Grant Agreement (Amendment No. 2) #28-647-1 (Grantor' s #1000044) with the Public Health Institute, to amend Contract 1#28-647, effective April 1, 2001, to extend the term of the Agreement through August 31, 2001 and to increase the total Payment by $60, 000 from $150, 000 to a new Contract Payment Limit of $210 , 000 , for the Development and Capacity-Building Project . FISCAL IMPACT: i Approval of this Grant Agreement Amendment will increase funding from the Public Health Institute by $6;0, 000 not to exceed a total of $210, 000, to carry out the purposes specified in the Partnership for the Public ' s Health (PPH) Development and Capacity-Building Grant . This is the first year of a four-year project . No County funds are required. I REASONS FOR RECOMMENDATIONS/BACKGROUND: I The Partnership for the Public ' s Health (PPH) is a program conducted by the Public Health Institute, a ; nonprofit organization dedicated to the advancement and improvement of public health. The PPH Program is funded by The California Endowment,) a foundation whose aims include promoting fundamental improvement in health status of Californians . The purpose of the PPH Program is to create sustainable partnerships between public health departments and the communities they service . I On May 23 , 2000, the Board of Supervisors approved Grant Agreement #28-647 (Grantor ' s #1000044) with the! Public Health Institute, for the period from April 1, 2000 through March 31, 2001, to assist the Department in developing a strategic actionlplan to create an effective, responsible, and sustainable public health system in Contra Costa County. Approval of Grant Agreement (Amendment) #28-647-1, will extend the term of the Agreement through August 31, 2001, and make technical adjustments to the Scope of Work. Three certified and sealed copies of. the Board Order should be returned to the Contracts and Grants Unit .
