SUBJECT: APPROVE AND AUTHORIZE AN AMENDMENT TO THE CONSULTING SERVICES
AGREEMENT FOR THE RICHMOND HEALTH CENTER PLANNING STUDY
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S):
1. APPROVE and AUTHORIZE the County Administrator, or designee, to execute an Amendment to
the Consulting Services Agreement with Anshen + Allen Architects, increasing the payment limit
by $60,000 to a new payment limit of $85,000 for feasibility planning services related to the
Richmond Health Center.
2. AUTHORIZE the County Administrator, or designee, to execute the Amendment and to issue
written authorizations for extra work, provided that the cumulative total extra cost for all such
authorizations shall not exceed $25,000.
FINANCIAL IMPACT:
These Amendments could increase the payment limit in aggregate by $60,000. Adequate funds for
these Amendments are included in the plant acquisition budget for this project.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
Several months ago, the Board of Supervisors of Contra Costa County directed staff to initiate efforts
to expand and upgrade the Richmond Health Center, currently located at 100 38th Street. Anshen
and Allen, a well-respected health care architectural firm, conducted a space study forecasting the
needs of the Richmond Health Center last year. They found that given the current patient caseload in
West County, the current facility provides only 50% of the space needed to meet the greater
Richmond community's health care needs.

The Richmond Health Center provides primary, specialty, and dental care to members of the Contra
Costa Health Plan, as well as offering the community services ranging from outpatient mental health
and public health services to Healthy Start, Born Free, and the Women, Infant and Children's
programs. The Health Center also offers evening and weekend clinics for working individuals that are
very popular. In all, the Richmond Health Center receives over 100,000 visits each year, making it
one of the two largest outpatient health centers in West County.
The Health Center serves a broad cross-section of the greater Richmond community. For example,
in February 2001, there were approximately 800 visits by senior citizens who receive health coverage
through Medicare, 1,620 visits by children who receive health coverage either through MediCal or the
County Health Plan, 355 visits by individual who work for the County, small businesses, or have
otherwise chosen to receive health coverage through the County Health Plan, and 819 other
individuals who do not belong to a health plan but paid directly for services. In addition, the
Richmond Health Center there were approximately 2,000 medically indigent adult patient visits that
month.
Subsequent to the release of the Anshen and Allen study, the dehumidification system at the forty-
year-old facility ceased functioning during a hot spell last October. As a result, the Health Center had
to close for the day, which greatly inconvenienced patients and resulted in operating losses to the
County Health Services Department. Emergency repairs to the HVAC system are nearly complete,
but it is clear that the HVAC and other building systems are near the end of their useful lives. Given
the vital importance of this facility to the County's health services delivery system, the Board of
Supervisors directed staff to conduct a building evaluation study of the facility and to report back to
them by summer regarding options for the cost-effective expansion and/or upgrade of the Richmond
Health Center.
This contract with Anshen and Allen will allow staff to conduct the building evaluation study and to
develop options for the expansion and upgrade of the facility.
