SUBJECT: Approve Grant Agreement #28-667 with the Public Health
Institute
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee
(Wendel Brunner, M.D. ) to execute, on behalf of the County, Grant
Agreement #28-667 (Grantor' s #1002739) with the Public Health
Institute, in an amount not to exceed $150, 000, for the period from
December 1, 2000 through March 31, 2003 , for the Monument Corridor
Asthma Project .
FINANCIAL IMPACT:
Approval of this Grant Agreement will result .in an amount not to
exceed $150 , 000 from the Public Health Institute as grantee of funds
from the California Endowment to carry out the purposes specified in
the grant . No County funds are required.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
The Monument Corridor Asthma Project will allow the Department to
conduct Regional Asthma Management and Prevention Initiative Program
(RAMP) activities to reduce the high incidence of asthma-related
hospitalizations and school absences among children living in the
target area. The RAMP Program is supported by the Public Health
Institute, a nonprofit organization dedicated to the advancement and
improvement of public health, and is funded by The California
Endowment, a foundation whose aims include promoting fundamental
improvement in the health status of Californians .
The Monument Corridor Asthma Project will interface with several other
existing County projects to avoid duplication and maximize each
program' s effectiveness . Participants will include Contra Costa
Health Plan, the Department ' s Interdepartmental Advisory Committee for
asthma services, mobile health clinics, neighborhood clinics and
Contra Costa Regional Medical Center. Coordination of services
includes identification of children with asthma; provision of clinic
and hospital pediatric services; case management; screening for health
care coverage; offering home assessments and providing asthma
management education to the students, their parents, to staff and
teachers of Cambridge and Meadowhomes schools .
Three certified and sealed copies of the Board Order should be
returned to the Contracts and Grants Unit .
